About this file

This is a Statement of Work (SOW) for a Blood Bank Reference Laboratory contract for Weed Army Community Hospital (WACH) at Fort Irwin, California. The contract requires a vendor to supply various blood products including Red Blood Cells (RBCs) of different blood types and Rh factors, Fresh Frozen Plasma (FFP), Cryoprecipitate, and other specialized blood components. The vendor must maintain specific inventory levels, rotating RBC inventory every two weeks, with designated quantities for O Positive, O Negative, A Positive, A Negative, and B Positive blood types, among others.

The vendor must be accredited by FDA, AABB, and College of American Pathologists, and maintain CLIA certification. Additional requirements include serving as a reference laboratory performing specialized testing (such as antibody identification and crossmatching), maintaining a base of operations within 100 miles of Fort Irwin, providing emergency blood delivery, and implementing a "look back" program to track potential infectious disease transmissions. The contract is structured for a 12-month base year with four 12-month option years, focusing on comprehensive blood bank services to support medical readiness at the military hospital.

STATEMENT OF WORK

BLOOD BANK REFERENCE LABORATORY CONTRACT STANDARDS

PURPOSE OF THE CONTRACT

1. The supplier must supply blood and blood products to Weed Army Community Hospital (WACH). These products include but are not limited to Red Blood Cells (RBC’s), Fresh Frozen Plasma (FFP), Cryoprecipitate (Cryo), Platelets, Plasma to include CCP plasma, and any other product made or derived from whole blood.

ACCREDITATION STANDARDS

2. The supplier must be a manufacturer or producer of the majority of the products supplied and must be accredited or meet the standards prescribed by the Food and Drug Administration (FDA) or any other government agency that regulates blood, the American Association of Blood Banks (AABB), and the College of American Pathologists. The vendor must also maintain standards set forth under the Clinical Laboratory Improvement Act (CLIA) and be a CLIA certified lab.

PRODUCTS AND INVENTORY REQUIRED

3. Blood types requested include A, B, AB, and O; Rh Positive and Negative; and will include blood which is compatible with certain other antigens or antibodies encountered in human beings.

4. The supplier will ensure the following inventory is supplied to WACH:

O Pos leuko-reduced RBC’s: 12 units Replace every 2 weeks O Neg leuko-reduced RBC’s: 6 units Replace every 2 weeks A Pos leuko-reduced RBC’s 12 units Replace every 2 weeks A Neg leuko-reduced RBC’s 6 units Replace every 2 weeks B Pos leuko-reduced RBC’s 2 units Replace every 2 weeks FFP 6 units Replace as needed Cryo 2 units As needed

The supplier will supply the following units to WACH as needed.

• B Neg leuko-reduced RBC’s

• AB Pos leuko-reduced RBC’s

• AB Neg leuko-reduced RBC’s

• Apheresis Platelets

• Covid Plasma (CCP)

• White blood cells (WBC’s)

5. All red cells must be leuko-reduced. From time to time this facility may also require red cells that are CMV negative or meet other special requirements that are common to the practice of transfusion medicine.

CONTROL, OWNERSHIP, AND REPLACEMENT OF INVENTORY

6. All units supplied will remain the property of the supplier until the units are used or rendered unusable by WACH, at which time the units will become the property of the government and will be paid for by WACH.

7. The vendor will rotate RBC inventory every two weeks and other products as needed based on the expiration date. It is the vendor’s responsibility to replace units before they expire. Any units in the inventory that expire do so at the expense of the vendor. The vendor will provide a schedule at the beginning of the contract to outline scheduled delivery dates. Delivery time and date will be on a weekday between the hours of 9:00 A.M. and 2:00 P.M. It is understandable that this date or time may need to vary on occasion, but the goal is to have a somewhat established and predictable day of the week and time of day that the exchange takes place. The “every three-week exchange” will further be known as a “scheduled” delivery day. Any deliveries outside the scheduled delivery day will be known as a “nonscheduled delivery day”.

8. The vendor is responsible for transportation costs to rotate blood products.

9. The cost to deliver any units not in inventory but are needed by WACH and must be delivered outside of the scheduled delivery dates will be borne by WACH.

10. The vendor will make every effort to deliver any blood needed in an emergency to WACH.

The vendor will also provide an address or location where WACH may dispatch a driver to pick up blood by ground vehicle or helicopter should WACH decide to pick up blood on nonscheduled days.

11. Because of the need for blood in an emergency, the vendor should maintain a base of operations within one hundred miles driving distance of Fort Irwin.

OTHER TESTING AND SERVICES

12. The vendor will serve as a blood bank reference laboratory for WACH and perform testing not performed at WACH or which is beyond the capability of WACH. The testing the vendor should provide to WACH per contract year include identification of antibodies (36 patients per year) found on an antibody screen, direct antiglobulin testing (12 patients per year), transfusion reaction workups (5 patients per year), and crossmatching of blood products (12 patients per year) if WACH is not able to perform the crossmatch test, and any other testing that may fall within what is generally considered blood banking.

13. The vendor must perform all testing to the units prior to issue to WACH that is required by law, rule, regulation, or directed by accrediting agencies. Further, should the vendor also collect any units from anyone at Fort Irwin, because of the unique personal jurisdiction which the military maintains over service members, the results of any positive test that might adversely affect a service member is to be reported to WACH within two weeks. The vendor will be provided with a point of contact at Fort Irwin who can receive the results. If the patient is not an active-duty service member, any positive result that may affect a patient’s health must be reported to that patient within two weeks. This requirement is to be tempered by any federal or state law that regulates release of patient information. So that the vendor may comply with this requirement, WACH will notify the vendor at vendor’s request whether a donor is a service member or not.

14. The vendor must provide a “look back” program to identify donations and product dispositions from individuals who subsequently test positive for transfusion associated infectious diseases and to identify recipients of the blood and blood products.

15. As part of the contract, the vendor must provide consultation services for any testing planned to be sent, or was sent, to the vendor, and for any product or potential product required by WACH.

BLOOD DRIVES

16. Nothing in the contract shall be construed to confer permission to the vendor to come to Fort Irwin and conduct a blood drive. That shall be done through a separate Memorandum of Understanding. However, should the vendor be allowed to come on Fort Irwin and conduct a blood drive, then the vendor shall give some sort of credit to WACH for any units collected on Fort Irwin against any blood used by WACH.

OTHER

17. It is understood by both parties that health care is a constantly changing field. What is not known today may be the common practice tomorrow. Therefore, the vendor will provide WACH with a price list of any products and services offered by the vendor that are not explicitly mentioned in the contract. That list of products and services offered will be updated by the vendor periodically. WACH reserves the right to order any product or service provided by the vendor so long as the overall value of the contract is not exceeded.
