About this file
This is a Single Source Justification document for a federal contract award by the Department of Veterans Affairs, VISN 04, Philadelphia VA Medical Center under simplified acquisition procedures for purchases exceeding the micro-purchase threshold but not exceeding the simplified acquisition threshold.
The contracting activity seeks to procure an Iridex Pascal Laser System to address an urgent patient care requirement. The facility's existing retinal laser has reached end-of-life and is irreparable, resulting in cancelled appointments and procedures, with veterans being referred to community providers for care that cannot be delivered in-house. The justification cites critical patient safety concerns, as delays in treating retinal conditions such as retinal tears, proliferative diabetic retinopathy, and diabetic macular edema can result in permanent vision loss or blindness. Market research, conducted minimally due to exigent circumstances, identified SDVOSB L1 Enterprises as the sole distributor of the Iridex Pascal Laser System, with Iridex Corp. as the American-made original equipment manufacturer and a small business concern. The document acknowledges that while other distributors or alternative retinal lasers may exist, the additional time required for expanded market research outweighs potential competitive gains given the urgent patient safety implications. The contracting officer certified the justification on March 17, 2026. The total estimated acquisition value is redacted in the document.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:
This is an urgent requirement that, if not acted on immediately, would cause a negative impact on patient care and patient safety. Purchasing other equipment would cause needless delays to the procurement by adding additional time for market research and solicitation. Appointments and procedures are being cancelled because the retinal laser at the facility has reached the end of its life and is irreparable. As such, our veterans are currently being sent into the Community as our facility cannot provide the care required. There is no guarantee that our patients are being seen in a timely manner. Our patients are facing conditions such as retinal tears, proliferative diabetic retinopathy, and diabetic macular edemas which, if left untreated, can lead to permanent blind spots, loss of central or peripheral vision, retinal detachment, and even complete blindness in the affected eye. Every delay causes an increased risk to patient safety because once the retina is damaged, vision usually cannot be fully restored; so, the longer treatment is delayed, the lower the chance of saving useful vision.
4. Description of market research conducted and results or statement why it was not conducted:
Market research awas minimal given the exigent circumstances surrounding this procurement. The market research conducted indicates that the Iridex Pascal Laser System is American-made, and SDVOSB L1 Enterprises is the sole distributor of the required items. OEM, Iridex Corp. is a small business. While there may be other distributors supplying these items or other retinal lasers available, the risk of
