STATEMENT OF WORK
Replace Four UST (Underground Storage Tank) Spill Buckets.
Location Building: 1639 (13001) By: Patrick LeBorgne
WORK ORDER NUMBER: 67547
DATED: 21 February 2019
1. GENERAL INFORMATION:
This project is intended to replace four UST (Underground Storage Tank) spill buckets in accordance with all applicable National, Air Force, State and local building codes and regulations. This work will consist of supplying/furnishing all labor, materials, tools, equipment, shop drawings, and expenses necessary to accomplish the work described below in compliance with the Commonwealth of Massachusetts Building Code and Massachusetts General Law requirements, Unified Facilities Criteria (UFC) state and federal codes, base standards and manufacturer’s instructions. Where there is a conflict between nationally recognized codes and local codes, the most stringent shall apply. Effective immediately: Due to safety concerns the use of manhole fixed ladders on Hanscom AFB is prohibited for the purpose of entry and egress is prohibited. An alternative means is authorized (i.e. appropriate portable ladder/personnel lowering device).
2. SCOPE: Replace UST (Underground Storage Tank) spill buckets.
Prior to the start of any work, all work must be coordinated with Base Civil Engineering.
Contractor must adhere to the Hanscom Air Force Base Contractor Environmental Guide.
A Dig Safe Permit is required.
All work must be performed IAW 310 CMR 80.21 (Requirements for Spill Buckets and
Overfill Prevention Equipment).
Contractor shall provide all necessary dumpsters for demolition debris and will be responsible for their removal.
Contractor shall install all necessary safety barricades to isolate the work area.
A site visit is recommended.
Field measurements are required.
Contractor shall be responsible for all work required to complete this job.
Contractor shall provide a warranty for materials and labor.
Work will include but will not be limited to:
o Cleaning the four existing spill buckets and removing oil containing water and debris.
o Removing concrete as required around the four existing spill buckets. This will include but will not be limited to saw cutting, using a demolition jackhammer, sledgehammer, etc.
o Excavating (manually) to expose the four existing spill buckets.
o Disconnecting the four existing spill buckets from the existing risers.
o Removing and properly disposing of the four existing spill buckets.
o Excavating (manually) surrounding the four existing fill pipes, exposing the top of each tank.
o Installing four new 5 gallon below grade spill buckets.
o Backfilling previously excavated area.
o Drilling the saw cut edges of the existing concrete slab (centered along the thickness of the existing slab) and installing #4 rebar pins as required to tie the newly poured concrete slab to the existing concrete slab.
o Installing #4 rebar as required (centered along the thickness of the new slab).
o Pouring new concrete slab (matching the existing thickness and existing finish).
The new concrete shall be finished even with the level with the existing concrete and shall be pitched for water runoff.
o Performing a hydrostatic test on the four new spill buckets ensuring that the four new spill buckets pass the tightness test and are liquid tight.
3. GENERAL REQUIREMENTS:
Work includes, but is not limited to the following:
a. Provide resolutions of any technical problems that arise during the construction.
b. Should any change in scope be required, it must be coordinated with the Project POC and approved as a change order prior to performing the change in work.
c. Provide updates to Project POC. Any construction issues, changes to schedule, or schedule slippages with justification must be reported at the periodic updates.
d. The Contractor is solely responsible for compliance with the Occupational Safety and
Health Act of 1974 (OSHA) and the protection of their employees and all sub-contractor employees. In performing work under this contract on a Government installation, the contractor shall report any accidents involving Air Force property/equipment damage, or contractor personnel injuries occurring on the job or an accident resulting in death, injury, occupational disease, or adverse environmental impact to the contracting officer within five days.
e. Any modifications and addition on a facility that contains Lightning Protections System (LPS) This includes paint projects, roofing projects, projects requiring roof penetrations, installation of new HVAC or other metallic equipment, antenna installation, or other work.
Additions and modifications to the facility envelope require re-evaluation of the LPS for the completed facility as a whole. The LPS required by the addition or modification shall not be a simple addition to the LPS. The LPS must be considered a single entity and must comply with requirements of a single LPS upon completion. The LPS shall be inspected by a commercial, third-party inspector whose sole work is lightning protection, and shall be certified by this third-party inspector as compliant with AFI 32-1065 and NFPA 780, in that priority order. Projects calling for an LPS addition to an existing LPS project shall consider the configuration of the final LPS in the initial design. Projects of this type shall ensure the final LPS as a whole is compliant with AFI 32-1065 and NFPA 780, in that priority order. The same third-party inspector requirements shall apply.
f. Complete the “builder data inventory and assessment sheet” for newly installed assets, ensuring the bar code number is attached to the new equipment. Transfer bar codes from any removed equipment to the builder assessment ½ sheet form
g. Provide three (3) copies of O & M manuals and warranty letter
h. Period of Performance 60 days after award of contract
