AIRCRAFT QUESTIONNAIRE

You may recreate this form but it must include all the information listed This Questionnaire completely and accurately completed will be used to determine your aircraft payload capability.

REPRODUCE AND SUBMIT FOR EACH AIRCRAFT OFFERED - COMPLETE SHADED BLOCKS

Minimum Aircraft Requirements:

* Must have a Standard Airworthiness Certificate.

* VFR/IFR, Day/Night

* Payload: Nine (9) insured passenger seats not including pilot, but including copilot seat in an aircraft, normally single-pilot operated.

* Payload: Non-jettisonable payload (HOGE)* of 1,400 pounds at 25 degrees C at 7,000 feet pressure altitude using a pilot weight of 200 pounds, a 35-pound survival kit and fuel for 1.9 hours.

* Airspeed: 100 Knots

* Landing Gear: Skid type

* Aux Fuel Tanks: 90 gallon internal Target Aircraft Requirements:

* Internal payload (HOGE) of 1,600 pounds at 25 degrees C at 7,000 feet pressure altitude using a pilot weight of 200 pounds, a 35-pound survival kit and fuel for 1.9 hours.

4. Interagency Load Calculation Form showing the performance characteristics of the aircraft. Calculations must include weight of aircraft in contract configuration, crew, and fuel as identified in Section A (using 7.0 lbs per gal.
