Amendment 002

The purpose of this amendment is to revise the language reflected in Section L.2, “Proposal Format and Submission Instructions”, Paragraphs L.2.2, L.2.3, and L.2.5 for clarity.

Accordingly, the affected sections of the RFP are modified as follows:

L.2.2 Organization of proposal

Volume I - shall contain the Technical Proposal consisting of the following sections as further described in L.2.5:

Base Contract

Section 1.a. – Understanding of Requirements and Approach in response to Base Contract

Section 2.a. – Expertise and Past Approaches Relevant to Base Contract

Section 3.a. – Key Personnel Proposed for Base Contract

Initial Task

Section 1.b. – Understanding of Requirements and Approach in response to Initial Task

Section 2.b. – Expertise and Past Approaches Relevant to Initial Task

Section 3.b. – Key Personnel Proposed for Initial Task

Volume II - shall consist of Past Performance Information.

Volume III - shall consist of a separate Business Cost/Price Proposal for each of the Base Contract and the Initial Task.

The content of proposal volumes must be identical to the electronic submissions and internally consistent with the organizational structure described herein. Those not adhering to this structure may be considered unacceptable.

The proposal must be clearly indexed and logically assembled, and written in a practical, clear and concise manner. Assertions made in the proposal should be specific and fact-based.

Proposals must be legible, double spaced (personnel résumés may be single spaced), typewritten (on one side only), in a type size not smaller than 12 point pitch, on paper not larger than eight and a half by eleven inches and not exceeding the page limits established in this solicitation. Pages in excess of the individual limitations shall not be read, and the proposal shall be evaluated as if the excess pages did not exist. Some foldout charts or diagrams may be used within the aforementioned restrictions/page limitations. Each 8½" x 11" page foldout will be counted as one page (i.e., one fold-out with two pages will be counted as two 8½" x 11" pages).

Each volume of the proposal must be submitted in a sealed package, clearly marked with the solicitation number. The proposal may have a cover letter (maximum 2 pages). Each volume and section must also include a table of contents. Tables of contents and blank section dividers are not included in the page limitations cited for each section. Proposals submitted in response to this solicitation must contain the information as outlined below.

“L.2.3 Page limitations for proposal volumes

“Volume I – Technical Proposal: 15 double-sided pages for the Base Contract (3 pages for each resume) and 10 pages for the Initial Task Order Response

Volume II – Past Performance Information: 1 page per project See Section J, Attachment J-2 for specific instructions”

“L.2.5 Content of proposals

Volume II - Past Performance Information

This section is modified as follows:

General Instructions. Offerors should note the difference between organizational experience and past performance. Organizational experience pertains to the types and amounts of work experience previously performed by a contractor, whereas, past performance relates to the quality and how well a contractor performs the services. Past performance information is an indicator of the Offeror’s ability to perform the contract. The assessment of past performance is separate from the responsibility determination required in accordance with FAR 9.103. The number and severity of an Offeror’s problems, the effectiveness of corrective action taken, the Offeror’s overall work record, and the age and relevance of past performance information shall be considered. Offerors lacking a record of relevant past performance or for whom information on past performance is not available, shall be evaluated neither favorably nor unfavorably in this area.

It is the responsibility of the Offeror to ensure all information supplied as references is current and accurate (i.e., contacts persons, phone numbers and fax numbers.) Offerors should contact their references prior to submittal of their written proposal to inform them of the urgency of their response to our past performance survey. The Contracting Officer will not contact Offerors if the information provided is incorrect, incomplete or no longer current. Lack of reference response may affect the Government’s evaluation of the Offeror’s past performance record. The collection of past performance information is not meant to violate the protection and guarantees of Attorney-Client privilege. In the event disclosure of any of the required information might constitute, or give the appearance of constituting, a violation of Attorney-Client privilege, the Offeror shall contact the Contracting Officer for guidance.

If clarifications are requested or negotiations are conducted with the Offeror, the Offeror will be afforded an opportunity to discuss the relevance of an Offeror’s past performance information and adverse past performance information to which the Offeror has not previously had an opportunity to respond. Names of individuals providing reference information on past performance will not be disclosed.

The Government shall not be limited to the information or references provided solely by the Offeror. Information offered or obtained independent of this solicitation may be used in the evaluation of the Offeror’s past performance.

Specific Instructions. Offerors shall submit a description of the last (3) contracts or subcontracts (Government or Commercial) awarded to the Offeror for similar services completed during the past three years and all contracts and subcontracts for similar services currently in progress. The following information must be included for each reference:

(1) Contract title

(2) Contract number

(3) Name of Government or commercial contracting activity, address and telephone number

(4) Procuring Contracting Officer’s name and telephone number

(5) Procuring Project Officer’s name and telephone number

(6) Date of contract award and period of performance, including options

(7) Contract type

(8) Contract award amount

(9) Final, or project final, contract price/cost

(10) Brief description of the requirement

(11) Description of contract performance to include a brief explanation of any problems or delays encountered and any corrective actions taken in regards to either cost, schedule, or performance.

Failure to submit 3 questionnaires will not disqualify an Offeror, however, it will make it more difficult for the agency to evaluate the Offeror.

Note: The Offeror shall ensure that all contact information for individuals and companies cited in past performance references is current, accurate, and complete to the best of its knowledge.

The Government will note any missing or unreadable information and, pursuant to FAR Part 15.207(c), notify the Offeror of any unreadable, incomplete, or suspected of being incomplete data immediately. The Government reserves the right to assess any additional past performance information it deems necessary outside of that required by this RFP.

General Instructions. In accordance with the instructions referenced therein, Offerors shall complete the “Contractor Past Performance Survey” in Section J, Attachment J-2, for the last (3) contracts or subcontracts (Government or Commercial) awarded to the Offeror for similar services completed during the past three years and all contracts and subcontracts for similar services currently in progress.

It is the responsibility of the Offeror to ensure all information supplied as references is current and accurate (i.e., contacts persons, phone numbers and fax numbers.) Offerors should contact their references prior to submittal of their written proposal to inform them of the urgency of their response to the survey.

Failure to submit 3 surveys will not disqualify an Offeror, however, it will make it more difficult for the agency to evaluate the Offeror.”
