About this file

This document is a Texas Family Code Schedule certification form required for Solicitation No. 601440000052528 issued by the State of Texas. The schedule serves as a compliance document to verify respondent eligibility to receive state grants, loans, or payments on state contracts under Texas Family Code Section 231.006, which addresses child support obligations. Respondents must certify that the individual or business entity named in the solicitation is eligible to receive the specified payment and acknowledge that the contract may be terminated and payment withheld if the certification is inaccurate. The document must be signed and returned with the proposal response, as failure to do so may result in the response being considered non-responsive.

For business entities with at least twenty-five percent ownership interests, respondents must provide the names and Social Security numbers of all persons or entities meeting this ownership threshold. Entities not meeting the ownership criteria may check a box indicating the schedule is not applicable. Per the Federal Privacy Act Notice, disclosure of Social Security numbers is required under Texas Family Code Sections 231.006(c) and 231.302(c)(2) to identify persons who may owe child support obligations. The SSN information will be kept confidential to the fullest extent permitted under Section 231.302(e) of the Texas Family Code. Authorized signatory and title are required on the form prior to submission.
