About this file
This is a Public Works Rate Sheet (Exhibit B-1) for the California Department of Corrections and Rehabilitation (CDCR) for synthetic turf installation at San Quentin Rehabilitation Center. The project is structured in two phases: Phase 1 encompasses submittals, and Phase 2 covers synthetic turf installation. Bidders are required to complete the Excel version of this rate sheet by entering dollar amounts for each phase and calculating the total bid amount for both phases combined. All prices must be submitted in U.S. currency and must be printed in ink or typewritten in clear, legible figures. Payment will be made upon completion of each phase, no more than monthly in arrears. The bidder must list all proposed subcontractors performing work exceeding one-half of one percent of the general contractor's total bid, including the subcontractor name, address, type of license, dollar amount, and percentage of work to be performed.
Bidders must certify under penalty of perjury whether they qualify as a California company under Public Contract Code Section 6107 to be eligible for reciprocal preference against nonresident contractors. Failure to complete any highlighted cell or provide a dollar amount for any item may result in bid rejection. A zero dollar amount will be interpreted as the bidder performing services at no cost to the State. Bidders may offer a discount of at least one percent of the invoice amount with a minimum of $50.00 for invoices paid within thirty days of receipt. In the event of a tie in bids, the lowest responsible bid with the highest discount will prevail; if discounts also tie, the awarded bidder will be determined by public coin toss. Prime contractors must list all certified Disabled Veteran Business Enterprise (DVBE) subcontractors used to fulfill DVBE participation goals.
