Section 3: Nature of Construction Activity
3.1 General Description of Project and Scope of Work:
(i.e. highway construction, roadway realignment, highway and bridge reconstruction, etc.) is described below:
Road 2324 MP 0.6 Soil erosion and pollution control and vegetation will be established. Logs and debris from upstream of the culvert will be removed while maintaining cascading streambed morphology.
The existing 36" culvert at 25 + 97.94 will be replaced with an 84" diameter pipe culvert spanning 48 feet to ensure passing of Q 100 flows and debris. A precast concrete headwall at the culvert inlet will be installed to prevent future damage and inlet I outlet riprap protection will be installed to prevent scour and backwater erosion. The road embankment will then be rebuilt, and aggregate road surface will be replaced.
Road 2325 MP 3.5 Soil erosion and pollution control and vegetation will be established. At station 43 + 45, the original culverts (one abandoned and one destroyed during rain event) will be removed. A 36" culvert spanning the original culvert length of 140 feet a built to pass Q 100 flows and debris, will be installed. Special rock embankment will be installed between 40 + 60 to 42 + 00 and between 43 + 00 to 43 + 20. The fill slope I shoulder embankment will be recontoured and the road base and aggregate surface will be replaced in kind.
Road 2500 MP 25.0 Soil erosion and pollution control and vegetation will be established around the 160' embankment failure and undermined pavement shoulder. The existing culvert and all of the debris blocking the inlet will be removed, the failed embankment materials subexcavated and the embankment recontoured. A reinforced soil slope will be implemented with the addition of a special rock embankment spillway. A standard underdrain system with a v-ditch will be installed on the north side of the roadway. A new 18" CMP culvert with spillway assembly featuring the existing down drain and class 3 riprap armored outlet, will be installed. The existing concrete barrier will be reset on the south side of the roadway after the damaged asphalt surface has been replaced.
Road 2900 MP 7.6 Soil erosion and pollution control and vegetation will be established. Woody debris and unsuitable material will be removed and placed downstream of the culvert on either bank or within the floodplain and a new roadway embankment will be constructed. Recontouring of the embankment will occur to achieve the desired grade. The existing 72" x 90' pipe culvert will be replaced with a QlOO flow-capable, 90" culvert with riprap headwall and riprap inlet I outlet protection. The road base and surface course aggregate will be replaced.
Rd 2900 MP 9.3 Soil erosion and pollution control and vegetation will be established. Debris will be removed and embankment contours reconstructed in kind. Upstream logs will be removed and placed downstream of the culvert, on either bank, or within the floodplain. The existing 60" pipe will be replaced with two, 72" culverts, capable of passing Q 100 flows, with riprap headwalls and a riprap apron at their shared outlet location. Contours will be shaped to
[9] desired profile and roadway surface will be replaced.
Rd 2900 MP 10.2 Soil erosion and pollution control and vegetation will be established. Debris is to be removed and placed in the downstream floodplain while maintaining cascading streambed morphology. The existing culvert at station 83 + 80 will be replaced with a 96" culvert featuring a riprap headwall and outlet apron, the fill slope I shoulder embankment reconstructed, and aggregate surface replaced.
Rd 2900 MP 10.4 Soil erosion and pollution control and vegetation will be established. Debris will be removed from the existing 84" pipe culvert and placed downstream on either side of the bank or within the floodplain. Slope I shoulder embankment will be reconstructed. An 18'x18', precast concrete box culvert with concrete headwall and wingwall will be installed to create uninhibited aquatic organism passage and meet Q 100 flow volumes. Finally, the project will provide aggregate surface replacement.
Rd 5400 MP 26.9 Soil erosion and pollution control and vegetation will be established. Damaged road fill and the existing 24" culvert at station 99 + 90 will be removed. In place of the old culvert, a 48" culvert featuring a class 4 riprap outlet apron will be installed. Fill will be added and road embankment will be replaced to bring the road to the existing contours. The double lane asphalt surface will be replaced.
Rd 9000 MP 11.5 Soil erosion and pollution control and vegetation will be established. Between stations 130 + 40 and 132 + 00, a rock buttress will be constructed to shore up the failed hillside north of the roadway. The existing drainage culvert at station 130 + 20 will be cleaned in place. The ditch between stations 130 + 00 and 132 + 00 will be reconditioned in its entirety.
Rd9000MP45 Soil erosion and pollution control and vegetation will be established. The western ditch is to be reconditioned between stations 114 + 20 and 146 + 80. The existing 36" culvert will be protected and retained. The eastern embankment between stations 144 + 40 and 145 + 60 will be reconstructed.

Section 6: Compliance Other Applicable Requirements
6.1 Endangered Species Protection
Under which criterion is project eligible for Endangered Species coverage under the NPDES permit (EPA)?
DA Dn De On ~E
Criterion A.
Criterion B.
Criterion C
Criterion D.
No federally-listed threatened or endangered species or their designated critical habitat(s) are likely to occur in your site's "action area" as defined in Appendix A of EPA permit.
The construction site's discharges and discharge-related activities were already addressed in another operator's valid certification of eligibility for your action area under eligibility Criterion A, C, D, E, or F and there is no reason to believe that federally-listed species or federally-designated critical habitat not considered in the prior certification may be present or located in the "action area". To certify your eligibility under this Criterion, there must be no lapse ofNPDES permit coverage in the other operator's certification. By certifying eligibility under this Criterion, you agree to comply with any effluent limitations or conditions upon which the other operator's certification was based. You must include in your NOI the tracking number from the other operator's notification of authorization under this permit. If your certification is based on another operator's certification under Criterion C, you must provide EPA with the relevant supporting information required of existing dischargers in Criterion C in the EPA NOI form.
Federally-listed threatened or endangered species or their designated critical habitat(s) are likely to occur in or near your site's "action area," and your site's discharges and discharge-related activities are not likely to adversely affect listed threatened or endangered species or critical habitat. This determination may include consideration of any stormwater controls and/or management practices you will adopt to ensure that your discharges and discharge-related activities are not likely to adversely affect listed species and critical habitat. To make this certification, you must include the following in your NOI: 1) any federally listed species and/or designated habitat located in your "action area"; and
2) the distance between your site and the listed species or designated critical habitat (in miles). You must also include a copy of your site map with the NOL
Coordination between you and the Services has been concluded. The coordination must have addressed the effects of your site's discharges and discharge-related activities on federally-listed threatened or endangered species and federally-designated critical habitat, and must have resulted in a written concurrence from the relevant Service(s) that your site's discharges and discharge-related activities are not likely to adversely affect listed species or critical habitat. You must include copies of the

Criterion F.
correspondence between yourself and the Services in your SWPPP and NOi.
Consultation between a Federal Agency and the U.S. Fish and Wildlife Service and/or the National Marine Fisheries Service under section 7 of the ESA has been concluded. The consultation must have addressed the effects of the construction site's discharges and discharge-related activities on federally-listed threatened or endangered species and federally-designated.critical habitat. The result of this consultation must be either:
i. a biological opinion that concludes that the action in question (considering the effects of your site's discharges and discharge-related activities) is not likely to jeopardize the continued existence of listed species, nor the destruction or adverse modification of critical habitat;
or
IL written concurrence from the applicable Service(s) with a finding that the site's discharges and discharge-related activities are not likely to adversely affect federally-listed species or federally-designated habitat.
You must include copies of the correspondence between yourself and the Services in your SWPPP and NOI.
Your construction activities are authorized through the issuance of a permit under section 10 of the ESA, and this authorization addresses the effects of the site's discharges and discharge-related activities on federally listed species and federally-designated critical habitat. You must include copies of the correspondence between yourself and the Services in your SWPPP and NOi.
Support Documentation for Endangered Species criterion eligibility is filed in TAB 16.
6.2 Historic Preservation
1. Will the project involve installation of any of the following stormwater .controls that entail subsurface earth disturbance? Yes
D Dike
D Berm
D Catch Basin

Section 7: Erosion and Sediment Controls
Control measures at the construction site are selected, installed, implemented, and maintained to minimize pollutants in the discharge as necessary to meet applicable water quality standards.
The objectives of this SWPPP include:
• Stabilize the site as soon as possible (targeting the preparation and implementation of erosion and sediment controls to get the site to final grade, and either permanently or temporarily stabilize all bare soil areas as soon as possible).
• Perimeter controls will be installed before ground disturbing activities are initiated.
• Perimeter protection will remain in place until final stabilization has been achieved.
• Slopes and channels will be protected by conveying concentrated stormwater runoff around the top of slopes and stabilize slopes as soon as possible. A void disturbing natural channels and the vegetation along natural channels as much as possible.
• Divert stormwater coming on to the site by conveying it safely around, through, or under the work area. (Divert the clean water, trap the dirty water.)
• Protect receiving waters and other environmentally sensitive waters or areas adjacent to the site.
• Follow pollution prevention measures by providing proper containers for waste and garbage at the site. Store hazardous materials and chemicals so that they are not exposed to stormwater.
• Minimize the area and duration of exposed soils. Clear only land that will be under construction in the near future.
7.1 Natural Buffers or Equivalent Sediment Controls (Required for EPA CGP states only - ID, WA)
This section only applies if a surface water is located within 50 feet of the project's earth disturbing activities.
Are there any surface waters within 50 feet of the project's earth disturbances? Yes
NOTE: Reference EPA CGP appendix G for buffer guidance.
If Yes, check which natural buffer compliance alternative is used at the project site:
D The project will provide and maintain a 50-foot wide undisturbed natural buffer.
The location and width of natural buffers are shown on the Erosion and Sediment Control Plan sheets in TAB 6.
D The project will provide and maintain an undisturbed natural buffer that is less than 50 feet in width and is supplemented by additional erosion and sediment controls, which in combination achieves the sediment load reduction equivalent to a 50-foot undisturbed natural buffer.
The location of natural buffers is shown on the Erosion and Sediment Control Plan sheets in TAB 6.
(22]
Insert the width of the natural buffer to be retained Click here to enter text.
Insert description of additional erosion and sediment controls to be used in combination with the natural buffer: Click here to enter text.
Document the method used to determine that the above combination of natural buffer and sediment and erosion controls equal the estimated sediment removal capacity of a 50-foot undisturbed natural buffer: Click here to enter text.
~ It is infeasible to provide and maintain an undisturbed natural buffer of any size, therefore, the project will implement erosion and sediment controls that achieve the sediment load reduction equivalent to a 50-foot undisturbed natural buffer.
Insert rationale for concluding that providing and maintaining a natural buffer of any size is infeasible: Most of the ERFO repair sites include in-water work and therefore it is not feasible to maintain a natural buffer between the work activities and streams.
Insert description of erosion and sediment controls to be used to achieve the sediment load reduction equivalent to a 50-foot undisturbed natural buffer: Fiber rolls and silt fence
D The project qualifies for one of the exceptions in EPA CGP Part 2.1.2.1.e
D For a "linear project", site constraints (e.g. limited right of way) make it infeasible for the project to meet any of the CGP part 2.1.2.1.a compliance alternatives.
D Other exception (state type): Click here to enter text.
Document why it is infeasible to meet one of the buffer compliance alternatives: Click here to enter text.
Insert description of erosion and sediment controls to be used to achieve the sediment load reduction equivalent to a 50-foot undisturbed natural buffer: Click here to enter text.
7 .2 Perimeter Controls
All Perimeter Sediment Controls will be installed prior to ground disturbing activities taking place. There will be no clearing or grubbing outside the physical clearance limits.
Locations and design specifications ofBMPs are shown on the erosion and sediment control plan sheets in TAB 6.
The following perimeter controls will be implemented:
Measure: Fiber rolls
Implementation: All fiber rolls will be installed prior to any ground disturbing activities. Please also refer to project schedule, contract, and plan set (Erosion Control section).
Maintenance: Sediment will be removed from fiber roll perimeter controls before deposits reach 50% of the above ground height. It is the Contractor's responsibility to upgrade erosion control measures as necessary if the sediment controls are ineffective and turbid water is observed discharging from the site. Fiber rolls will not be removed until vegetation is satisfactorily established.
(23]
Contractor and PE Note: Add additional measures used during construction.
7 .3 Sediment Track-Out
The following methods will be implemented to minimize off-site vehicle tracking of sediment onto paved surfaces. If sediment escapes the construction site, off-site accumulations of sediment will be removed at a frequency sufficient to minimize off-site impacts.
Locations and design specifications of BMPs are shown on the erosion and sediment control plan sheets in TAB 6.
Measure: Washing within perimeter, without detergents or solvents
Implementation: During active construction, as necessary. Please also refer to project schedule, contract, and plan set (Erosion Control section).
Maintenance: Care must be taken that runoff is directed within construction perimeter and is to be managed by established control methods already in place. Any uncontrollable runoff must be remedied and additional BMPs put in place.
Measure: Stabilized Construction Exit Implementation: Wherever traffic will be leaving a construction site and traveling onto paved roads or other paved areas, within 1,000 feet of the site. Please also refer to project schedule, contract, and plan set (Erosion Control section).
Maintenance: Any sediment that is tracked onto pavement shall be removed by shoveling or sweeping. The sediment collected by sweeping shall be removed or stabilized on site. The pavement shall not be cleaned by washing down the street, except when sweeping is ineffective and there is a threat to public safety.
Contractor and PE Note: Add additional measures used during construction.
7 .4 Stockpiled Soil or Other Erodible Materials
Stockpile management includes measures to minimize surface erosion and sediment transport from soil stockpiles or other erodible materials.
Locate erodible stockpiles away from drainages including storm sewer inlets and culvert inlets.
Locations of stockpiles are shown on the erosion and sediment control plan sheets in TAB 6.
Measure: Revegetation with contractor-approved seed mix
Implementation: In places where exposed soil surfaces are not to be fine-graded for periods longer than 30 days. Please also refer to project schedule, contract, and plan set (Erosion Control section).
[24)
Maintenance: Frequent inspections until proper vegetative establishment has been made.
Measure: Silt Fence
Implementation: The silt fence shall be functional before other land disturbing activities take place. Silt fence should be functional throughout the duration of construction and for a long enough duration following construction to allow disturbed areas to stabilize following revegetation and seeding. Please also refer to project schedule, contract, and plan set (Erosion Control section).
Maintenance: Silt fences should be inspected periodically for damage (such as tearing by wind, animals, or equipment) and for sediment that has accumulated. Remove the sediment when it reaches one-half the height of the silt fence. In situations where access is available, machinery can be used. Otherwise, silt should be removed manually. The following are key elements to remember:
• The sediment deposits should be removed when heavy rain or high water is anticipated.
• The sediment deposits should be placed in an area where there is little danger of erosion.
• The silt fence should not be removed until adequate vegetative growth ensures no further erosion of the slopes. Generally, the fabric is cut at ground level, the wire and posts are removed, then the sediment is spread, seeded,· and protected (mulched) immediately.
Measure: Plastic sheets Implementation: Soil stockpiles and other erodible materials should be covered by plastic as soon as they are generated. Please also refer to project schedule, contract, and plan set (Erosion Control section).
Maintenance: Contractor should frequently check plastic to ensure soil or other erodible material is covered.
Measure: Temporary construction access Implementation: Temporary construction access will be installed at the beginning of construction and will provide access to and from the sites. Vehicle use will be restricted to designated construction entrances to minimize the tracking out of sediment. Please also refer to project schedule, contract, and plan set (Erosion Control section).
[25]
Maintenance: Where sediment has been tracked-out from the site and onto the surface of off-site streets, other paved areas, and sidewalks, remove the deposited sediment by the end of the same work day in which the track-out occurs, or by the end of the next work day if track-out occurs on a non-work day. Remove the track-out by sweeping, shoveling, or vacuuming these surfaces, or by using other, similarly effective means of sediment removal. Hosing or sweeping tracked-out sediment into any stormwater conveyance (unless it is connected to a sediment basin, sediment trap, or similarly effective control), storm drain inlet, or surface water is prohibited;
Contractor and PE Note: Add additional measures used during construction.
7 .5 Minimize Dust
Dust control includes practices used to reduce or prevent the surface and air transport of dust during construction
Measure: Application of water for dust control
Implementation: During active construction as needed. Apply at a rate of 3 gallons per acre, using a diffused or sprinkled flow.
