PERFORMANCE WORK STATEMENT
Replacement of Bowling Center Ball Returns and Bumper System/Lights
Rough Rider Lanes Bldg 140
Minot Air Force Base, ND 58705
27 March 2014
1. GENERAL: The contractor shall provide all labor, materials and equipment to complete the removal, then installation/replacement of 11 ball returns and 22 bumper system/lights. The installation shall be in accordance with manufacturer’s instructions and requirements of the
Performance Work Statement (PWS) as stated in the following paragraphs. The contractor must be Qubica/AMF certified in order to be considered.
2. PERFORMANCE PERIOD: This contract shall have a 60 days performance period. Work shall begin on or after 1 May 14 and completion date no later than 31 Aug 14. Coordinate work area with customer Point of Contact (POC).
3. GENERAL SCOPE OF WORK: Work includes Replacement of 11 ball returns and 22 bumper system/lights. All transportation, equipment, tools, supervision, labor, materials and performing the work in accordance with the specifications, standard industry practice, National
Fire Prevention Association (NFPA) codes, the National Electrical Code (NEC), Uniform
Mechanical Code (UMC), Uniform Plumbing Code (UPC), Uniform Building Code (UBC), manufacturer’s recommendations and terms of the contract.
4. NOTIFICATION: Contractor shall provide written notification to the Contracting Officer two weeks prior to commencement of work. The Contracting Officer shall notify the Project
Inspector who in turn will notify the Using Agency.
5. DEBRIS: Contractor shall remove all construction rubble from the work site to a Contracted
Authorized onsite Construction Dumpster or remove from Minot AFB on a daily basis.
Contractor is responsible for the security of tools and equipment being used at the project site.
6. FACILITY ACCESS: Contractor will have full access to the facility Monday-Friday between 0700-1600.
7. SITE DAMAGE: Contractor shall be responsible for all damage incurred during this project and shall return damaged areas to original condition at no additional cost to the government.
8. CONTRACTOR WORK HOURS: Working hours for the Contractor will be Monday-
Friday, 0700-1600.
