About this file
This is a Proposer's Disclosure Statement for Design Professional Services (Project No. J-465-DP) for the One Arsenal Place Renovation at Columbus State University in Columbus, Georgia, submitted to the Board of Regents (BOR). The disclosure statement is a mandatory component of proposal submissions and requires all proposers to provide written responses to three specific questions regarding their business relationships, gifts or benefits provided, and any potential conflicts of interest with BOR, the Using Agency, or the end-user. Proposers must complete each section, date the form, and obtain authorization signature from an authorized company representative prior to submission with their formal proposal.
The disclosure requirements are designed to ensure compliance with Georgia state laws, rules, and Executive Orders by preventing conflicts of interest and maintaining the appearance of propriety throughout the selection and contract performance process. Each proposer must disclose any business transactions occurring within the two years prior to submission, any gifts or hospitality provided within the one-year prior period, and any conflicts or potential conflicts of interest as defined by applicable state standards. All responses must be submitted as part of the proposer's formal submittal package.
