About this file
This is a Conflict of Interest Disclosure form (Exhibit D) for RFP No. 301-24-04118 from the Texas Office of the Governor for American Sign Language Interpreter Services. The form requires respondents to certify their understanding of and compliance with various conflict of interest provisions, contract certifications, and statutes cited in the RFP, including sections on prohibited bids, gifts to public servants, and disclosure requirements.
The document requires respondents to either disclose any actual or potential conflicts of interest or certify that none exist. Additionally, respondents must verify compliance with Texas Government Code sections regarding restrictions on contracting with executive heads of state agencies, employment of former state officers who participated in procurement, and contracts with former agency employees. If any of these situations apply, respondents must provide specific information about former executives or employees, including their state agency affiliation and employment dates. The form emphasizes that failure to complete, sign, and submit this disclosure may result in disqualification of the respondent's proposal.
