STATEMENT OF WORK FOR

MAINTENANCE AND SERVICING OF PORTABLE FIRE EXTINGUISHERS

OFFUTT AFB 24 June 2020

1. DESCRIPTION OF SERVICES: The contractor shall provide all tools, supplies/parts, equipment, extinguishing agent, excluding Halon 1211, and labor necessary to maintain, service and repair all real property portable and flight line wheeled Halon 1211 fire extinguishers at Offutt AFB, Nebraska in a manner that will ensure their serviceability and prevent deterioration in value or effectiveness. Following are estimated quantities of portable fire extinguishers to be used in determining workload.

1.1. MAINTENANCE AND REPAIR SERVICES: The contractor shall perform annual inspection of all fire extinguishers at the fire extinguisher shop located in the fire station, Bldg. 288 every Tuesday and Thursday from 0800 to 1600 hours. Base customers shall transport fire extinguishers to the fire extinguisher shop for inspection and servicing. The contractor shall perform six-year maintenance on all ABC, D, Halon 1211, and clean agent fire extinguishers as required by NFPA 10 at the government owned fire extinguisher shop or off-site at the contractor’s owned/operated extinguisher maintenance shop. The contractor shall perform six-year maintenance on all BC, clean agent, Class K, and Purple K (PK) fire extinguishers at the intervals required offsite at the contractor’s owned/operated extinguisher maintenance shop. The contractor shall perform hydrostatic test on all fire extinguishers at the intervals required. Hydrostatic testing of fire extinguishers shall be performed off-site at the contractor’s owned/operated extinguisher maintenance shop. The contractor will repair extinguishers as needed to bring the extinguisher to a serviceable condition. If cost of parts and/or repairs exceeds 75% of replacement costs, the extinguisher will be placed out of service and the customer/owner will have to replace the extinguisher at their cost. The contractor must ensure that adequate replacements are provided for customers when fire extinguishers are being serviced or tested when immediate turn around isn’t available. This can be taken from in stock extinguishers or contractor provided replacements. The contractor shall coordinate with the fire prevention office, the removal of any extinguisher from the base by the means of a hand receipt. Extinguishers needing to be transported to an off-site location for maintenance will be the contractor’s responsibility.

1.2. CORRECTIVE MAINTENANCE: Corrective maintenance returns a fire extinguisher to full operating capacity as described below.

1.2.1. Fire Extinguisher Recharge: Fire extinguishers shall be recharged as indicated in the list of work provided to the contractor. The replacement of extinguisher agent also includes the agent expellant for certain types of extinguishers. The Government will furnish, at no cost to the contractor, halogenated (Halon 1211) fire extinguishing agent (if required). Each fire extinguisher shall have a “Verification of Service” collar located around the neck of cylinder provided by the contractor.

1.2.2. Labeling. The contractor shall replace fire extinguisher identification labels, classification symbols, product inspection stickers/plates/decals and other required information labeling if the originals are damaged or missing. Labeling shall be in accordance with the manufacturer’s specifications. At no time will the contractor place any labels with their logos on government owned fire extinguishers other than labels for six-year maintenance and hydrostatic test.

1.2.3. Annual Inspection: The contractor shall perform a thorough examination of each fire extinguisher’s mechanical parts, extinguishing agent, and expelling means. The tamper seal of each fire extinguisher shall be removed by operating the pull pin/locking device and a new seal shall be installed. Annual inspection shall be recorded on a suitable label and affixed to the fire extinguisher. A written log will be kept for all extinguishers serviced by the contractor.

1.2.4. Six-Year Maintenance: The contractor shall perform overhaul maintenance in accordance with the manufacturer’s and National Fire Protection Agency (NFPA) 10 requirements for each type extinguisher. Six-year maintenance shall be recorded on a suitable metallic label or equally durable material, and shall be affixed on the rear exterior of the extinguisher shell. Old labels will be scraped off. Each fire extinguisher shall have a “Verification of Service” collar located around the neck of cylinder provided by the contractor. A written log will be kept for all extinguishers that a six-year maintenance was performed on, by the contractor.

1.2.5. Hydrostatic Testing: The contractor shall hydrostatically test fire extinguishers as specified in NFPA 10 and/or manufacturer’s recommendations. Fire extinguishers that pass hydrostatic test shall have the test information recorded on a suitable metallic label or equally durable material, and affixed to rear exterior of the fire extinguisher shell. Old labels will be scraped off. Each fire extinguisher shall have a “Verification of Service” collar located around the neck of cylinder provided by the contractor. No other label shall be affixed on the extinguisher shell. Extinguishers that fail the hydrostatic test shall be identified/labeled “Failed Test” and shall be returned empty to the base fire department for disposition.

1.3. REPORTS. The contractor shall maintain an automated data base and submit a complete report of all maintenance and servicing accomplished when fire extinguishers are returned to service to the government utilizing the Daily Activity Report. The reports shall be submitted electronically to the Government representative responsible for this contract on a monthly basis.

All records shall be able to show how many fire extinguishers are serviced by individual base organization. The reports shall include a record of repairs, recharging, and all replacement parts used. All records, documents, and associated papers provided by the Government or generated during the period of this contract become government property and will be returned to the government upon contract termination or completion. During the contract period, all records, documents, and associated papers will be available for Government review.

1.4. ENVIRONMENTAL PROTECTION: The contractor shall ensure that the purchase, use, handling, storage, transportation, and disposal of all toxic, hazardous and special materials and wastes are in accordance with federal, state and local environmental protection regulations. The contractor will provide Safety Data Sheets (SDS), on site, for all extinguisher chemicals used with the exception of Halon 1211 which will be provided by the fire department. A closed recovery system is required for the recovery Halon 1211 fire extinguishing agent and is provided by the government.

2. SERVICE DELIVERY SUMMARY.

Hydrostatically Test Fire Extinguishers.

Fire extinguishers are properly tested. Testing is performed IAW NFPA 10 and/or manufacturer’s recommended frequency.

1.2.5.
100% of fire extinguishers are tested when required unless extinguisher is condemned.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The government shall furnish a facility that measures 624 square feet. The facility is located at 911 Garland Street, west of the Offutt Field House. Contractor and COR will inventory all government furnished property within the first week of award of the contract. Refer to appendix B

3.1. Government Furnished Equipment: The government shall furnish to the contractor a Halon 1211 recharge/recovery unit, a TIF Instruments Halon leak detector, a dry chemical servicing unit, various capacity scales, a nitrogen cylinder storage cart, a shop vacuum cleaner and recharge adapters.

3.2. Government Furnished Utilities: The government shall furnish to the contractor utilities necessary to perform all operations required by this PWS.

3.3. Telephone Services. A government furnished telephone with a Class A line shall be furnished to the contractor at no additional cost. Class A telephone allows access to local off-base and base facilities only. The contractor is responsible for obtaining off-base capability for long distance communication.

3.4. Security Police and Fire Protection: The Base Security Police and the Base Fire Department can be reached by dialing 911 on Offutt Air Force Base.

4. GENERAL INFORMATION:

4.1. QUALITY CONTROL: Contractor shall develop and maintain a quality program to ensure maintenance and repair services are performed in accordance with NFPA and other applicable standards and the manufacturer’s instructions. The contractor shall develop and implement procedures to identify and prevent defective services from reoccurring. As a minimum, the contractor shall develop quality control procedures that address the areas identified in paragraph 2, Service Delivery Summary. The government evaluator must have a specific quality control inspector to notify in case of customer complaints. The name and contact information will be provided to the Fire Prevention office no later than 30 calendar days from contract award.

4.2. QUALITY ASSURANCE: The government will periodically evaluate the contractor’s performance in accordance with the Quality Assurance Surveillance Plan.

4.3. GOVERNMENT REMEDIES. The contracting officer shall follow the requirements of FAR 52.212-4, Contract Terms and Conditions for Commercial Items (May 1997), for contractor’s failure to correct nonconforming services.

4.4. HOURS OF OPERATION: The contractor shall perform the services required under this contract during the following hours: The contractor shall have the facility open on Tuesdays and Thursdays of each week for customer service hours from 0800 to 1200 hrs and 1300 to 1600 hrs. During this time period the contractor must be at the shop and available so base personnel can deliver and pick up fire extinguishers requiring annual inspections.

4.5. SECURITY REQUIREMENTS: Contractor personnel shall obtain a Government Identification (ID) card. The following is the form each Contractor personnel is required to fill out for a Government ID card before beginning work on Offutt AFB:

4.5.1. All contractor personnel who shall be driving on Offutt AFB need to receive an ACC Form 75 Vehicle Pass. The OFFUTT AFB Security Forces Pass & ID section located in the Visitor Control Center adjacent to the USSTRATCOM gate will issue this form. This vehicle pass shall be kept inside the vehicle at all times while it is on Offutt AFB.

4.5.2. The contractor is responsible for controlling and accounting for all vehicles passes, government ID cards, and restricted area badges. When employee(s) leave the employment of the contractor, the contractor is responsible to recover the government ID card(s) and all other entry/security documents from the terminated employee and immediately deliver these items to the Contracting Officer Representative (COR).

4.6. SPECIAL QUALIFICATIONS: Maintenance, servicing, repair, and recharging shall be performed by trained and certified persons in accordance with NFPA 10, having available, the appropriate servicing manual(s), the proper types of tools, recharge materials, lubricants, and manufacturer's recommended replacement parts or parts specifically listed for use in the fire extinguishers. A document or certificate showing personnel are certified to perform maintenance and recharging of extinguishers shall be made available when requested by the Offutt Fire Prevention Office.
