About this file
This Request for Informal Bids (RFQ) document is issued by the Idaho Division of Veterans Services for medical waste pick-up and disposal services at the Idaho State Veterans Home in Pocatello. The Idaho Division of Veterans Services seeks qualified vendors to provide biohazard waste collection, transportation, and disposal services compliant with OSHA regulations 29 CFR 1910.1030 and 1910.120. The contractor must supply a minimum of eight hard-sided biohazard containers (maximum 18"x18"x23") with lids and appropriate markings, including free replacement for unserviceable containers. Service requirements include a minimum of twice-monthly pick-ups on a schedule agreed upon in writing, with the facility responsible for delivering waste to a central location. Quotes are due by 5:00 PM MST on December 2, 2025, and interested vendors may schedule site visits prior to the closing date by contacting the Maintenance and Operations Supervisor at 208-235-7881. Award will be made to the vendor with the lowest total cost as submitted on Exhibit B. The contract term is one year with four optional one-year renewals, and all bids must be submitted exclusively through LUMA with a signature page included.
Pricing is based on cost per container multiplied by eight containers, with vendors able to specify container size in gallons and establish a "no waste fee" for scheduled pick-ups cancelled with less than one business day notice. Payment terms are NET 30, and invoices must be submitted monthly by the fifth day of each month with supporting documentation. The contractor must maintain Commercial General Liability and Commercial Automobile Liability insurance with minimum limits of $1,000,000 each occurrence, workers compensation insurance, and name the State of Idaho as an additional insured. The contractor is responsible for all disposal costs and regulatory compliance and must maintain all records for seven years from final payment. The contract may be terminated by either party with 60 calendar days' written notice or immediately for cause, including material breach, suspension of required licenses, or non-compliance with applicable law. No specific set-asides are identified in the solicitation.
