About this file
This is a Budget with Justification Form template from the Colorado Department of Human Services, Division of Youth Services, designed for organizations submitting proposals to this state agency. The form serves as a standardized tool for offerors to detail all anticipated expenditures associated with their proposed projects. The template is organized into several primary budget categories: Administrative Personal Services, Administrative Operating Expenses, Direct Staff Personal Services, Contractual Services, Supplies and Operating Expenses (including travel), and Indirect Costs or Cost Allocation. For each personnel line item, offerors must provide position titles, detailed work descriptions, gross annual salaries, fringe benefit amounts and component breakdowns, and the percentage of time the employee will dedicate to the project. The form requires justifications explaining how costs were calculated and the rationale for all budgeted items.
The template emphasizes the importance of providing comprehensive cost justifications, particularly regarding personnel allocations and fringe benefit calculations. Offerors must specify how fringe benefits are projected and identify components such as insurance, paid time off, and pension contributions. For subcontractors and contractual services, applicants must describe the selection process, work to be performed, deliverables, and cost calculation methodology. Additionally, organizations requesting indirect costs must provide copies of their negotiated indirect rate agreements or cost allocation plans, with CDHS honoring state or federal negotiated rates. The form accommodates multiple budget line items within each category and includes a final calculation totaling all direct costs, indirect costs, and any projected profit, allowing for a comprehensive financial submission to support program proposals.
