PERFORMANCE WORK STATEMENT (PWS)

FOR

21ST MEDICAL GROUP

ENTRYWAY FLOOR MAT SERVICES

22 AUGUST 2019

PERFORMANCE WORK STATEMENT

Entryway Floor Mat Services

1. SCOPE OF WORK:

1.1. The Contractor shall provide pick-up, cleaning, placement and delivery of Contractor owned entryway floor mats throughout the facilities listed in this Performance Work Statement

(PWS) for the 21 Medical Group (21 MDG) at Peterson Air Force Base (AFB) and Schriever

AFB, CO.

2. OPERATIONAL REQUIREMENTS AND SPECIFIC TASKS:

2.1. The Contractor shall place, pick-up, replace and deliver clean entryway floor mats once per week on Wednesdays. The floor mat sizes shall adhere to the prescribed sizes in para 3.1. Floor mat colors shall be neutral brown, gray or dark blue, and without any specific designs or insignia.

2.2. Contractor shall provide a delivery/pick-up ticket for each location to the Government for all floor mats. All delivery/pick-up tickets shall be consolidated and turned into the 21 MDG

Service Contract Manager located in Medical Logistics Section within the Main Clinic. There will be no performance during Federal holidays, planned or unscheduled closures, in this case, the floor mats shall be picked-up and delivered on the next regular business day.

2.3. The operating hours for normal business on the base are 7:30 a.m. to 4:30 p.m. In the event of base closure, arrangements for service will be coordinated with the 21 MDG Service Contract

Manager, and in addition, if severe weather conditions exist which prevents Contractor's performance within the specified hours or operations within one hour of the estimated time of arrival. An alternate method of pick-up and delivery of floor mats shall be provided by the

Contractor should this situation arise, if feasible.

4. CONTRACTOR POINT OF CONTACT:

4.1. The Contractor shall designate, in writing, a primary and alternate person responsible for handling any contract performance issues. This person shall have full authority to act for the

Contractor on all Contract matters. The Contractor representative and alternate shall be able to fluently read, write, speak, and understand English.

4.2. The contract manager or alternate shall be available during normal duty hours 0730-1630, to meet on the Peterson AFB installation with government personnel designated by the contracting officer to discuss problem areas. After normal duty hours, the manager or designated alternate shall be available within 24 hours.

5. CONTRACTOR EMPLOYEES:

5.1. Appearance. Contractor personnel shall present a neat appearance and be easily recognized.

This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contain the company name and employee name.

Inappropriate clothing includes but not limited to clothing with profanity, offensive pictures, or any other items that do not present a professional image. Clothing must be clean and serviceable.

5.2. Employment Restriction. The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest nor shall the Contractor employ any person who is an employee of the Department of the

Air Force, either military or civilian, unless such person seeks and receives approval in accordance with DoD 5500.7-R (for military and civilians).

6. SECURITY REQUIREMENTS: The Contractor shall ensure all personnel employed in the performance of this contract, or any representative of the Contractor entering the Government installation, abides by all applicable security regulations (AFI 31-204, Air Force Motor Vehicle

Traffic Supervision, Chapters 2 & 3).

7. BASE ACCESS: The Contractor shall furnish a current Contractor Employee Roster/Access

List, and submit it through the 21 MDG Service Contract Manager. The roster shall be updated within ten (10) days of changes and copy provided to 21 MDG Service Contract Manager. The

Contractor shall obtain contractor badges for all Contractor employees’ requiring entry onto

Peterson AFB and Schriever AFB. Vehicle registration, proof of insurance and a valid driver's license must be presented and maintained for all vehicles to be allowed on the installation in accordance with AFI 31-204, Chapter 3.
