About this file
This document is Enclosure 8 - Minimum Qualifications and Certification of Compliance Form for a county-level Request for Proposal (RFP) focused on an interim housing program for transitional age youth. The form requires proposers to certify their compliance with several key qualifications, including being legally authorized to conduct business in California, not being on federal debarment lists, and having a minimum of three years' experience operating an interim housing program serving transitional age youth. Proposers must also attach a completed W9 form and disclose any current or past licensure, HIPAA, and discrimination claims against their organization.
The form outlines specific requirements for proposers, such as the ability to enter into a county standard agreement and provide detailed information about any potential non-compliance with contract terms. Proposers are expected to demonstrate their organizational capability, legal standing, and operational experience in serving transitional age youth. The document provides a structured format for potential service providers to attest to their qualifications, with checkboxes and spaces to detail any exceptions or clarifications to the proposed contract terms and conditions.
