About this file
This document is an Evaluation Criteria file for a Lowest Price Technically Acceptable (LPTA) procurement for a Deformable Mirror (DM). The technical evaluation will focus on assessing offerors against specific minimum requirements for the DM, which is intended for use as an active optical component in an adaptive optics retinal imaging system. Key technical specifications include: more than 90 actuators in a circular layout, ability to correct Zernike orders up to 10, silver coating with over 95% reflectivity, 15 mm effective diameter, and specific surface quality requirements.
The evaluation process will be conducted in three steps: first, a technical acceptability review of each offer against the specified criteria; second, price evaluation only for technically acceptable proposals; and third, selection of the lowest-priced, technically acceptable offer for award. The procurement is being conducted by the U.S. Food and Drug Administration's Center for Devices and Radiological Health (CDRH) to support regulatory science investigations into ophthalmic devices, with the solicitation issued as a combined synopsis/Request for Quotation (RFQ) for commercial items.

Lowest Price Technically Acceptable (LPTA) Evaluation Criteria
1. Introduction This document outlines the evaluation criteria for determining the lowest price technically acceptable offer in accordance with Federal Acquisition Regulation (FAR) 15.101-2. The evaluation process will consist of a technical evaluation and a price evaluation. Only offers that meet the minimum technical requirements will be considered for the price evaluation.
2. Evaluation Process Overview
· Step 1: The contracting officer will first evaluate the technical acceptability of each offer based on the specified technical criteria.
· Step 2: Only those offers that are determined technically acceptable will be evaluated for price.
· Step 3: The lowest-priced, technically acceptable offer will be selected for award.
3. Technical Evaluation Criteria The technical evaluation will assess the offeror’s proposal against the following minimum technical requirements (adjust as needed for the specific solicitation):
· Criterion 1 - The Government requires a DM for use as an active optical component in an adaptive optics retinal imaging system with the following minimum requirements:
Number of actuators (>90) Actuator layout (circular) Zernike orders corrected (up to 10) Coating and reflectivity (silver, >95%) Effective diameter (15 mm) Surface quality (<10 nm RMS) Stroke (±50 µm) AR coated window in front of mirror for protection Driver electronics and computer interface (no external hardware required, prefer USB3 interface) Additional Requirements User manual: The Contractor shall provide a comprehensive user manual that includes detailed instructions for device operation, maintenance procedures, troubleshooting guidance, safety warnings, and complete performance specifications with operating parameters.
Warranty: The Contractor shall provide a minimum one (1) year warranty from date of Government acceptance of the DM. The Contractor shall guarantee full device replacement at no additional cost to the Government for any manufacturing defects or operational failures during normal use as defined in the user manual. Warranty coverage shall include all associated shipping costs for replacement units.
Shipping: All proposed prices must include shipping, handling, insurance, and customs clearance costs. The contractor is responsible for all costs associated with delivery to the specified destination and shall handle all customs documentation and clearance procedures for international shipments.
· Criterion 2 - The Contractor shall provide the following items within 30 days of contract award:
1. DM System: Complete device as specified, factory-tested and calibrated, with all necessary components for immediate operation
2. User Manual: Comprehensive documentation including installation procedures, operating instructions, maintenance guidelines, troubleshooting protocols, safety warnings, and complete performance specifications
3. Technical Support: One (1) year of technical support from delivery date, including phone and email assistance during normal business hours (8:00 AM - 5:00 PM ET, Monday-Friday, excluding federal holidays)
4. Warranty: One (1) year manufacturer warranty covering full replacement at no cost for manufacturing defects or operational failures during normal use
Delivery Requirements All shipping, handling, insurance, and customs clearance costs are the Contractor's responsibility. The Contractor shall handle all documentation and procedures for US customs clearance. The Contractor shall deliver the DM within 30 days of contract award. The Contractor shall provide tracking information and delivery confirmation.
Delivery Address:
FDA
Silver Spring, MD 20993
4. Price Evaluation
· The price evaluation will assess the total proposed price for the contract.
· The contract award will be made to the lowest-priced offeror whose proposal is determined to be technically acceptable.
· Price Reasonableness: The contracting officer may, at their discretion, review the proposed price for reasonableness based on market research, historical prices, or other available data sources. If the price appears unreasonably low, the contracting officer may request additional information or clarification.
5. Acceptance of Technical Proposal Proposals will be reviewed to ensure they meet all of the following minimum standards. Failure to meet these minimum technical standards will result in the proposal being rated as "Technically Unacceptable," and it will not be considered for price evaluation.
The technical evaluation will assess the offeror’s proposal against the following minimum technical requirements (adjust as needed for the specific solicitation):
• Criterion 1 - The Government requires a DM for use as an active optical component in an adaptive optics retinal imaging system with the following minimum requirements:
• Criterion 2 - The Contractor shall provide the following items within 30 days of contract award:
1. DM System: Complete device as specified, factory-tested and calibrated, with all necessary components for immediate operation
2. User Manual: Comprehensive documentation including installation procedures, operating instructions, maintenance guidelines, troubleshooting protocols, safety warnings, and complete performance specifications
3. Technical Support: One (1) year of technical support from delivery date, including phone and email assistance during normal business hours (8:00 AM - 5:00 PM ET, Monday-Friday, excluding federal holidays)
4. Warranty: One (1) year manufacturer warranty covering full replacement at no cost for manufacturing defects or operational failures during normal use Delivery Requirements All shipping, handling, insurance, and customs clearance costs are the Contractor's responsibility. The Contractor shall handle all documentation and procedures for US customs clearance. The Contractor shall deliver the DM within 30 days of contract award. The Contractor shall provide tracking information and delivery confirmation.
6. Unacceptable Proposals A proposal will be deemed "Technically Unacceptable" if it fails to meet the minimum requirements as described in the solicitation. Offers that are deemed technically unacceptable will not be evaluated for price.
7. Final Evaluation The final evaluation will compare the prices of all technically acceptable proposals. The contract will be awarded to the offeror whose price is the lowest among the technically acceptable offers.
