About this file

This document is a Description of Intended Single Source Purchase (PUR 7776) issued by the Florida Department of Health (DOH), specifically the Public Health Statistics & Performance Management (PHSPM) division. The document outlines a single-source procurement for Public Health Accreditation Board (PHAB) services for a one-year term, with a posting date of 3/31/2025 and a last day for receipt of information of 4/21/2025. The accreditation services are designed to help the Department improve and advance its effectiveness in improving health communities throughout the state, including an assessment against nationally adopted standards, an assigned accreditation specialist, education materials, peer review of documents, a virtual site visit, and ongoing guidance to support a quality improvement culture.

The estimated dollar amount for these single-source services is $40,000.00, with PHAB identified as the exclusive provider of these services in the United States. The justification for the single-source acquisition is based on PHAB being the only provider of these specific accreditation services, and their national standards promoting efficiency, effectiveness, and continuous quality improvement for public health. The procurement ensures compliance with Florida Statutes sections 154.01, 154.011, and 381.001. The document was approved by Allegra Small, Purchasing Director, and invites prospective vendors to provide information about their ability to supply similar services, though it emphasizes PHAB's unique position as the sole provider.
