58.2 & 58.6 - UQ to I" high buildugs of dirt, gravel & debris on top of the deck on either side of the longitudinal rmmer timbers with 1/2" of same on top of the longitudinal numer timbers.
58.3 - The timber curbs are weathered with scattered SQlintering & vandalism.
58.4- The deck timbers are 'vveathered. Decay found in the tog surface of the UQstream 3'- 0" (+/-) offive
(5) non-adjacent randomly located deck timbers with the decay being Qartially to comQletely through the thickness of the deck timbers (See Reg. Photo). These same conditions are also found in the downstream 3'- 0" (+/-) of all deck timbers (See Rep. Photo).
58.11 - There are two (2) sets of four ( 4) longitudinal runner timbers on tog of the deck that traffic travels across. Both sets consist of three (3) 12" wide x 3'' high timbers & one (1) 6" wide x 3" high timber. The toQ of all longitudinal runner timber are traffic worn & weathered. Scattered areas of heavy decay are found throughout the entire lengrth of the UQStream most 12" wide x 3" high timber (See Rep.
Photo). The tv.1o adjacent 12" wide x 3" high timbers next to the timber mentioned in the grevious sentence have decay in their north most 1'- 0". Decay is also found in the north most 1'- 0" of the 6" wide x 3" high timber in both sets of longitudinal runner timbers (See ReQ. Photo).

60.2 - The bulkhead timbers are weathered at both abutments with scattered decax, However, the fill is being adeguatel:t retained. Also; decax in the toQ most 1'- 0" of the bulkhead retaining piles.
60.5 -The timber caps are weathered with UQ to 4" cleeQ decay in the upstream most I 0" of the tog surface of the caQ at both abutments (See Rep.
Photo). SQlintering along the bottom stream face edge of the caps. Scattered 2" deep decay in the tog & end surface of the downstream end of the cag at Abut. #2. The decay along the tog of the ca12 at this location is found in the downstream 1'- 2" of the cag & is causing a minor bearing area loss beneath Joist# ! I.
60.6 - The stream bed degradation/erosion & the stream bank erosion found at Abut. #2 during the 2011 ins12ection have been regaired or have fi lied back in. Undermining was found beneath the bulkhead at Abut. #2 during the 2013 insgection, but the void created bx the undermining is onlx 6" high x 6" deeg (See Sketch on Sheet #4A of Report).
60.8- The piles are weathered.
61.1 -The south edge of the creek flows against the ugstream end of Abut.# I. The majorit:t of the creek flows aginst & along Abut. #2. Also; two channels & the main creek come together on the ugstream side ofthe bridge to form one creek.
61 .2 - Ripra12 has been glaced at the downstream end of Abut. #I & at the upstream & downstream ends of Abut. #2 & is presentlx stable (See Photos)
61.3 & 61.6 - Please refer to Item #60.6. Also;
machinerx (?)has been used to excavate the stream channel beginning at 40'- 0" (+/-)downstream from bridge (See Photo).
