Combined Synopsis/Solicitation
Amendment 1, 16 Sep 20
1. Revise the point of contact email on pg.2 from tiffany.thornton.3@us.af.mil to tiffany.thornton.3@spaceforce.mil.
2. Incorporating Attch 4, Past Performance Questionairre.
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
The solicitation number FA2521-20-Q-B074 shall be used to reference any written quote provided under this request for quote.
The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2020-09, effective 27 August 2020.
This is 100% set-aside for small business. The North American Industry Classification System (NAICS) code for this project is 813110 with a size standard of G099.
The purpose of this requirement is to provide non-personal chapel support services in accordance required to support the religious and spiritual mission of Patrick Air Force Base (PAFB), to include the musical and religious education components of the Catholic and Protestant communities of the South Patrick and Seaside Chapels at PAFB. The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision and other items necessary to perform the following eight (8) positions in accordance with (IAW) the attached Performance Work Statement (PWS):
Musician for Catholic Services Music Director for Catholic Services Coordinator for Catholic Religious Education (RE) Coordinator for Catholic Youth Musician for Protestant Services Music Director for Protestant Services Coordinator for Protestant Religious Education (RE) Coordinator Protestant Youth Contract Type: Firm-Fixed Price, Indefinite Delivery Indefinite Quantity (IDIQ). Minimum order: 3 MO service.
Period of Performance: The proposed period of performance for the subject requirement will be a five year, single-award IDIQ with a six month option to extend services under FAR 52.217-8, priced and evaluated at time of contract award, as illustrated below:

All companies must be registered in the System for Award Management at https://beta.sam.gov/ to be considered for award. The Government will not provide contract financing for this acquisition. Invoice instruction shall be provided at time of award.
Pursuant to FAR 12.602 contract award will be made using Simplified Acquisition Procedures in accordance with FAR 13.106. Offeror’s submissions will be evaluated under a best value determination, based upon the following:
(1) Past Peformance: Offeror is required to provide all management and labor required for the operation and management of the required services. Include three (3) past performance questionnaires with current contact information and contract number. Recent, relevant experience in all areas should be provided. Teaming and/or subcontracting arrangements should be clearly delineated and previous experience in teaming must be provided. The response must not exceed 5 pages.
(2) Technical Capability: Quote must be rated as technically acceptable to be eligible for award.
In order to be deemed technically acceptable, the proposal must meet all requirements as stated in the PWS.
(3) Price: Award will be made to the lowest priced technically acceptable offeror.
Past Performance and Technical Capability, when combined, are more importance when compared to price.

Discussions: The government intends to award a purchase order without discussions with respective vendors/quoters. The government however, reserves the right to conduct discussions if deemed in its best interest.
Include descriptive literature such as illustrations and drawings.
FAR Provision *52.212-3, Offeror Representations and Certifications -- Commercial Items (Jan 17), with its Alternate I (Oct 14), applies to this acquisition. All vendors must be registered in System for Award Management at https://www.sam.gov/portal/public/SAM/ at the time of Quote submittal.
FAR clause at 52.212-4, Contract Terms and Conditions -- Commercial Items (Jan 17), applies to this acquisition with the following Addendum: Paragraph (c) of this clause is tailored as follows: Changes in the terms and conditions of this contract may be made only by written agreement of the parties with the exception of administrative changes, such as changes in the paying office, appropriations data, etc., which may be changed unilaterally by the Government.
FAR clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. This clause incorporates by reference only those clauses required to implement provisions of law or Executive orders applicable to the acquisition of commercial items.
Note: The vendor acknowledges that should the quote or proposal’s terms and conditions and/or agreement conflict with mandatory provisions of the Federal Acquisition Regulation (FAR) and other Federal law applicable to commercial acquisitions, to the extent of such conflict the FAR and Federal law govern and conflicting vendor terms and conditions and/or agreement are unenforceable and are not considered incorporated into any resultant contract.
