(PWS)

Oil Water Separator (OWS) Removals and

Tank Vapor and Interstitial Monitoring

SECTION I - GENERAL REQUIREMENTS

A. Safety Program Requirements:

VOLUNTARY PROTECTION PROGRAM (VPP). The Air Force (AF) is an active participant in the Occupational Safety and Health (OSHA) Voluntary Protection Program

(VPP) in accordance with OSHA’s Instruction, CSP 03-01-002, Voluntary Protection

Programs (VPP) Policies and Procedures Manual. We recommend all contractors become familiar with VPP. Information on VPP can be accessed at:

http://www.osha.gov/dcsp/vpp/index.html. The AF’s intent in this participation is to ensure the safety and health of all employees, including contractors, and communicate our safety and health expectations with contractors. The AF’s interest is to protect AF personnel working in or around contractor operations and to protect AF equipment and property. In the event an unsafe condition is discovered with AF provided facilities or equipment, the contractor shall immediately notify the Contracting Officer and Safety

Office associated with that Delivery/Task Order (D/TO) for evaluation. This requirement applies to on and off base D/TOs.

1. The contractor shall conform to all applicable State and Federal Occupational Safety and Health Administration (OSHA) Standards, National Fire Protection Association

Standards and Air Force Occupational Safety and Health (AFOSH) Standards for the protection of USAF property and personnel entrusted by this contract while working on the installation. The government shall assume no liability or responsibility for the contractor’s employees.

2. The contractor will implement a safety program that ensures protection of Government personnel and property. The program will consist of, as a minimum:

a. A Safety Plan that addresses, as a minimum, the elements listed below. The

Safety Plan will be approved by the 78 ABW/SEG (Safety Office) prior to commencement of any work described in this contract.

b. The contractor shall ensure subcontractors comply with the safety provisions of this contract, as applicable.

SAFETY PROGRAM ELEMENTS

1. MISHAP REPORTING AND INVESTIGATION:

The contractor shall report mishaps or incidents exceeding $2,000 (material + labor), disabling injuries to contractor employees and all injuries to government personnel within one (1) hour by phone to the Contracting Officer and 78 ABW/SEG during normal day shift hours. This report shall contain all available facts. Mishaps/Incidents occurring at other times of the day shall be reported as soon as possible the next normal workday.

The contractor shall immediately secure the mishap scene and damaged property and impound pertinent maintenance and training records until released by the 78 ABW/SEG

Safety Office. Such release shall be accomplished through the Contracting Officer Also, the contractor and the subcontractors shall cooperate fully and assist government personnel until the investigation is finalized and closed out.

2. OSHA INSPECTIONS: Department of Labor (DOL) OSHA inspectors may arrive at contractor work sites without formal notification in the event of an employee complaint or a no-notice inspection. Noncompliance with safety requirements can result in work stoppage, inexcusable delays, and/or costly fines issued by the DOL.

3. ELECTRICAL WIRING AND EQUIPMENT: All electrical wiring shall be performed IAW the National Electric Code (NEC). Electrical wiring and equipment shall be a type listed by UL or another recognized listing agent. All extension cords shall be kept in a good state of repair. All portable electrical appliances and equipment shall be unplugged at the end of each workday. Only explosion-proof electrical fixtures and appliances shall be used in areas where explosive vapors might be present.

4. CONTROL OF HAZARDOUS ENERGY (Lockout/Tagout): Lockout-tagout procedures will be used and comply with 29 CFR 1910.147 during performance of electrical work. Authorized personnel shall ensure the control of all hazardous energy

(active or stored) when servicing and maintaining equipment or machines. Power shall be disconnected and all energy isolating devices will be locked out and/or tagged out before starting work. The authorized person will verify the isolation and deenergization.

5. SMOKING: Smoking is prohibited in all Robins Air Force Base facilities. Smoking is permitted in designated and properly identified areas IAW RAFBI 32-2001.

6. HAZARD COMMUNICATION: In any contract where hazardous materials are involved, the contractor must comply with 29CFR 1910.1200, Hazard Communication, which include training their personnel. Contractors must provide 78 AMDS/SGPB the

Material Safety Data Sheets (MSDSs) for each chemical used at least 5 working days prior to start date. This includes, but is not limited to, all solvents, paints, adhesives, sealants, coatings, primers, mastics, etc. MSDSs must be the most current available.

MSDSs are available for hazardous materials that contractor personnel may come in contact with on RAFB. Contact your contract monitor or government point of contact for assistance.

7. PROTECTIVE BARRIERS/WARNING SIGNS: When it is necessary to barricade an area for the protection of personnel from hazardous operations, barricades are to be provided by the contractor. Barricades must be erected before the work requiring the barricades begins. When the work is complete, the barricades must be removed from the job site.

8. MOTOR VEHICLES: The contractor shall comply with AFI 91-207, The US Air

Force Traffic Safety Program, regarding the use of safety belts and other protective devices during vehicle operations. It is illegal to talk on a cell phone while driving on

Robins AFB unless you are using a hands-free device. No vehicle shall be stopped, parked, or left standing on any road or adjacent thereto or in any area in such a manner as to endanger the vehicle, other vehicles, equipment or personnel using or passing that road or area. Roads shall be swept if spillage occurs during hauling. Ensure safe operating condition of all contractor-owned vehicles. Unsafe and unserviceable vehicles shall be removed from service immediately. Futhormore, the contactors will comply with all traffic regulations for vehicle operation on Robins AFB.

9. DRIVING ON FLIGHTLINE: Personnel engaged in vehicle operations on the flightline shall be trained and certified in flightline procedures IAW Robins Air Force

Base Instruction 13-206, Control and Operation of Vehicles on Robins AFB Flightline and Industrial Area. Sponsoring units are responsible for the flightline and industrial area driving training of contractor personnel assigned to or performing work associated with their organization. Certification shall be on AF Form 483, Certificate of

Competency, and must be in the possession of the vehicle operator when operating on the flightline. The contractor is responsible for furnishing a trained and certified vehicle operator/escort for all infrequent vehicle operations such as material deliveries. The certified escort can be a passenger or operate a lead vehicle no further than three (3) vehicle lengths in front of the escorted vehicle. The contractor shall ensure that all subcontractors comply with these requirements.

10. FOREIGN OBJECT DAMAGE PREVENTION PROGRAM: The contractor shall establish and maintain an effective FOD prevention program that meets or exceeds the requirements of AFMCI 21-122, Foreign Object Damage (FOD) and Dropped Object

Prevention (DOP) Program, and AFI 21-101, Aerospace Equipment Maintenance

Management. The program is designed to eliminate the introduction of any foreign object that could damage an aircraft, aerospace component or aerospace support equipment operated at this depot facility. The FOD program must be integrated into the day-to-day operations to reduce/eliminate FOD incidents. Such program shall include covering waste dumpsters, barriers in place to stop migrating FOD from dirt and gravel piles, and end-of-shift or daily clean-up. The contractor must brief his personnel at least weekly on any FOD requirements. All contractors/sub-contractors performing maintenance in a FOD-potential area will receive and have documented Initial FOD

Awareness and Prevention training. Course number MHPMAS00001300 may be used to satisfy this requirement. All vehicle operators are responsible for performing a Foreign

Object (FO) inspection on their vehicles including all towed equipment/vehicle tires and open cargo areas of vehicles prior to entering the runway, taxiway, flightline and aircraft parking ramps and other areas directed by RAFB FOD Awareness and Prevention

Officer. All “open–air” contractor/sub-contractor and delivery vehicles must be free of loose items/debris that could potentially fall from the vehicle and cause a FOD hazard.

The cargo beds of all pick-up trucks must be clean or covered prior to entering the airfield. Vehicles will be subjected to inspection and denied entry if found to be unacceptable. The office/representative responsible for oversight (Government Quality

Assurance Evaluator or other designated individual) will ensure the contractors are properly briefed/ trained and understand vehicle requirements. Subject training will also be given during Flightline Drivers Training by 78 OSS. Ensure strict control for all tools, equipment, rags, residue and hardware. Also, contractors will adhere to all procedural guidance in accordance with Air Force Occupational Safety and Health (AFOSH)

Standard, 91-100, Chapter 6, Flightline Vehicle Operations, Unified Facilities Criteria

(UFC) 3-260-1, Attachment 15, Construction Phasing Plan and Operations Safety on the

Airfield During Construction and Robins AFB Instrucion 13-206, Control and Operation of Vehicles on Robins AFB Flightline and Industrial Area.

11. PORTABLE HEATERS AND LIGHTING:

a. All temporary heat that is provided by portable electric heaters SHALL be FM or

UL approved. Heaters will be kept away from combustible or flammable materials.

b. All extension cords used must be of sufficient gauge to operate heaters and lighting without heating the cord or plug.

c. All unnecessary appliances will be unplugged at the end of the workday.

d. Only explosion proof electrical fixtures and appliances will be used in areas where flammable vapors are present.

13. HAND TOOLS/ELECTRICAL TOOLS: All hand tools shall be kept in good repair and used only for the purpose intended. Defective tools shall be acceptably repaired or removed from service. Tools shall not be thrown from one level to another, and when used overhead, shall be secured or placed in holders when not in actual use.

All electrical tools shall be of the approved double-insulated type or grounded. Hand and portable power tools and equipment shall be guarded IAW 29 CFR 1910.243.

14. DRINKING WATER DISTRIBUTION SYSTEMS: Any requirement to use fire hydrants or to penetrate water mains must be approved by 78 AMDS/SGPB and coordinated with the base fire department (926-3487).

15. RADIATION: The proposed entry and use of radioactive material, lasers, and/or radiofrequency emitters, other than common handheld radios, must be coordinated with

78 AMDS/SGPB at least 45 days in advance as required by RAFBI 48-101.

16. HOUSEKEEPING PRACTICES: It is the responsibility of contractor personnel to maintain good housekeeping. The work site shall be kept in a neat and orderly condition.

Scrap material shall be removed by the contractor at the end of each workday. 29 CFR

1910.141

17. SUB-CONTRACTORS: The prime contractor shall include a clause in all subcontracts requiring the subcontractors to comply with the safety provisions of this contract, as applicable.

SECTION II – SPECIFIC REQUIREMENTS

The contractor’s prepared Safety Plan shall clearly define procedures, personnel qualifications, facilities, required equipment and any other items necessary to fulfill the following elements for the protection of government personnel and property. The contractor’s safety plan shall meet or exceed the referenced standards below.
