About this file

Exhibit E is a vendor disclosure and notification form required by the Colorado Department of Corrections for non-construction contracts. This document serves as official notice to the Department of Personnel when a contractor or subcontractor plans to perform services outside the United States or the State of Colorado, in compliance with Colorado Revised Statutes 24-102-206. Bidders are required to complete and return this form with their bid submission to the sole contact listed on the solicitation. The form requires contractors to identify whether any services under the primary contract or subcontracts will be performed outside the United States or Colorado, describe the specific types of services to be performed externally (such as software development, hosting, technical support, or system monitoring), specify the countries and states where services will be performed, and provide justification for why performance outside these jurisdictions is necessary or advantageous.

The form must be signed by the business President, Board Chairperson, or Contract Authorized Signatory and submitted with the bid package. The Colorado Department of Personnel uses this disclosure to track contract performance locations and monitor compliance with state statutes. Vendors are required to provide notice within twenty days of contracting or deciding to contract for services performed outside the United States or Colorado. The form includes a section for procurement use only, which captures the solicitation number, contract type, services performed outside the specified jurisdiction, and verification of timely notice submission by the vendor.
