PERFORMANCE WORK STATEMENT (PWS)
FOR
AUDIO VISUAL SYSTEM REPAIR
7TH SFG(A)
EGLIN AIR FORCE BASE, FLORIDA

1. Purpose
The 7th Special Forces Group (Airborne) requires a repair and maintenance contract for an Audio Visual System for Liberty Chapel located on the 7th SFG (A) compound. The purpose of this contract is to ensure the complete Audio and Visual System at Liberty Chapel is operational to ensure the spiritual resiliency of the Soldiers of 7th SFG (A) are operating at high levels. The exercise of the freedom of religion is a Title 10 right for all Soldiers, and this AV system is necessary to support their rights at Liberty Chapel.
2. Scope
The Contractor will provide Maintenance, Repair & Replacement, and Training on the Liberty Chapel Audio and Visual System as defined in 3.1 General Requirements. The AV system requires occasional service for operation and maintenance in support of all 7th SFG (A) Soldiers & Families.
3. Place of Performance The 7SFG (A) Liberty Chapel is located at Bldg. 4475 77th Special Forces Way, Eglin AFB, FL 32542.
4. Requirement/Description of Services
The Contractor will provide all tools, supplies, and labor necessary to repair and maintain the complete Liberty Chapel Audio and Visual System. The end product of services will be that Liberty Chapel AV System has been properly maintained and repaired, and Unit Ministry Teams of 7th SFG (A) are properly trained on the equipment to troubleshoot any problems that may affect the system, in turn affecting ministry to soldiers and families of 7th SFG (A).
5. General Requirements
Contractor will provide troubleshooting of the following Audio Visual equipment at Liberty Chapel.
- Yamaha LS9-32 Soundboard
- Biamp Audio Flex CobraNet Module
- Christie LW401 LCD Projectors
- Crestron CP3 Control system
- Crestron DGE-2 Digital graphics Engine
- Crestron V15 15" Touch Screen Display
- Crestron V24 24" Touch Screen Display
- Drake DSE24 HD Modulator w/ HDMI
- Extron 60-440-01 Audio Mini Distribution
- Extron 60-742-01 ISS Switcher
- JVC SR-HD125US Blu-Ray Video recorder
The Contractor will remove and re-install any defective parts of the audio or video portion of equipment. The Contractor will need to provide a minimum of 2 on-site service calls during a period of 12 months to ensure the system is functioning properly, and that any issues found during their checks are resolved. The Contractor shall also provide at least 1 Emergency on-site service request within the 12 months period if needed within 24 hours of request. The Contractor shall provide training on the entire Chapel Audio Visual system to all Unit Ministry Teams within 7th SFG (A). A minimum of 16 hours of phone support will need to be provided from the Contractor. Contractor will provide support for non-emergency service call within 72 hours of request.
5.1 Hardware, Tools, System Maintenance
All hardware provided in support of solutions under this contract shall include all software and associated hardware required for operations (such as controllers, connectors, cables, drivers, adapters, etc.). Unless specified otherwise in the PWS, the contractor shall provide all supplies, parts, tools and test equipment required for maintenance of the system and be responsible for total system maintenance. All tools and test equipment shall remain the property of the contractor.
5.2 Performance Objectives
5.2.1 Overall Outcome:
1. Liberty Chapel installed AV equipment operating as originally intended to operate when installed.
5.2.2 Specific Outcomes:
1. Ensure maintenance, repairs, replacement of defective parts, and training are conducted on AV
2. Provide satisfactory product repairs or advance replacement, as appropriate.
3. Deliver proper services as outlined in contract. Conduct monthly inspections to ensure equipment is functioning and conduct training to ensure government personnel can operate all equipment effectively.
5.2.3 Tolerance: 98% of the time
5.3 Services Summary
Performance of a service shall be evaluated to determine whether or not it meets the performance requirements of this contract Re-performance of unacceptable services at no additional cost is the preferred course of action when appropriate. In the case of issued Corrective Action Reports (CARs), the Contracting Officer shall take appropriate measures according to the “Inspection of Services” clause.
5.4 Management
The contractor shall establish and provide a qualified workforce capable of performing the required tasks. The workforce may include a project manager who will oversee all aspects of the PWS. The contractor shall use key performance parameters to monitor work performance, measure results, ensure delivery of contracted product deliverables and services, support management and decision-making and facilitate communications. The contractor shall identify risks, resolve problems and verify effectiveness of corrective actions. The contractor shall institute and maintain a process that ensures problems and action items discussed with the government are tracked through resolution and shall provide timely status reporting. Results of contractor actions taken to improve performance should be tracked and lessons learned incorporated into applicable processes. The contractor shall establish and maintain a documented set of disciplined, mature and continuously improving processes for administering all contract and PWS efforts with an emphasis on cost-efficiency, schedule, performance, responsiveness and consistently high-quality delivery.
5.5 Government Oversight of Work
A government representative (uniformed service member) who works in the Liberty Chapel on a normal basis will be responsible for supervision of the contractor/s while they are on government property at Eglin AFB. They will ensure the contractor/s have access to the Chapel for work calls/installation/repairs.
5.6 On-Site Task Approval Process
The contractor shall, notify the on-site COR in writing before a requirements analysis/conceptual design visit, site survey and other on-site tasks are to be performed. The following information must be provided: Names of Employees, SSAN, Security Clearance, Location, Project Number, On/About Date Planned for On-Site Work, Anticipated Duration of Visit, Support Required.
6. Government Furnished Property and Services The Government will provide, without cost to the Contractor, the facilities, Water and electricity will be furnished to the Contractor by the Government from existing systems outlets at no cost to the Contractor.
The Contractor shall manage Government furnished property in accordance with FAR 52.245-1 Alternate I Government Property (Fixed-Price Contracts). At the close of each work period, Government facilities, property, and materials shall be secured.
6.1 Facilities
No smoking is allowed in Government facilities. The Contractor shall return the facilities to the Government in the same condition as received, fair wear and tear and approved modifications excepted. Keep work areas clean. Storage areas shall be kept in an orderly and professional manner to facilitate use and minimize loss or damage. The Government will furnish or make available facilities described in 1.3. Place of Performance.
6.2 Hazardous Material
Comply with base Hazardous Material (HAZMAT) regulations for all issues of hazardous material, and ensure storage procedures meet all federal, state, and local requirements. Prior to use on Eglin AFB, the Contractor shall provide the Base Hazardous Material Pharmacy, through the contracting officer, a list of all hazardous materials the Contractor brings onto Government property and shall provide a Material Safety Data Sheet (MSDS) for each Hazardous Chemical listed in OSHA Hazard Communication Standard 29 CFR 1910.1200 in accordance with Eglin AFB Hazardous Material Management.
6.3 Government-Furnished Services
Government-furnished services include services provided by Government activities as well as contracted services.
6.4 Utilities
The Government shall furnish utility services including water, sewer, electric, and heating for Contractor use in performing this contract. The Contractor shall ensure employees conserve utilities.
6.5 Telephone
The Contractor shall be allowed the use of the telephone for contacting the Contracting Officer, Civil Engineer, Fire Department, Security Forces, receiving customer complaints and any other directly contract related communications.
6.6 Communication Security (COMSEC)
All DoD communications are subject to COMSEC review. Use of DoD telephones and telephone systems constitutes consent to COMSEC monitoring. The contractor shall comply with EGLIN AFB COMSEC procedures and rules. For copies of these documents please contact Jose Torres at 310-653-0027 or 3845.
6.7 Refuse Collection
Waste generated by the Contractor in performance of the services specified with this PWS shall be deposited in Government dumpsters, excluding hazardous materials in which proper disposal instructions shall be complied with. The Contractor shall comply with base recycling regulations.
6.8 Emergency Medical Service
Emergency medical treatment and emergency patient transportation service is only available through the local community.
6.9 Security Forces
The Government will provide general security service. Security Forces phone extensions are 911 for emergencies, and 363-2121 for routine calls.
7. General Information
7.1 Contractor Personnel
Contract Manager The Contractor shall appoint a Program Manager (PM) to provide daily supervision and quality control, and who shall represent the Contractor and be responsible for the performance of the work throughout the term of the contract. The name of this person, and an alternate(s) who shall act for the Contractor when the manager is absent, shall be designated in writing to the CO and the Technical Point of Contact (TPOC). The Program Manager (PM) shall be able to speak, read, write, and understand English. The PM shall be able to effectively communicate with Contractor employees.
7.2 Contractor Employees
The Government reserves the right to restrict the employment, under this contract, of any Contractor employee, or prospective Contractor employee who is identified as a potential threat to health, safety, security, general well-being, or the operational mission of the installation and its population. Contractor shall perform background checks, at Contractor's expense, to ensure that said employee has not been convicted of any sexual crime or any crime against children.
7.3 Contractor Manpower Reporting Application (eCMRA)
The Contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract via the secure data collection site (i.e. Contract Manpower Reporting Application). The contractor is required to completely fill in all required data fields using the following web address http://www.ecmra.mil. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year. Contractors may direct questions to the above eCMRA help desk. Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data. Data for Air Force service requirements must be input at the Air Force CMRA link. However, user manuals for government personnel and contractors are available at the CMRA link at http://www.ecmra.mil.
7.4 Employee Certifications
The Contractor shall make sure employees have the following current and valid professional certifications before starting work under this contract: The ability to troubleshoot/repair/replace all equipment items listed in section 3.1 (General Requirements).
7.5 Common Access
The Contractor’s PM or alternate shall complete all necessary documents for all Contractor personnel requiring access to Eglin AFB. The Contractor’s PM shall ensure the Government representative in the local organization receives a Visitor Request for each employee through the Trusted Agent Security System (TASS). Contractor personnel shall electronically submit application to the TA, once approved set up appointment at https://rapids-appointments.dmdc.osd.mil/ or go to the Eglin Military Personnel Section located in Bldg 210 to receive their CAC.
7.6 Government Security Instructions
The Government official designated as having security supervision over all security aspects of this contract. Any other security requirement not specifically defined in this contract or the Industrial Security Manual will be provided by the SMC Director of Security by written instructions to the Contractor via a Contractor Visitor Group Security Agreement in accordance with paragraph 1-108e, DOD Directive 5220.22R.
7.7 Parking
Contractor employees are not guaranteed to be able to park on EGLIN AFB proper. In order to park at EGLIN AFB, all Contractor employees must have a valid EGLIN AFB Mission Essential (ME) or lottery parking pass. ME passes are distributed by the 61 CELS/CC and are valid indefinitely, unless the pass is reassigned by the 61 CELS/CC.
7.8 Physical Security
The Contractor shall be responsible for safeguarding all Government property provided for Contractor use. At the close of each work period, Government facilities, equipment, and materials shall be secured.
7.9 Dress and Appearance
Contractor personnel shall present a clean, neat and professional appearance at all times and be easily recognized as contract employees. This may be accomplished by wearing a Contractor provided badge depicting company name, employee’s name, and title. Each employee shall wear the badge on the outer clothing on the front of the body between the neck and waist so the badge is visible at all times. No item of military clothing shall be worn as an outer garment.
7.10 Employment of Government Employees
The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest, or the appearance of a conflict of interest, nor shall the Contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives proper approval. The Contractor is cautioned that off-duty active military personnel hired under this contract may be subject to permanent change of station (PCS), change in duty hours, or deployment. Military reservists and National Guard members may be subject to recall to active duty. Their absence at any time shall not constitute an excuse for nonperformance under this contract. The Contractor is prohibited from employing off-duty TPOCs who are managing any contracts or subcontracts awarded to the Contractor.
7.11 Hours of Operation
Normal Hours of Operation Normal Duty hours for EAFB are 0600-1900 Monday through Friday, except federal holidays. The contractor may work, with prior approval, extended hours to ensure timely completion of work at no additional cost to the government.
7.12 Federal Holidays
The Contractor is not required to provide services on the following federal holidays:
New Year’s Day Martin Luther King’s Birthday Presidents Day Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Day Christmas Day
If the federal holiday falls on a Saturday or Sunday, it may be observed on Friday or Monday as directed.
7.13 Emergency or Special Event Services
Performance of Services During Crisis Declared by the National Command Authority or Overseas Combatant Commander
(IF ANY OR ALL OF THE SERVICES TO BE PERFORMED UNDER THE CONTRACT HAVE
BEEN DETERMINED TO BE ESSENTIAL FOR PERFORMANCE DURING CRISIS AND,
ACCORDING TO DEPARTMENT OF DEFENSE INSTRUCTION (DODI) 1100.22 AND AIR
FORCE IMPLEMENTATION THEREOF, IT IS DETERMINED THAT THE CONTRACTOR
WILL BE REQUIRED TO PERFORM DURING CRISIS, SPECIFY THE HOURS OF
OPERATION REQUIRED AND THE PROCEDURES FOR NOTIFICATION TO THE
CONTRACTOR THAT SUCH DUTY HOURS ARE IN EFFECT. SPECIFIC CONTRACT
REQUIREMENTS ARE PROVIDED IN SECTION C-5 AND SHOULD BE CROSS-
REFERENCED HERE.)
In Accordance With DoDI 1100.22, (12 Apr 2010) Continuation of Essential DoD Contractor Services During Crises, DFARS 237.76, and the Air Force implementation thereof, unless otherwise directed by an authorized government representative, it is determined that Repair and Maintenance services as identified in this PWS are not essential for performance during a crisis.
7.14 Inspection Files
The Contractor shall electronically maintain records to document inspections, corrective or preventative actions taken, and the results of such actions.
7.15 Quality Assurance
The Contractor shall be responsible for quality assurance. The assigned government inspector(s) shall evaluate and approve all work before and after completion.
7.16 Discrepancy Notification
The Technical Point of Contact (TPOC) will inform the Contractor PM (or authorized representative) when discrepancies occur and will request corrective action. The TPOC will make a notation of the discrepancy on their surveillance checklist with the date and time the discrepancy was noted and will request the Contractor PM (or authorized representative) to initial the entry on the checklist.
7.17 Scope Changes
Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the Contracting Officer and not the Technical Point of Contact (TPOC).
7.18 Administrative Records
All records, documents, and associated papers provided by the Government and generated during the period of this contract become Government property and will remain in place upon contract termination or completion. During the period of the contract, records, documents, and associated papers shall be available for review.
7.19 Safety
It is the contractor's responsibility to ensure its employees and managers have a comprehensive understanding of and full compliance with OSHA requirements. Detailed information is available on the OSHA website at http://www.osha.gov. It is the contractor's sole responsibility for compliance with OSHA, Public Law 91-596. The contractor shall comply with all federal, state and local laws, and Air Force Instructions for the protection of their employees while working on an Air Force Installation or property. The protection of contractor employees is exclusively the obligation of the contractor. Contractor shall manage all work areas to ensure the safety of building occupants, base personnel, or visitors in or near the areas. The contractor shall submit a Safety and Health Plan that describes the capabilities and processes to ensure compliance with safety and health requirements during the performance of the work described in the solicitation. The contractor's plan shall include appropriate measures to ensure the contractor reacts promptly to investigate, correct and track alleged safety and health violations and/or uncontrolled hazards in contractor work areas. Contractor must ensure that personnel are properly trained on the equipment they are tasked to operate. The contractor shall develop site specific mishap reporting procedures which result in damage to government property or injury to Air Force personnel. These procedures are to be included in the safety and health plan for review. In accordance with AFI 91-202, AFMC Supplement, Safety Program, the contractor shall submit a Safety and Health Plan to Contracting for review and acceptance by the safety office prior to contract award.
