About this file

This is a Form 1 Execution of Bid document for the Texas Workforce Commission (TWC) related to Solicitation 3202600161 for SA Assistive Technology Home Modification services. The form serves as the official bid submission document that requires bidders to acknowledge receipt of all TWC Terms and Conditions outlined in the Invitation for Bid, including all attachments, and to commit to providing the items and services as described. Bidders must complete all required information fields, including bidder name, mailing address, billing address, contact information, Employer Identification Number (EIN), and Texas Identification Number (TIN). The form requires an authorized representative to sign and certify compliance with all contract terms and conditions, including obligations regarding subcontractors as identified in IFB Section 3.19.

The form addresses several critical bidder obligations and certifications. Bidders must disclose any financial interests in proposed subcontractors and notify TWC of any changes in financial condition prior to award or within ten days of occurrence during contract performance. The Successful Bidder must provide thirty days' notice of any substantial changes in ownership or control and must notify TWC's Contract Manager within thirty days of any pending litigation, investigations, or adverse actions that could materially impact contract performance. The document also includes a comprehensive list of Texas procurement preferences, allowing bidders to claim preferences for goods or services that meet specific criteria, including those produced or offered by Texas-based businesses, service-disabled veteran-owned enterprises, products made from recycled materials, energy-efficient products, and goods from economically depressed areas. Bidders may also take documented exceptions to specific Terms and Conditions at the time of bid submission, though any exceptions not clearly identified will not be considered.
