About this file

This is a Scope of Work document for the Texas Department of State Health Services (DSHS) outlining an annual full-service maintenance contract for two Qiagen EZ1 Advanced XL extraction instruments (Serial Numbers L199A2576 and L199A2602) utilized by the Texas Public Health Laboratory for rabies virus identification and testing. The equipment is located at the DSHS Laboratory, 1100 W. 49th Street, Austin, Texas. The contract encompasses preventive maintenance, on-demand repair services, parts replacement, and technical support for both instruments. The annual full-service plan must include one recommended preventive maintenance visit per year, unlimited telephone support calls, on-site remedial maintenance within twenty-four hours of request, and coverage during standard business hours (8:00 am to 5:00 pm Central Time, Monday through Friday), with technical call center support available twenty-four hours per day, seven days per week.

Service requirements mandate that all labor, travel, expenses, and parts be furnished by the vendor, with all replacement parts provided on an exchange basis as new, standard, equivalent refurbished, or equivalent quality components. The vendor must directly perform all service and maintenance without subcontracting due to the critical public health nature of the equipment. Service engineers must be factory-trained and qualified to work on Qiagen equipment hardware, with respondents required to submit training certificates and experience summaries for service personnel at the time of bid submission. The DSHS reserves the right to add or remove equipment from the contract during its term.
