About this file
The document is a Governmentwide Debarment and Suspension form that recipients, contractors, and subcontractors must complete when entering into covered transactions to verify that the entity, its principals, and affiliates are not excluded or disqualified. The form requires the prospective lower-tier participant to certify compliance with U.S. DOT regulations, including that they and their subrecipients are not currently debarred, suspended, proposed for debarment, declared ineligible, voluntarily excluded, or disqualified. The form also requires the participant to disclose any criminal or civil judgments within the past three years and to promptly inform the Federal Transit Administration (FTA) of any contradictory information.
The related state and local contract opportunity is for "Citibus ADA Paratransit Client Assessments" issued by the City of Lubbock, Texas. The solicitation is seeking proposals for this service, but no other details about the project, response due dates, award information, or contract term are provided.
