PERFORMANCE WORK STATEMENT

FOR

AUTOMATIC DOORS

MAINTENANCE, REPAIR, AND UPGRADE

ON

PETERSON AFB, COLORADO

21 CES/CEOE

Peterson AFB CO 80914-2370

SECTION I

DESCRIPTION OF SERVICES

1. SCOPE OF WORK. The contractor shall provide all management, tools, supplies, equipment and labor necessary to maintain, repair and upgrade the Automatic Doors at Peterson Air Force Base, Colorado, in a manner that shall ensure continuous and reliable operation and in accordance with (IAW) the Performance Work Statement (PWS). Work must be performed IAW commercial industry standards/codes, all federal, state, local regulations in a manner that shall comply with commercial standards for maintaining automatic doors. IAW Air Force Instructions and Federal Regulations; Air Force Operational Safety and Health (AFOSH) standards; and federal laws/publications which are referenced in the PWS, Section V- Appendix A “Estimated Work Load” and appendix B “List of Automatic Doors on Peterson AFB.”

1.1. MAINTENANCE AND REPAIR SERVICES. Work shall be in accordance with (IAW) commercial industry standards, and all federal, state, and local codes and regulations, including the current National Electric Code.

1.2. ROUTINE REPAIRS. When a routine automatic door repair is required, 21st Civil Engineering Squadron (21 CES) Contracting Officer Representative (COR) at (719) 556-4952 shall call the contractor and provide a call number to the contractor. The call number is used when invoicing in Wide Area Work Flow (WAWF). The Contractor shall respond WITHIN TWO (2) WORKDAYS after notification.

Contractor shall notify COR of estimated completion of repair to the automatic door(s) to an operational state.

1.2.1. Repair by Replacement. In the case where parts can no longer be obtained due to discontinued manufacturer, or because it would be more cost effective to replace a part than to repair a part, the contractor shall provide via email to the COR, a proposal to remove, dispose, and replace the entire automatic door assembly (motor, switches, etc.) so that after completion, the automatic door is “turn-key” operational. If any door needs to be replaced the required electrical wiring for door replacement shall comply with the National Electric Code and be installed by a licensed electrician, at the expense of the contractor. Proof of such installation shall be made available to the government upon request. Contractor shall provide normal warranty of parts and labor.

1.2.2. Scheduling Routine Repairs. Contractor shall contact COR (719) 556-4952 to schedule the repair.

The COR shall coordinate access/escort with the Building Manager if necessary.

1.3. EMERGENCY REPAIRS. When an emergency automatic door repair is required, the 21 CES/COR shall call the contractor and provide a call number associated with the emergency call number. The call number is used when invoicing in WAWF. Contractor shall respond regardless of the time of day or day of week, including Federal holidays, WITHIN TWO (2) HOURS after notification by the COR.

Contractor shall notify the COR of an estimated completion of repair to the automatic door(s) to an operational state.

1.4. REQUIRED GOVERNMENT APPROVAL. Total costs per service call, including labor and parts, SHALL NOT EXCEED $500.00 PER SERVICE CALL WITHOUT PRIOR GOVERNMENT

APPROVAL. When any Routine Service call, including “repair by replacement,” is expected to exceed $500, the contractor shall provide via e-mail a proposal to complete the entire call including all parts and labor within 2 workdays after initial service call for Routine Services. For Emergency Services expected to exceed the $500 threshold, the contractor shall call the government representative for verbal approval.

After government approval, the contractor shall proceed with completing repairs. Invoices via Wide Area Work Flow (WAWF) will state repair work performed with itemized listing of labor and parts by facility and service call number.

1.5. CONTRACTOR MANPOWER REPORTING. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the UNITED STATES AIR FORCE via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address: http://www.ecmra.mil/.

Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2016. SECTION IV

GENERAL INFORMATION

4.1. HOURS OF OPERATION.

4.1.1. NORMAL DUTY HOURS. Contractor shall perform Routine Services required under this contract during the following hours: 7:30 a.m. to 4:00 p.m. Monday through Friday.

4.1.2. HOLIDAYS. Routine repair work shall not be required on the following holidays or any holidays observed in lieu thereof:

4.2. QUALITY CONTROL. The contractor shall develop and maintain a quality program to ensure services are performed. Contractor shall develop and implement procedures to identify and prevent defective services from recurring. As a minimum, the contractor shall develop quality control procedures that address the areas as identified in Section II, Service Summary.

4.3. LICENSES AND PERMITS. Obtain American Association of Automatic Door Manufactures (AAADM). Contractor shall provide proof of such applicable licenses and permits upon request by the government.

4.3.1. Special Qualifications: For the life of the contract, Contractor employees shall be skilled technicians and currently certified in service/installation by the American Association of Automatic Door Manufactures (AAADM). Documentation of training or certification shall be made available to the CO or the COR upon request.

4.3.2. Each technician shall be knowledgeable to work on the basic types of automatic doors-swinging, sliding and folding doors and a variety of configurations (i.e., to include but not limited to single door that swings in or out and is left-handed or right-handed, and a pair of doors that simultaneously swing in the same direction. The door operator is concealed or surface-applied. The doors are center-pivoted, offset hung, balanced or hinged and some of our automatic swinging doors also feature sensors or control mats and safety signage).

4.4. CONTRACTOR PERSONNEL.

4.4.1. Contractor Employees: Contractor shall not employ persons for work on this contract if such employee is identified to the Contractor as a potential threat to the health, safety, security, or operational mission of the installation and its population.

4.4.2. Appearance: Present a neat appearance and be easily recognized as Contractor employees.

Identification of all employees shall be accomplished by wearing Contractor-provided distinctive clothing bearing the name of the Contractor. A Government-issued identification badge shall be worn visibly at all times.

4.5. QUALITY ASSURANCE. Contracting Officer (CO) will appoint a primary and alternate Contracting Officer Representative (COR) for management of the day to day activities of this contract. The identity, title, and authority of these CORs will be provided in writing to the contractor after contract award. The COR shall inspect and evaluate the contractor’s performance to ensure services are received IAW the requirements set forth in this contract. If the contractor and COR do not agree on an issue, the contractor shall contact the 21st Contracting Office at (719) 556-8528/7996 to discuss the issue. If a performance threshold has not been met or a contractor performance has not been accomplished, the COR will initiate and provide the CO, a performance assessment report (PAR) for issuance to the contractor.

The contractor shall respond to the PAR instructions provided and return the PAR to the CO within 10 calendar days of receipt.

4.6. SAFETY. The Contractor shall comply with Air Force Instruction (AFI) 91-207 “The US Air Force Traffic Safety Program, (dated 27 Oct 2011), Chapter 3”, is a mandatory policy for each individual while on all Air Force Installations.

4.7. SECURITY REQUIREMENTS. The Contractor and all employees shall comply with all Peterson AFB security requirements imposed by the local installation commander at all times while on the premises to include not using cell phones while operating a vehicle on base. Contractor shall obtain necessary badges(s) required for access to Peterson AFB.

4.7.1. The information contained in the Badge Request Letter is subject to the Privacy Act of 1974, as follows: AUTHORITY: 5 U.S.C. 4501; 10 U.S.C. 1124; EO 9397; “PRINCIPAL PURPOSE: to list the names of personnel authorized to perform various functions under this contract. ROUTINE USES: To ensure positive identification of personnel authorized to perform the listed functions. Last four numbers of the social security number is needed for this identification. Disclosure is voluntary, however, failure to supply any required information may cause denial of entry to or delays in processing access onto Peterson

AFB.”

4.7.2. Badge Request: The contractor shall send a letter on company stationery. The company information shall include address, telephone number, and contract number. For employees who require access to Peterson AFB list: full names of employee(s), last four numbers of their social security number, and date of birth. Send the letter to the Contracting Officeby email to the contracting specialist/officer.

Each contractor employee must obtain the necessary badge within the 10 day period. When a badge is requested for a new employee, an updated employee list (with the same information described above) must be sent to the contracting office for processing.

4.7.2.1. Individual Identification Badges: Contractor employees shall apply for individual identification badges at the Peterson AFB Security Force Squadron (SFS) at the Visitor Center, Building 1334, upon employment after the request letter is on file with the SFS. Badges are processed Monday through Friday from 6:00 a.m. to 2:30 p.m. No appointment is necessary. Upon completion or termination of the contract or termination of a Contractor employee, the Contractor shall surrender any identification badge(s) and vehicle identification to the COR who in-turn shall surrender the badge(s) to security forces the following business day.

4.7.2.2. Vehicle Requirement: Proper state vehicle registration, proof of insurance and a valid driver’s license are required to drive on base. After obtaining the proper badges/vehicle identification at the Visitor Control Center (Bldg. 1334), all commercial vehicles must enter through the East Gate (Marksheffel Road), primary search area for all commercial delivery vehicles. Contractor vehicles will have the company name prominently displayed on both sides of the vehicles. All vehicle cleaning and maintenance will be done off-base. The Contractor is responsible for cleanup of any vehicle leaks.

4.8. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL

COMMAND AUTHORITY. IAW Department of Defense Instruction (DODI) 3020.37, paragraph

E2.1.1., a crisis situation is defined as: Any emergency so declared by the National Command Authority or the overseas Combatant Commander, whether or not U.S. Armed Forces are involved, minimally encompassing civil unrest or insurrection, civil war, civil disorder, terrorism, hostilities buildup, wartime conditions, disasters, or international conflict presenting a serious threat to DOD interests. The contractor is not required to perform services during a crisis unless otherwise directed by an authorized government representative.

4.9. INVOICING. After job completion or within five (5) working days, Contractor shall process invoice(s) via Wide Area Work Flow (WAWF) to include copies of material receipts and forward electronically to the Government.
