About this file
This is a Texas Family Code Schedule form submitted as part of Solicitation No. 601320000071459 for Traffic Signal Poles for the TxDOT Corpus Christi District. The schedule serves as a compliance certification document required by the State of Texas to verify that respondents and beneficial owners meet eligibility requirements under Texas Family Code Section 231.006 regarding child support obligations. Respondents must provide the names and Social Security numbers of all individuals or business entities holding at least twenty-five percent ownership interest in the submitting organization. The form requires completion, signature, and return with the bid response, with failure to do so potentially resulting in the response being deemed non-responsive.
The document emphasizes that inaccurate certification may result in contract termination and payment withholding. Under the Federal Privacy Act, Social Security numbers are collected solely to identify persons with outstanding child support obligations and are kept confidential to the fullest extent permitted by Texas Family Code Section 231.302(e). Respondents who do not meet the ownership criteria must indicate this by checking the applicable box and signing the schedule. This certification is mandatory and must accompany all submitted responses to ensure compliance with state requirements.
