1. Input Offeror’s Name (Name of Company)

2. Offerors shall input proposed Pricing Data in the Total Price column for Transition Period (can propose $0 Transition Price) and Year 1 through Year 5, then calculate the total Base Period Labor Amount.

3. Offerors shall input pricing data in Total Price column for each of Year 6 through Year 10 and calculate the total Option Period Labor Amount.

4. Offerors shall fill in the Material Profit Rate % and calculate the Material Profit Rate Amount in the Base Period and Option Period. Material amounts provided are notional in nature and are presented for evaluation purposes only

5. Offerors shall calculate the Total Proposed Price by summing the Base Period Labor Amount, the Option Period Labor Amount, the Material Profit Amount for the Base Period, and the Material Profit Amount for the Option Period for the Total Proposed Price.
