About this file

This is an insurance requirements exhibit (Revision 1, dated March 7, 2024) for a Linn County Road Department fence installation project on Seven Mile Lane in Albany, Oregon. The exhibit establishes mandatory insurance coverage that the contractor must obtain and maintain at its own expense prior to performing work and throughout the contract duration, including any warranty periods. Required coverages include workers' compensation insurance with employer's liability limits of not less than $500,000 per occurrence, and marine protection and indemnity coverage with per-occurrence limits of $1,000,000 for a single claimant and $2,000,000 for multiple claimants. All liability insurance except workers' compensation and professional liability must name the State of Oregon, its officers, employees, and agents as additional insureds using ISO Form CG 20 10 07 04 for ongoing operations and ISO Form CG 20 37 04 13 for completed operations. The contractor must waive all subrogation rights and provide certificates of insurance to the agency before beginning work, listing all endorsements and required coverage details.

The contractor must provide at least 30 days' written notice to the agency before any cancellation, material change, or non-renewal of required insurance coverages. If any required insurance is claims-made basis coverage without an extended reporting period of at least 24 months, the contractor must maintain tail coverage or continuous claims-made coverage for a minimum of 24 months following contract completion and agency acceptance, contract termination, or expiration of all warranty periods. All insurance providers and policies are subject to agency acceptance and verification, with the agency retaining the right to request complete copies of policies, endorsements, and related documentation. The contractor must also comply with periodic insurance requirement reviews as mutually agreed upon with the agency.
