SECTION C – DESCRIPTION/ SPECIFICATIONS/ WORK STATEMENT

C.1 INTRODUCTION

The National Highway Traffic Safety Administration (NHTSA) is an agency of the U.S.

Department of Transportation (DOT). NHTSA’s mission is to save lives, prevent injuries and reduce traffic-related health care and other economic costs. The agency develops, promotes and implements effective educational, engineering and enforcement programs with the goal of ending preventable tragedies and reducing economic costs associated with vehicle use and highway travel.

NHTSA’s Office of Emergency Medical Services’ (OEMS) mission is to reduce death and disability by providing leadership and coordination to the Emergency Medical Services (EMS) community in assessing, planning, developing, and promoting comprehensive, evidence-based emergency medical services and 9-1-1 systems.

A key component to this mission is EMS education. NHTSA’s OEMS is dedicated to promoting a sufficient, stable, and well-trained EMS workforce, as well as enhancing the health and safety of all EMS providers. To this end, OEMS supports workforce research and develops resources designed specifically for EMS providers.

C.2 BACKGROUND

In 2000, NHTSA released the National EMS Education Agenda for the Future: A Systems Approach (Education Agenda). The Education Agenda lays out a vision for helping the EMS education system align with other health professions and enhances the professional credibility of EMS practitioners. It defines an EMS education system that is national in scope while being responsive to local needs – fostering consistency of provider levels across political jurisdictions.

The Education Agenda includes five major components: National EMS Core Content (published in 2005); National EMS Scope of Practice Model (published in 2007); National EMS Education Standards (published in 2009); National EMS Education Program Accreditation; and National EMS Certification.

The 2007 National EMS Scope of Practice Model (Scope Model) followed a consensus based process and defines national levels of EMS provider, including their entry-level skills and knowledge. The Scope Model was successfully used to assist states as they reviewed and revisited the medical appropriateness of their EMS interventions based on the 2005 National EMS Core Content. The Scope Model also acknowledged that each State has the statutory authority and responsibility to regulate EMS within its borders and to determine the scope of practice of State-licensed EMS personnel.

Today, the Scope Model is still being used by States to establish their own scopes of practice for their state which is consistent with the national model. This helps to improve the consistency and uniformity of EMS throughout the nation while still honoring the unique needs of individual states. Because newer scientific evidence is currently available that demonstrates improved patient outcome, several states have proceeded with implementation of new skills and interventions that exceed the national Scope Model. However, other states are prohibited from implementing new interventions (e.g. therapeutic hypothermia in cardiac arrest, naloxone administration, hemorrhage control, etc.) until these new skills are added to the Scope Model and/or otherwise adopted through an Evidence-Based Guideline or consensus-based process.

In 2012, NHTSA tasked the National EMS Advisory Council (NEMSAC) to revisit the Education Agenda to identify any minimal updates since the Education Agenda and all of its components were successfully developed and implemented. In late 2014, NEMSAC presented NHTSA with its final recommendations. One of the recommendations calls for revising the Scope Model on a periodic basis to remain up to date with medical practice and science.

C.3. OBJECTIVE

The objective of this contract is to identify new evidence and science regarding the regulation and State licensure of EMS personnel; facilitate implementation of the Scope Model with State EMS regulators including medical directors; provide opportunities for broad public and EMS community input and participation; and revise the Scope Model, incorporating the new evidence and science as well as public and EMS community input into the document.

C.4. GENERAL REQUIREMENTS

EMS practitioners and their medical directors want to give their patients the best possible care.

To do this, they need to keep up to date with the evolving body of scientific research and combine this scientific knowledge with their own clinical experience and each individual patient's circumstances and preferences.

The Contractor shall coordinate the Scope Model revision with the Model EMS Clinical Guidelines process, Evidence Based Guidelines development process, National Guidelines Clearinghouse, and work with the team producing the EMS Compass project.

Stakeholder consensus and coordinated efforts by the states are instrumental in removing unnecessary barriers to enable the full use of competent EMS professionals. The Contractor and its project team shall need to consider several questions related to new psychomotor skills prior to recommending them as a minimum in the revised Scope Model:

1. Is there evidence that the procedure or skill is beneficial to public health?

2. What is the clinical evidence that the new skill or technique as used by EMS practitioners will promote access to quality healthcare or improve patient outcomes? (The base of evidence should include the best available clinical evidence, clinical expertise, and research.)

Changes to the Scope Model must provide a greater integration of psychomotor skills for patients of all ages. To ensure consistency with the state regulatory process, state Offices of EMS must be integrally involved with helping to determine the Scopes of Practice for EMS providers in their states, but may need to design a more flexible process that is adaptable to rapidly changing levels of information, such as, information learned from the military.

The Contractor shall lead the process so State Offices of EMS are prepared to address regulatory issues related to the proposed changes to the Scope Model. While necessary to ensure consistency with the EMS Education Agenda for the Future, itis essential that revisions to the Scope Model be developed in the spirit of teamwork with appropriate EMS stakeholder involvement. Effective communications with appropriate facets of the EMS community is critical for project success.

The Contractor shall also establish a process and the criteria for regularly scheduled revisions of the Scope Model as part of this project. The scheduled revision cycle may coincide with the American Heart Association’s five-year cycle for revising the CPR guidelines, the National Registry of EMT’s Practice Analysis of the EMS personnel levels and/or other related efforts. In addition, the Contractor shall also establish a rapid process (which includes triggers and criteria) for making emergent changes to the Scope Model (i.e. public health emergencies such as the opioid overdose epidemic, emerging infectious diseases such as pandemic influenza or ebola virus disease, disaster situations (both naturally occurring and man-made) under conditions of scarce resources, etc.) that must be done between regularly established revision cycles.

C.5 SPECIFIC REQUIREMENTS

Specifically, the Contractor shall perform the work outlined below to accomplish objectives of this project:

C.5.1 - Kick-Off Meeting

Within four (4) weeks of contract award, the Contractor shall participate in a kick-off meeting via teleconference with the COR, Contracting Officer, and other appropriate NHTSA staff to review the project objectives and the approach to reach these objectives. The Contractor shall allow at least four (4) hours for this meeting.

C.5.2 –Develop a Subject Matter Experts Panel (SMEP) descriptions and recruitment document The Contractor shall work in coordination with the COR to develop a SMEP descriptions and recruitment document to facilitate recruitment and solicitation of SMEP members. Within six

The Contractor shall collect information on new science and technologies that will lead to improvement of clinical outcomes in patients treated and transported by EMS. The Contractor shall identify practice gaps between the 2007 Scope of Practice Model, emerging science, and current EMS patient care needs. The Contractor shall coordinate the Scope Model revision with the newly created Model EMS Clinical Guidelines process, Evidence Based Guidelines development process, National Guidelines Clearinghouse, and the team producing the EMS Compass project.

In addition, the Contractor shall also submit a draft rapid process for making emergent changes to the Scope Model. The draft rapid process shall include recommended changes to the 2007 National EMS Scope of Practice Model to add the administration of narcotic antagonists to the Emergency Medical Responder and Emergency Medical Technician scopes of practice.

Within four (4) weeks from the time NHTSA receives the Contractor’s draft assessment tool and practice gaps document and draft rapid process for emergent changes, the COR will either approve the drafts or contact the Contractor for further revisions.
