About this file

This is an Execution of Proposal form (Attachment D) for RFP No. 781-6-04389 issued by the State of Texas. The document serves as a mandatory submission requirement that respondents must complete, sign, and submit with their proposal to be considered for evaluation. The form requires respondents to provide comprehensive company information, including Federal Employer ID and Texas Tax ID numbers, entity name, physical address, contact information, and website URL. Respondents must identify three key contacts: an executive representative, a point of contact for proposal matters, and a point of contact for contract receipt. The form also requires disclosure of all persons with at least 25 percent ownership interest in the business entity, with names to be provided at the proposal stage and social security numbers required only from the selected awardee prior to contract award.

The document includes compliance certifications regarding Texas Government Code §669.003, which addresses employment restrictions for former state agency executives. Respondents may identify applicable statutory preferences under Texas Government Code, including provisions for products from persons with disabilities, energy-efficient products, Texas and U.S. products, recycled materials, products from economically depressed areas, and foods of higher nutritional value. The form provides space for respondents to list any exceptions to the stated terms and conditions, though none are mandatory. The respondent must certify by signature that all responses are true and accurate, acknowledge receipt and acceptance of the RFP requirements, and confirm compliance with Texas residency bidder definitions if applicable. Failure to sign and submit this form may result in disqualification of the proposal.
