About this file
This is a Texas Family Code Schedule document for Solicitation No. 601440000051404, issued by the State of Texas to establish eligibility requirements for contract payment and state grants or loans. The schedule requires respondents to provide certification of compliance with child support obligations under Texas Family Code Section 231.006, and to submit names and Social Security numbers of any individuals or business entities holding at least twenty-five percent ownership interest in the submitting organization. Respondents must complete the ownership information section with applicable names and Social Security numbers, or check a box certifying that the ownership criteria do not apply. The document must be signed and returned with the response to avoid the response being deemed non-responsive.
This schedule serves as a compliance verification tool rather than a procurement document with pricing or performance specifications. Social Security numbers disclosed are used solely to identify persons with outstanding child support obligations and are protected under Texas Family Code Section 231.302(e) to maintain confidentiality. The State of Texas reserves the right to terminate any resulting contract and withhold payment if the certification provided is determined to be inaccurate. Completion and return of this signed schedule is mandatory for all responses submitted under this solicitation.
