Statement of Work Purchase Request #: F4D3D90043AG02 Project Title: Repair Dormitory Bathtubs Facility #: B13320, B13321, B13322, B13323
1. The scope of work for this project includes, but is not limited to, all design, material, procurement and construction to provide a complete and usable product to the Government. The contractor shall provide all labor, materials, equipment, transportation, and supervision required to repair dormitory bathtubs in the above noted facilities and as outlined in the attached spreadsheet. All work shall be accomplished in accordance with all contract specifications, common work practices, and industry standards. The detailed statement of work is as follows:
2. Statement of Work:
a. All construction materials incorporated into this project shall be recycled, reused or green products. The proposal shall list all materials used, to include the product manufacturer’s name and model, part or other distinguishing identification and numbers. The use of recycled, reused or green product substitutes is not required where the substitute is not equal in quality or overall life cycle cost compared to its virgin equivalent. Where applicable, the proposal shall state why substitute products are not being incorporated using one of these criteria.
b. The work consists of inspecting the bathtubs noted in the attached spreadsheet, determining that the proposed work as noted in the attached spreadsheet is adequate and finally executing the repair. Repairs will range from repairing chipped porcelain, removing and restoring rusted areas, to fully resurfacing the porcelain tub as well as leveling low areas adjacent to the drain. All bathtubs shall be returned, as best as possible, to a new condition with a sealed porcelain finish with no rust visible and a smooth surface throughout.
c. The facilities involved will be occupied by dorm residents during the work. All access must be scheduled through the 30th Civil Engineer Squadron Dormitory & Furnishings Management (30 CES/CEACU). Access will be granted with a Government supplied escort to accompany the contractor during the execution of work during normal duty hours, Monday through Friday.
d. The contractor shall provide an overall schedule showing an approximate time frame to complete all work not in 60 days or less. In addition to the overall schedule, contractor shall provide a two week “look ahead” schedule, forecasting the units to be scheduled over the next two weeks and an estimate on how long each bathtub will be inoperable. This will allow 30 CES/CEACU to notify the residents of the impending work to be accomplished.
e. Note that each dorm unit, or MOD, has two bathrooms with each having a separate bathtub. The contractor shall schedule work in such a manner that at no time will both tubs in an individual MOD be under renovation or unusable at the same time.
