I.

INTRODUCTION

Project number and title: AJXF 16-1550 Repair Taxiway Echo, Navy Taxiway & Taxiway Echo Taxiladder E6, Joint Base Andrews, MD 20762-4803.

A. Background: This project involves the partial and full depth repairs of a concrete airfield taxiway on the east side of the airfield at Joint Base Andrews. The average thickness of the existing PCC pavement is approximately 14.5 inches. It is anticipated that this work will occur along the entire run of the taxiway Echo, Navy Taxiway and Taxiway Echo Taxiladder E6.

B. Specific Requirements: Provide construction in accordance with the following:

1. The contractor shall furnish personnel, services, equipment, materials, and other requirements necessary for, or incidental to, the performance of work set forth herein.

a) Taxiway Echo North: Taxiway Echo North consists of one section (T28A) of PCC pavement composed of 250 slabs. Remove and replace approximately 3,500 LF of deteriorated joint sealant.

b) Taxiway Echo Mid: Taxiway Echo Mid consisted of three sections (T26A, T27A1, and T27A2) of PCC pavement composed of 1435 slabs. Repair spalls, patch linear cracking and replace approximately 15,000 LF of damaged Joint seal at sections T26A and T27A1.

c) Taxiway Echo South: Taxiway Echo South consists of three sections (T25A1, T25A2, and T25A3) of PCC pavement composed of 868 slabs. Replace approximately 19,800 LF of joint sealant at Section. Repair spalls, cracking and damaged segments throughout and replace slab at section T25A2.

d) Navy Apron Access Taxiway: The Navy Ramp Access Taxiway consisted of one section (T31A1) of PCC Pavement. Replace approximately 3,500 LF of joint sealant and repair cracking at damaged segments throughout.

e) Taxiway Echo Taxi-Ladder E-6: Taxiway Echo Taxi-Ladder E-6 consists of one section (T49A) of PCC pavement composed of 59 slabs. Remove and replace approximately 2,900 LF of deteriorated joint sealant.

2. Construction shall comply with all applicable federal, state, local laws, Air Force Instructions and regulations including attached Specifications 00 73 05 Special Conditions and 01 57 20.00 20 Joint Base Andrews Environmental Protection.

3. The contractor shall provide 30,000 SF of partial depth spall repair in accordance with UFC 3-270-03, Concrete Crack and Partial Depth Spall Repair and attached Specification 32 01 29.61 Partial Depth Patching of Rigid Paving. 4. The contractor shall provide 40 CY of full depth concrete repair in accordance with UFC 3-270-04, Concrete Repair. Concrete shall meet Aggregate Industries’ mix design for 650 psi flexural strength or equal mix design. The UFC can be found at website: http://www.wbdg.org/ccb/DOD/UFC/ufc_3_270_04.pdf

5. The contractor shall provide saw cutting, removal of any existing joint sealant, construction of new joint sealant reservoir for various types of joints, and provide new joint sealant for 45,000 LF of joints, which constitutes all joints on the involved area of the east ramp. In the case of reflective cracking, the joint shall be sealed prior to any concrete repair. Joint sealant shall meet attached Specification 32 01 19 Field Molded Sealants for Sealing Joints in Rigid Pavements.

6. The contractor shall provide 1,500 LF of new line striping. Line striping shall meet attached Specification 32 17 24.00 10 Pavement Markings and ETL 04-2 Standard Airfield Pavement Markings, which can be found at website: http://www.wbdg.org/ccb/AF/AFETL/etl_04_2.pdf

7. The contractor shall provide 30 CY of removal of un-suitable base and subgrade material and provide new material meeting attached Specification 32 11 23 Aggregate & Graded-Crushed Aggregate Base Course.

C. Base Access

1. Contractor Access Passes: All Contractor employees must initially be sponsored on Base for up to 5 days. The Contractor shall provide the following information to the 11 CES at least 72 hours prior to entry for sponsorship:

Name; Date of Birth; Social Security Number; Driver’s License Number & State of Issue; Vehicle License Tag Number & State of Issue; Name of Company; Date & Time of Arrival.

On the date of arrival, all personnel can enter through the Main Gate (off Allentown Road), Virginia Avenue Gate (of Old Alexandria Ferry Road) or the Pearl Harbor Gate (off Dower House Road) to enter the Base. The employees do not need to enter the Visitor’s Control Center (VCC). If any of the Contractor’s employees are expected to be on Base longer than 5 days they must schedule a time and date with the 11 CES/CENM Project Manager to obtain a Long Term Contractor’s Badge request form. Once completed by the Air Force Project Manager, the Long Term Pass Request shall be taken by the Contractor to the VCC at the Main Gate for processing. The Contractor shall have current driver’s license, insurance information and current registration of the vehicle. All non-U.S. citizens shall have documents to prove legal status. All Contractor employees applying for a security badge shall be subjected to a background check through the Nation Crime Information Center conducted by the Security Forces Squadron. These Base access procedures are subject to change.

2. Contractor Vehicle Access Requirements: All Contractor vehicles except cars or empty beds of pick-up trucks must enter through the Pearl Harbor Gate and will be subject to a search, which will include all vehicle compartments and transported containers. The Pearl Harbor Gate is open 24 hours a day 7 days a week. Contractors are advised that such searches may result in delays averaging 30 to 60 minutes depending upon the day of the week and time of day.

D. Site Access

1. Escorts: This project is on an active airfield and the contractor must be escorted at all times by a government provided escort. The contractor can only gain access to the job site on the airfield by a government provided escort and will not be allowed to access the site on their own. The contractor will remain with the escort at all times while working on the airfield. The contractor shall make requests for escorts at least 14 days in advance of work.

2. Special Requirements: Cameras, cell phones, laptops or any electronic device are not allowed inside the controlled movement area.

E. Phasing Plan for Construction Work

This project requires multiple phases. Due to mission requirements on the ramp, the contractor will only be allowed to work one area on the ramp at a time. Before moving to the next work area the previous work area must be completed and re-opened to aircraft.

F. Civil/Utility Considerations

1. The water distribution and wastewater collection systems at Joint Base Andrews are owned and operated by Terrapin Utility Services Company (Terrapins). To ensure bids/proposals meet Terrapin’s construction standards all modifications or connections to the existing water distribution and wastewater collection components identified on the drawings as owned by Terrapin Utility Services Company must be coordinated with Terrapin. II. CONSTRUCTION

A. Submittal Procedures: The contractor shall meet the requirements of attached specification 01 33 00 Submittal Procedures.

B. Preconstruction Conference: After award of the contract but prior to commencement of any work at the site, meet with the Contracting Officer to discuss and develop a mutual understanding relative to the administration of the safety program, preparation of submittals, and prosecution of the work. Subcontractors who will engage in the work shall also attend.

C. Staging: The contractor will be provided a staging area at the pre-construction meeting. The staging area will be located outside the Pathfinder fence and the contractor will not be allowed to stage equipment or material on the airfield.

D. The contractor shall provide removal, handling, hauling, and disposal of all construction debris off base.

E. The contractor shall coordinate with the 11 CES Construction Management Inspector to ensure the appropriate installation offices are contacted and to arrange access to the construction site.

F. Base Civil Engineering Work Clearance Request, AF Form 103: Prior to commencing construction work or whenever the ground surface is to be disturbed deeper than three inches (3”) or when erecting structures or operating equipment near overhead lines, the Prime Contractor performing work shall be responsible for obtaining from the 11 CES Customer Service Office (CSO) in Building 3466 and having in their possession at all times while digging, an approved AF Form 103, in accordance with JBA Regulation 91-5.

G. AF Form 103: This form is available through the 11th CES Customer Service Office (CSO), Building 3465, Room #106. The Contractor performing the digging shall submit the completed questionnaire to their 11 CES/CEPM Construction Management Representative in Building 3710. The AF Form 103 will expire 30 calendar days from the date that it was approved (Line 17a). The Contractor shall initiate renewal of the AF Form 103 to 11 CES/CEPM a minimum of 5 work days prior to each expiration date. For a renewal of the AF Form 103, all initial utility marks must be preserved and/or maintained and work areas must remain within the boundaries highlighted in the initial request.

H. Temporary Construction Waiver: ANY work on the airfield will require a temporary construction waiver. It is to be completed by the 11 CES Engineering Management Flight (11CES/CENM) and signed by the 11 Wing Commander (11WG/CC) IAW UFC 3-260-01, Appendix B, paragraph B1-2.2.1. The contractor must provide information such as a phasing plan in accordance with Appendix B, section 14 to the Construction Manager for the waiver. This waiver can take up to 60 days for approval from the date the Operations Risk Management meeting is held.

I. The Contractor shall submit an FAA Form 7460-1, Notice of Proposed Construction or Alteration, IAW UFC 3-260-01, paragraph B14-5 to the FAA. This form shall be initiated as soon as the project is awarded as the approved FAA 7460-1 MUST be included in the Temporary Construction Waiver package before it can be signed by the Wing Commander. III. CONSTRUCTION SCHEDULE:

A. The contractor shall use Gantt charts to show all details of construction of each phase of the project and submit in pdf and Primavera form 14 days prior to start of construction for approval.

IV. WAIVERS, PERMITS & SUBMITTAL APPROVALS:

A. The contractor shall allow up to 45 calendar days for Waivers, Permits & Submittal Approvals.

B. Construction activities on the airfield will not commence without an approved TACW.

V. WARRANTY:

The contractor shall provide a one (1) year warranty on labor and material in addition to manufacturer warranties.

VI. CLOSE-OUT-OF PROJECT:

A. The contractor shall schedule pre-final and final inspections of the project with the Contracting Officer. The contractor shall submit any warranty information and a completed DD Form 1354 within 10 days, which is attached.
