ATTACHMENT E RFP No. DU208WP-14-R-0002
PROPOSAL PREPARATION INSTRUCTIONS (ADDENDUM TO 52.212-1)
PROPOSAL CONTENT
Proposals shall be submitted in two separate parts, Part I – Technical Proposal and Part II – Business Proposal as shown below. Each part must be complete in itself so that evaluation of each part may be conducted independently, and so the technical and management part may be evaluated strictly on its own merit. Proposals shall be submitted in the format, if any, prescribed elsewhere in this solicitation. Proposals shall be submitted via e-mail. The offeror’s name and address, the solicitation and the date and time specified in the solicitation for proposal submission must appear in the subject line of the e-mail submission.
Proposals must contain enough detail to allow for thorough evaluation and sound determination of whether or not the offeror will be able to perform in accordance with the solicitation requirements. Offerors shall be careful that their proposals are neither too elaborate nor too general. Proposals shall address this particular solicitation with specific statements relevant to the PWS. The Government considers all unsubstantiated statements such as “The offeror understands and will perform in accordance with the Performance Work Statement” as being technically unacceptable.
A facsimile proposal will not be accepted under this solicitation.
PROPOSALS ARE DUE NOT LATER THAN 1:00 PM (ET) ON February 17, 2014. Proposals shall be submitted via e-mail to debra.a.mccraw@hud.gov and the offeror shall submit the Technical Proposal as a separate attachment and the Business Proposal as a separate attachment. Due to HUDs e-mail limitations, attachments shall not be larger than 5 megabytes. If applicable, a proposal may need to be submitted in more than one email.
Please include the following in the subject line of the email, “Proposal Submission for Solicitation DU208WP-14-R-0002, NASS Business Support”.
A facsimile proposal or proposal received through the mail will not be accepted.
PART 1 - TECHNICAL FACTORS PROPOSAL
1.1. FACTOR 1 – TECHNICAL APPROACH (limited to 25 pages) – The offeror shall provide a Technical Approach, addressing three components – a Technical Plan, Key Personnel/Staffing Plan, and a Quality Control Plan. The proposal shall emphasize the ability of the offeror to adequately plan, staff, and perform the contract requirements as described in the Performance Work Statement.
1.1.1. Technical Plan - the plan shall address the offeror’s strategy for ensuring timely completion of the work and how work deadlines will be met and shall include a work flowchart that will fully demonstrate the offeror understands the contract requirements.
1.1.2. Key Personnel/Staffing Plan - the staffing plan shall provide the offeror’s staffing by task, illustrating the number of hours allocated by each employee by task; Staffing plans including experience of any Key Personnel, continued training, ability to monitor current staff and processes to bring new staff into the contract when necessary. Resumes are required.
1.1.3. Quality Control Plan (QCP) - the QCP shall address the offerors methods and procedures to ensure quality and timely performance and prevention and detection of performance defects, with minimal intervention or oversight by the government.
1.2. FACTOR 2 – MANAGEMENT WORK PLAN (limited to 25 pages) - The offeror shall provide a draft of the Management Work Plan which will contain:
1.2.1. a plan for oversight of the contract requirements including critical path activities, task dependencies, task milestones, rationale for project organization and resource, and;
1.2.2. an explanation of how it will perform the HUD work in competition with any other work the offerors personnel are performing.
1.2.3. Provide current work commitments of proposed management staff, as well their availability to perform on this contract.
1.3. FACTOR 3 – PAST PERFORMANCE – (limited to three references, 2 pages per reference) – The offeror must identify the three most recent contracts, either on-going or completed not more than 3 years ago, that demonstrate performance relevant to the solicitation requirements. The offeror may submit information on major subcontract, similar work performed as part of a team or joint venture, or similar work performed as part of a predecessor organization. The offeror is encouraged to discuss any negative performance issues that occurred during performance of the identified contracts and any corrective actions taken.
For each identified contract, the offeror must specify the predominant product or service provided under the contract, the contract value, the contract award date, the contract completion date, and the name, title, address, telephone number, fax number, and e-mail address (if available) of a person familiar with the offeror’s performance.
The offerors proposal shall provide the offeror’s past performance focusing on three components – relevancy, quality, and sufficiency. This factor’s primary emphasis is to assess the past performance of the proposed prime contractor (or in the case of joint ventures or some other teaming arrangement, the primary member of the team).
a) Relevancy - The offeror shall prepare and submit a chart clearly identifying contracts for the same or similar services performed in the last 3 years prior to submission of proposal. The chart shall conform to and provide all the information as shown in the chart attached to this solicitation announcement.
b) Quality - The offeror shall provide references performed within the three years immediately prior to submission of the proposal. Of those within the three years, the offeror will provide the degree of relevancy (contracts similar in size, content, and complexity), quality (assessment of the between the past performance efforts submitted and the solicitation requirements.
c) Sufficiency - The offeror shall provide references that clearly indicate if the offeror was a prime contractor on the stated reference or was a subcontractor and who the prime contractor was.
PART II - BUSINESS PROPOSAL
2.1. Original SF-1449: Fully complete Blocks 30a-30c and sign all applicable portions.
2.2. Attachment B: All prices shall be rounded to the nearest dollar.
2.3. Section 504 and Section 508 Compliance Information, if applicable.
COST OR PRICING DATA. This solicitation does not require submission of certified cost or pricing data, however the government may conduct a price realism analysis, which may also be used in evaluating your understanding of the government’s requirements under one or more of the factors. Supporting cost information shall be provided to enable the government to determine that your company has a complete understanding of the requirements to be performed under this contract.
SOLICITATION QUESTIONS. All questions must be submitted via email NLT 1:00pm ET on January 3, 2013 to debra.a.mccraw@hud.gov. Questions received after that date may not be answered. Questions will be answered via an amendment to the solicitation.
