Description of Service The contractor shall provide all personnel, equipment, tools, materials, supervision necessary to provide laundry service for Fort MacArthur Inn Lodging Facility located in San Pedro, CA in accordance with all applicable local, state, and federal laws, regulations, standards, instructions, commercial practices, and as described in this Performance of Work Statement (PWS). The contractor is encouraged and expected to use innovative approaches to efficiently and effectively accomplish PWS requirements in a timely manner and in a way that fosters pride and ownership in the work performed. Contractor will pick-up soiled linen, clean and return linen to Fort MacArthur Inn. The lack of an invoice will be grounds for non-payment. The contractor must inventory upon pickup and provide the Contracting Officer Representative COR) or other authorized Government representative with a copy. Estimated workload factors are in Appendix A. These amounts are based on historical records and are not guaranteed to be accurate or current count.

1.1. Basic Services

The contractor shall provide laundry cleaning service to include: pick up and deliver of clean finished linen to Ft. MacArthur Lodging Facility, Bldg 37. Service calls shall be made during the established schedule made by the Government in “1.2.2. Delivery and Pick-Up Schedule.”

1.1.1. Pick-Up and Delivery

1.1.1.1. Pick-Up

The Contractor shall provide pick up service two times each week on Tuesdays and Fridays. Pick-ups must be within the designated time provided by the Government. All Linen carts must be clean and wash after each pickup. Counting of articles, including the laundry bag will be done by authorized Government personnel, prior to pickup. The laundry that the contractor shall pick up will be dry/wet soiled linen. The contractor shall verify the count and sign a pick up ticket. Such signature constitutes agreement to the Government’s Count unless specific exception is taken.

A copy of the pick-up slip will be retained by the government.

1.1.1.2. Delivery

The Contractor shall deliver all items are subject to inspection to ensure proper cleaning and handling of soiled materials in accordance with state and federal regulations. Delivered finished articles shall be sanitized, free of stains, free of dust, neatly packaged, and subject to inspection. The delivered finished articles shall match the quantities recorded for pick-up.

The contractor shall record number of cleaned pieces delivered on the delivery ticket and the Government representative will verify this amount. Items delivered will be within 98 percent of items picked up plus IOUs from Previous deliveries. Contractor shall provide IOU for items not delivered on schedule. The laundry that the contractor does deliver shall be clean, dry and wrapped. The contractor shall provide the tickets and furnish the government the original and one copy.

1.1.1.3. Inventory Reconciliation

The contractor shall provide completed laundry slip to the linen exchange personnel with each delivery 100% of the time.

1.1.1.4. Linen Packaging

The contractor shall provide complete linen bundles that are shrink-wrapped. Method of finish shall be wash, dry, finish (WDF) Press (P), and wrap finished work in plastic (WP) in accordance with Appendix D.

1.1.2. Service Calls

Service calls will occur when there is a possible discrepancy in the quantities that were delivered. The contractor shall respond to calls within four (4) hours. Service calls will be made between the hours of 0830-1200, and the Contractor will be required to deliver the correct quantities to correct the deficiencies amounts.

1.2.

Hours of Operation

1.2.1. Delivery and Pick-Up Location

The contractor shall deliver and pick up laundry at Building 40 located at Ft. MacArthur facility, San Pedro, California.

1.2.2. Delivery and Pick-Up Schedule

The contractor shall deliver and pick up laundry between the hours of 8:30 A.M. and 10:45 A.M. only unless otherwise stated or a special order is to be delivered at a designated time agreed upon by the activity management and the contractor or his/her representative.

1.3.

Repairs and Serviceability

1.3.1. Unserviceable Articles

The contractor will deliver any unserviceable and un-repairable articles wrapped and marked as damaged or soiled. Allowance standard of (2%) for unserviceable items.

1.3.2. Inventory Reconciliation

The contractor will conduct a reconciliation of their inventory within 24 hours (what was delivered compared to what was picked up) on a weekly basis in person.

1.3.3. Vehicle Interior Requirements

Clean items are to be protected from contamination and soiling at all times.

1.3.4. State and Local Department of Health Requirements

Compliance with any certification of labeling requirement arising from State or local Department of Health Regulations is the responsibility of the contractor. (OSHA)

1.4.

Workmanship and Sanitation

1.4.1. Sanitation

Contractor delivered finished articles shall conform to the standards of quality of cleanliness. All work performed shall be done under sanitary conditions. The items shall not only be clean, in the sense of being free from soil and stains, but also free from bacteria, fungi and other microorganisms, including pathogens, or disease producing organisms. 98% of laundry bundles shall be delivered clean, soil free, and guaranteed to be accurate or current; 2% for unserviceable items is allowed standard.

1.4.2. Re-Performance of Laundering

Notwithstanding the provision of this contract covering inspections, acceptance and deduction from payment; any items found to have been unsatisfactorily cleaned shall be re-cleaned at no additional cost to the government. Such items shall be identified by COR after the unsatisfactory condition is discovered. The contractor shall pack these items separately and identified by marking: “ATTENTION COR.”

1.4.3. Cart Exchange Service

Contractor must insure that there are two clean and serviceable laundry carts or bins available for use at all times to hold laundry located in Bldg 40.

Services Summary The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.

Government-Furnished Property and Services The Government shall not provide facilities, equipment, materials or services to perform laundry services. Fort MacArthur Inn will provide all linen.

4.

General Information

4.1. Personnel

4.1.1. Contract Manager

The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person and alternate(s), who shall act for the contractor when the manager is absent, shall be designated in writing to the contracting officer.

4.1.1.1. Authority

The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract.

4.1.1.2. Availability

The contract manager or alternate shall be available during normal duty hours 0730-1630 to meet within two (2) hours on the installation with government personnel (designated by the contracting officer) to discuss problem areas. After normal duty hours the manager or designated alternate shall be available within 24 hours.

4.1.1.3. English Proficiency

The contract manager and alternate(s) must be able to read, write and understand English.

4.2. Employees

4.2.1. Attire and Badges

Contractor delivery personnel shall present a neat appearance and be easily recognized. Wearing distinctive clothing bearing the name of the company or wearing appropriate badges that contain the company name and employee name may accomplish this.

4.2.2. Conflict of Interest

The contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest nor shall the contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval.

4.2.3. Military Personnel

The contractor is cautioned that off duty active military personnel hired under this contract may be subject to permanent change of station, change in duty hours, or deployment. Military Reservists and National Guard members may be subject to recall to active duty. The abrupt absence of these personnel could adversely affect the contractor’s ability to perform. Their absence at any time shall not constitute an excuse of non-performance under this contract.

4.2.4. Passes and Identification

The contractor shall obtain and identification items from pass and identification office and vehicle pass for personnel vehicle requiring access to Los Angeles AFB.

4.3. Quality Control

4.3.1. Quality Control Plan

The contractor shall establish and maintain a complete Quality Control Plan to assure the requirement of the contract is provided as specified. One copy of the contractor’s Quality Control Plan shall be provided to the contracting officer on contract start date and as changes occur.

4.3.2. Service Delivery Summary

The Plan shall include an inspection system covering all services listed on the Service Delivery Summary. List must specify the areas to be inspected on either a scheduled or unscheduled basis, how often inspection will be accomplished, or the title of the individual(s) who will perform the inspection.

4.3.3. Acceptability

The methods of identifying and preventing defects in the quality of service performed before the level of performance becomes unacceptable.

4.3.4. Inspections

On-site records of all inspections conducted by the contractor and the necessary corrective action taken shall be made available to the government during the term of the contract.

4.4. Quality Assurance

The government will evaluate the contractor’s performance under this contract using the method of surveillance specified in the QASP. The government will record all surveillance observations. When an observation indicates defective performance, the COR will request the contractor’s representative to initial the observation.

4.5. Performance Evaluation Meetings

The contract manager may be required to meet at least weekly with Contracting Officers Repetitive (COR) and the contracting officer during the first month of the contract. Meetings will be as often as necessary, thereafter, as determined by the contracting officer. However, if the contractor requests a meeting, it will be held whenever a Corrective Action Report (CAR) is issued. The written minutes of these meetings shall be signed by the contractor’s manager; he/she shall submit a letter of non-concurrence within five days of receipt of the minutes.

APPENDIX B

Definitions 1.

Standard Definitions

1.1. Defective Service

A service output that does not meet the standard of performance specified in the contract for that service.

1.2. Lot

The total number of service outputs in a surveillance period, as defined in the

Performance Objective column of the SS.

1.3. Performance Threshold

The point that divides acceptable and unacceptable performance of an outcome-based requirement.

1.4. Service Summary

Identifies the key service outputs of the contract that will be evaluated by the

Government to assure contract performance standards are met by the contractor.

1.5. Quality Assurance

Those actions by the government to assure services meet the requirements of the

Performance Work Statement (PWS).

1.6. Performance Work Statement (PWS)

An organized written document used for quality assurance. The document contains specific methods to perform surveillance of the contractor.

1.7. Quality Control

Those actions taken by the contractor to control the performance of services so that they meet the requirements of the PWS.

1.8. Random Sampling

A sampling method in which each service output in a lot has an equal chance of being selected.

1.9. Sample

A sample consists of one or more service outputs drawn from a lot. The number of outputs in the sample is the sample size.

2.

Technical Definitions Relevant to the PWS

2.1. Laundering

The processes by which soiled fabric items are treated and cleaned include cleaning by mechanical action or by hand in water solutions of detergents and other additives. For the purpose of the contract, the term includes replant treating, non chlorine bleaching, bluing, chemical rinse (scouring, starching, ironing and pressing operations as appropriate to the type and nature of soil and other agent for controlling the growth of microorganisms such as molds and bacteria.)

2.2. Repairable Items

The articles with minor (less than four inches) rips, tears and open seams, holes, missing buttons and broken zippers.

2.3. Unserviceable Items

No longer usable or repairable for purpose intended.

2.4. Clean

Finished articles are dry, free of visible soil, lint and objectionable odors.

2.5. Hot Water Washing

Linen should be washed with a detergent in water hotter than 160 degrees Fahrenheit for more than twenty-five minutes, since this is an effective method for cleaning and for killing vegetative bacteria.

2.6. Exchange Cart System

A system by which clean and serviceable laundry carts are available for use within the activity at all times to hold laundry.

APPENDIX C

Applicable Publications and Forms Publications and forms applicable to the Performance Work Statement are listed below. The publications have been coded as mandatory or advisory. The contractor is obligated to follow those publications coded as mandatory and shall be guided by those coded advisory to the extent necessary to accomplish the requirements of this PWS. The publications are mandatory to the extent (that is a specific procedure, in a paragraph, section chapter or volume) specified in the PWS. All publications and forms listed will be made available by the government at the start of the contract. The government will maintain the publications. Supplements and amendments to the publications from any organizational level may be issued during the life of the contract. Changes in the contract price due to supplements and amendments shall be considered under the “changes” clause. The contractor shall immediately implement those changes in publications, which result in a decrease, or no change in contract price. Prior to implementation any such revision, supplement, or amendment that will result in an increase in contract price, the contractor shall submit to the Contracting Officer a price proposal. Said proposal shall be submitted within 30 days from the date the contractor receives notice of the revision, supplement or amendment giving rise to the increase in cost of the performance. Failure to submit the proposal as provided above, shall entitle the government to perform in accordance with such change at no increase in price.
