About this file
This is a Rate Sheet fillable document for the California Department of Corrections and Rehabilitation (CDCR) Master Agreement C5612448-D, Rebid 7, which covers High Voltage Equipment Maintenance and Repair Services. The contractor will perform comprehensive maintenance, inspection, calibration, continuous load recording, testing, and repairs on all high voltage equipment specified in Exhibit A-1 across CDCR locations identified in Exhibit E. Services must comply with California Code of Regulations Title 8 Electrical Safety Orders, the National Electrical Code, NFPA 70E, California Code of Regulations Title 24 Part 3, California General Order 95, ANSI C57.12.90, and relevant ASTM standards. The rate sheet serves as the compensation mechanism for all labor, personnel, transportation including travel and per diem, fuel, mileage, materials, supplies, tools, equipment, permits, licenses, taxes, fees, and shipping charges necessary to deliver the full scope of services.
The contractor must provide rates on the fillable Exhibit B-1 Rate Sheet that will be used for all compensation under this master agreement. The contractor is required to guarantee both labor and material on all services and work provided. The compensation structure is designed to be comprehensive, with rates covering all direct and indirect costs associated with service delivery across multiple CDCR participating locations statewide.

Rate Sheet
| HIGH VOLTAGE EQUIPMENT MAINTENANCE AND REPAIR SERVICES | |
| Electronic bid packages include an Excel version of this Rate Sheet (Exhibit B-1) in addition to the PDF file in order to assist bidders in computing accurate totals. Bidders should download the Excel version and complete ALL highlighted cells. | |
| The Contractor will be compensated for services performed in accordance with the rates specified on the Exhibit B-1, Rate Sheet which shall include all labor, personnel, transportation (including travel, per diem, fuel, and mileage), materials, supplies, tools, any necessary equipment, permits, licenses, taxes, fees, shipping/freight charges, and every other item of expense necessary (unless otherwise specified herein) to provide all identified services. | |
| Bidders may bid on one or more locations. Bidders are not required to bid on all locations; however, bidders must include a rate for all items within that location. Failure to indicate a dollar amount (blank cell) on any item may be grounds to reject the entire bid for that location. A zero dollar amount ($0.00 or $ -) listed for any and/or all items shall be interpreted and understood by the State to mean that the bidder will perform any such services at NO COST to the State. | |
| Any and all services performed outside the scope of this Agreement shall be at the sole risk and expense of the Contractor. All prices and notations must be printed in ink or typewritten. All prices must be bid in U.S. currency and shall encompass every item of expense necessary to perform the services. | |
| Quantities and services listed on the Exhibit B-1, Rate Sheet are the State's estimates only and are offered as a basis for the comparison of bids. The State does not expressly or by implication agree that the actual amount of work will correspond therewith and reserves the right to increase/omit portions of the work as may be deemed necessary or advisable by the State. | |
| Hourly rates must meet minimum prevailing wage determination made by the Director of the California Department of Industrial Relations. | |
| Each participating location’s total basis of award includes a Rate Sheet line item totaling 25% of the sum of the bidder’s service rate(s) to allow for additional as-needed, unforeseeable services. As-needed services will be payable at the applicable service rate(s) on the Exhibit B-1, Rate Sheet. | |
| All CDCR estimated quantities and Contractor provided rates are for the entire term of this agreement. | |
| Contractor may offer a discount on invoices paid within thirty (30) days of receipt. Discount offered must be at least one percent and a minimum of $50.00. | |
| Discount offered on invoices paid within 30 days of receipt = | * |
| *In the event of tie bids, CDCR reserves the right to determine the bidder entitled to the contract award based on the percentage of discount. If not applicable or a discount tie, a coin toss or lot drawing will be used in accordance with the Invitation for Bid, Basis for Award. |
| There are no guarantees that any services will be requested from this agreement. | ||||||||
