1.

Description of Service

The Contractor shall furnish all personnel, equipment, labor, tools, materials, supervision, transportation and other items and services necessary to perform instructor services for the Child Development Center, Los Angeles Air Force Base (LAAFB), El Segundo, California and at the LAAFB Youth Center at Fort MacArthur, San Pedro, California.

1.1.

Basic Services

The Contractor shall plan and lead music activities for the Child Development Center (CDC), Los Angeles Air Force Base (LAAFB), El Segundo, California and at the LAAFB Youth Center at Ft. MacArthur, San Pedro, California.

1.2.

Programs 1.2.1.

Child Development Center Group Music Sessions The Contractor shall lead musical songs, instrumental music making, movement, and musical games for pre-school aged classes at the Child Development Center. Defining movement for youth lessons means to add hand gestures and body motions to accommodate the songs being taught.

1.2.2.

Fort MacArthur Youth Center Guitar Lessons

The Contractor shall assist musicians (focus on teens, all ages welcome) in developing skills for performing and improvising on guitar or other instruments. The Contractor shall teach children the intermediate skills of beginning guitar lessons. This class shall be geared towards teens. If space is available, The Contractor will fill the class with children of other ages appropriate for the instruction.

1.2.3.

Summer Youth Center Day Camp Music Hour

The Contractor shall lead musical songs, instrumental music making, movement, and musical games for the elementary school aged students at the Child Development Center Day Camp program. Instrumental music making is defined as construction of instruments by crafts such as a noise maker. The Youth Programs will provide the materials. The Contractor will decide on the activity and lead the lesson. The children will make individual pieces of craft sound makers.

1.3.

Schedules 1.3.1.

Child Development Center Group Music Sessions Schedule

The Contractor shall provide five (5) hours of classes including transition time (i.e., the time of moving the children from their assigned classroom to the area where the music classes will be taught). between 08:30 hrs and 11:00 on Thursday for an average of three (3) weeks per month for a total of one hundred eighty (180) hours. The Contractor must complete one hundred eighty (180) hours of instruction. If an emergency arises and the Contractor has to reschedule or the facility is closed, the Contractor can reschedule to make up classes to meet the one hundred eighty (180) hour commitment. An amplifier has been purchased. In the event there is complications with the amplifier, purchase of a replacement will depend upon available funds at the facility.

1.3.2. Fort Mac Author Youth Center Guitar Lessons Schedule

The Contractor shall lead music students in sessions from 1400 hrs to 1600 hrs on an average of three (3) Tuesday afternoons per month for a total of seventy-two (72) hours.

1.3.3. Summer Youth Center Day Camp Music Hour Schedule

The Contractor shall provide one (1) hour of musical activities for the elementary age group between 13:00 and 14:00 for three (3) Thursdays in July and August, 2012 for a total of six (6) hours. Sessions may be rescheduled as necessary.

2.

Service Delivery Summary 2.1.

General The Contractor service requirements are summarized into performance objectives that relate directly to mission essential items. Unless indicated elsewhere, these performance objectives will be measured monthly. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.

2.2.

Performance Evaluation

Performance of a service shall be evaluated to determine whether or not it meets the performance requirements of this contract. Re-performance of unacceptable services at no additional cost is the preferred course of action when appropriate. In the case of CARs issued as the result of other methods of surveillance, the CO shall take appropriate measures according to the “Inspection of Services” clause.

3.

Government Furnished Property and Services

The Government will provide, without cost to the Contractor, the facilities, equipment, materials, and services listed below and in Appendix B. However please note the following:

· Child Development Center : Music sessions will be held in Child Development Center classrooms and common area. Rhythm instrument purchases have been proposed. Contractor purchased equipment (personally owned) would greatly enhance the program.

· Youth Center Guitar Lessons: Workshop will be held at Youth Center facilities. Individuals will provide their own instruments.

· Youth Center Elementary Music Hour. Day Camp Music Hour will be held at Youth Center facilities.

3.1.

Facilities

No smoking is allowed in Government facilities. The Contractor shall leave the facility in the same condition (or better) as received. The Contractor or its employees are prohibited from making alterations to any facility. When storing equipment, storage areas shall be kept in an orderly and professional manner to facilitate use and minimize loss or damage of equipment.

3.1.1.

Hazardous Material

The Contractor shall comply with base Hazardous Material (HAZMAT) regulations for all issues of hazardous material, and ensure storage procedures meet all federal, state, and local requirements.

3.1.2.

Utilities

The Government shall furnish utility services such as water and electric for Contractor use in the performance of this PWS. The Contractor shall ensure employees conserve utilities.

3.1.3.

Communication

3.1.3.1. Telephone

The Contractor shall be allowed the use of the telephone for contacting Government Officials (e.g. CO, Civil Engineer, Fire Department, Security Forces) facilitating this contract, responding to customer complaints and any other directly contract related communications.

3.1.3.2. Communication Security (COMSEC)

All DOD communications are subject to COMSEC review. Use of DOD telephones and telephone systems constitutes consent to COMSEC monitoring. The Contractor shall comply with LAAFB COMSEC procedures and rules. For copies of these documents please contact Martha Shepard at 310-653-5383.

3.1.4.

Refuse Collection

Waste generated by the Contractor in performance of the services specified with this PWS shall be deposited in Government dumpsters, excluding hazardous materials in which proper disposal instructions shall be complied with. The Contractor shall comply with base recycling regulations.

3.1.5.

Hazardous Materials

Prior to use on Los Angeles AFB, the Contractor shall provide the Base Hazardous Material Pharmacy, through the CO, a list of all hazardous materials the Contractor intends to bring onto Government property and shall provide a Material Safety Data Sheet (MSDS) for each Hazardous Chemical listed in OSHA Hazard Communication Standard 29 CFR 1910.1200 in accordance with LAAFB Hazardous Material Management.

3.1.6.

Emergency Medical Service

Emergency medical treatment and emergency patient transportation service is only available through the local community.

3.1.7.

Security Forces

The Government will provide general security service. Security Forces phone extensions are 911 for emergencies, and 653-5787 for routine calls.

4.

General Information 4.1.

Contractor Personnel 4.1.1.

Contract Manager

The Contractor shall appoint a Program Manager (PM) to provide quality control, and who shall represent the Contractor and be responsible for the performance of the work throughout the term of the contract. The name of this person and alternate(s) who shall act for the Contractor when the manager is absent shall be designated in writing to the CO and the COR. The PM shall be able to speak, read, write, and understand English. The PM shall be able to effectively communicate with Contractor employees.

4.1.2.

Contractor Employees

The Government reserves the right to restrict the employment, under this contract, of any Contractor employee, or prospective Contractor employee who is identified as a potential threat to health, safety, security, general well being, or the operational mission of the installation and its population. Employees shall not be allowed to work a game if it is apparent he/she has been consuming alcoholic beverages or controlled substances. At no time shall an employee consume alcoholic beverages or use tobacco products in or around, the playing area. Employees shall not engage in arguments or behave in an unprofessional manner during, prior, or after scheduled games. All employees for adult games sponsored or supported by the Fitness Center shall be eighteen (18) years of age.

4.1.2.1. Physical Security

The Contractor shall be responsible for safeguarding all Government facility and sports equipment provided for each game. All portable Government equipment and materials that were provided will be returned to the Fitness Center for security daily.

4.1.2.2. Employment of Government Employees

The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest, or the appearance of a conflict of interest, nor shall the Contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives proper approval. The Contractor is cautioned that off-duty active military personnel hired under this contract may be subject to permanent change of station, change in duty hours, or deployment. Military reservists and National Guard members may be subject to recall to active duty. Their absence at any time shall not constitute an excuse for nonperformance under this contract. The Contractor is prohibited from employing off-duty COR who are managing any contracts or subcontracts awarded to the Contractor.

4.2.

Hours of Operation

4.2.1.

Normal Hours of Operation

The Contractor shall perform the services required under this contract during scheduled hours.

4.2.2.

Federal Holidays

The Contractor is not required to provide services on the following federal holidays:

Presidents Day

Memorial Day

Independence Day

Labor Day

Columbus Day

Veterans Day

Thanksgiving Day

Christmas Day 4.3.

Quality Control (QC)

The Contractor shall be required to submit a Quality Control Plan before the contract start date. The Contractor’s quality control plan shall contain, as a minimum, the following items:

a. A description of the inspection system to cover all services. Description shall include specifics as to the areas to be inspected on a scheduled and unscheduled basis, frequency of inspections, and the title and organizational placement of the inspector(s).

b. A description of the methods to be used for identifying and preventing defects in the quality of service performed.

c. A description of how the records will be kept. Records must document all inspections and corrective or preventive actions taken.

4.3.1.

Performance Evaluation Meetings

The CO may require the PM to meet at least weekly with the Contracting Officer Representative(s) (“COR”) and the CO during the first month of the contract. Meetings will be as often as necessary thereafter as determined by the CO. However, if the Contractor requests, a meeting will be held whenever a Contract Discrepancy Report is issued. The Contractor’s manager, CO and COR, shall sign the Contract Administrator’s written minutes of these meetings. Should the Contractor not concur with the Contractor Administrator's minutes, the Contractor shall so state any areas of non-concurrence in writing to the CO within ten (10) calendar days of receipt of the signed minutes.

4.3.2.

Government Quality Assurance

According to the Inspection of Services clause (52.246-4 Inspection of Services—Fixed-Price), the Government will evaluate the Contractor's performance under this contract. Government personnel will record all surveillance observations. When an observation indicates defective performance, the COR will require the PM or representative at the site to initial the observation. The initialing of the observation does not necessarily constitute concurrence with the observation, only acknowledgment that he or she has been made aware of the defective performance. Government surveillance of tasks not listed in the PWS or by methods other than those listed in the PWS (such as provided for by the Inspection of Services clause) may occur during the performance period of this contract. Such surveillance will be done according to standard inspection procedures or other contract provisions. Any action taken by the CO as a result of surveillance will be according to the terms of this contract.

4.3.2.1. Quality Assurance

The Government will periodically evaluate the Contractor’s performance in accordance with the Performance Plan. The Government may inspect each task as completed and increase the number of quality assurance inspections if deemed appropriate because of repeated failures discovered during surveillance inspections or because of repeated customer complaints. Likewise, the Government may decrease the number of quality assurance inspections if performance dictates. The Government shall also receive and investigate complaints from customers. The Contractor shall be responsible for initially validating customer complaints; however, the Government representative shall make the final determination of the validity of customer compliant (s) in the cases of a disagreement between the Contractor and the customer.

4.3.2.2. Quality Assurance Personnel

The COR are representatives of the CO and will participate in the administration of this contract. Subsequent to contract award, the Contractor will be provided with a letter identifying COR and setting forth their duties and authorities.

4.3.2.3. Discrepancy Notification

The COR will inform the PM (or authorized representative) when discrepancies occur and will request corrective action. The COR will make a notation of the discrepancy on their surveillance checklist with the date and time the discrepancy was noted and will request the PM (or authorized representative) to initial the entry on the checklist.

4.3.2.4. Scope Changes

Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the CO and not the COR.

4.4.

Administrative Records

All records, documents, and associated papers provided by the Government and generated during the period of this contract become Government property and will remain in place upon contract termination or completion. During the period of the contract, records, documents, and associated papers shall be available for review.

4.5.

Safety

The applicable Occupational Safety and Health Act (OSHA) standards will be mandatory for this contract. The Contractor shall comply with all Federal, State and Local laws concerning safety. The Contractor shall manage all work areas to ensure the safety of base personnel, or visitors in or near the areas. Contractor must ensure that personnel are properly trained on the equipment they are tasked to operate.

4.6.

Phase Out

If the contracted operation reverts to in-house operations, the Contractor shall cooperate with the 61FSS personnel to ensure an orderly change over. In the event a follow-on contract is awarded to other than the incumbent, the incumbent Contractor shall cooperate to the extent required to permit an orderly transition to the successful Contractor. Recruitment notices may be placed in each facility to allow successor Contractor's access to incumbent employees.

Appendix C - Applicable Publications and Forms

Publications (to include their supplements at any organizational level) and forms that apply to this PWS are listed below. The publications and forms have been coded as mandatory or advisory. The Contractor is obligated to follow those publications and use those forms coded as mandatory to the extent specified in other sections of this PWS. The Contractor shall be guided by those publications or use those forms coded advisory to the extent necessary to accomplish requirements in this PWS. All publications and forms listed shall be made available by the Government at the start of the contract. Supplements or amendments to listed publications from any organizational level may be issued during the life of the contract. The Contractor shall immediately implement those changes that result in a decrease or no changes in the contract price and notify the CO in writing of such changes. Before implementing any change that will result in an increase in contract price, the Contractor shall submit to the CO a price proposal within 30 calendar days following receipt of the change by the Contractor. The CO and the Contractor shall negotiate the change into the contact under the provisions of the contract clause entitled “Changes”. Failure of the Contractor to submit a price proposal within 30 calendar days following receipt of the change entitles the Government to performance according to such change at no increase in contract price (unless the time requirement is waived by the CO according to Paragraph C of the changes clause).
