About this file
This is a vendor disclosure and notification form from the Colorado Department of Corrections required for non-construction contract performance outside the United States or Colorado, as mandated by Colorado Revised Statutes 24-102-206. The form serves as notice to the Department of Personnel when a vendor or its subcontractors intend to perform services under an awarded contract outside the United States or the State of Colorado. Bidders must complete and return this document with their bid submission to the sole contact listed on the solicitation. The form requires the business's President, Board Chairperson, or Contract's Authorized Signatory to certify whether any services or subcontract work will be performed outside U.S. or Colorado borders, and if so, to provide detailed information regarding the nature of services, geographic locations, and justification for performing work outside these jurisdictions.
The form establishes a disclosure requirement for vendors planning to conduct contract performance internationally or outside Colorado, with specific sections requiring contractors to describe the type of services (such as software, hosting, technical support, design and development), identify the countries or states where services will be performed, and explain the necessity or advantage of performing work outside the United States or Colorado. The procurement section of the form requires documentation of whether the vendor provided notice within 20 days of contracting or deciding to contract outside the U.S. or Colorado, ensuring compliance with statutory notification timelines.
