PERFORMANCE WORK STATEMENT
Title: Training Center Hampton Roads (TCHR) Fall Protection Tower Maintenance
1.0 SCOPE OF WORK:
The Contractor shall provide all personnel, certifications, supplies, transportation, supervision, and non-personal services necessary to perform preservation work, qualification work, and needed additions to TCHR’s Fall Protection Towers located at Fort Eustis Virginia. There are two steel (painted) towers with ladders and deck grating. One tower is 40’ tall. The second tower is 20’ tall.

| 3.0 | SPECIFIC TASKS: |
| 3.1 | The Contractor shall: |
| 3.1.1 | Conduct work on one tower until completion. When one tower is complete, work on the second tower shall begin. One tower will be used to conduct training while the other is being worked on. |
| 3.1.2 | Be certified to work on steel structures and follow industry guidelines for welds and structural tests. (AISC) |
| 3.1.3 | Erect scaffolding and containment as necessary to complete all work. |
| 3.1.4 | Remove all deck grating from the 40’ tower. |
| 3.1.5 | Inspect deck grating for compromised areas and replace or repair as needed. |
| 3.1.6 | Remove all ladder safety systems from both towers. |
| 3.1.7 | Remove all ladders from both towers and inspect ladder rung welds. Re-weld any rungs not passing inspection. |
| 3.1.8 | Remove all rust and old paint near rust areas from tower. |
| 3.1.9 | Verify all welds using industry standard (AISC) and replace any compromised welds, steel, or bolts. |
| 3.1.10 | Re-install deck grating, ladders and ladder safety systems. |
| 3.1.11 | Install four (4) new engineered anchorages on 20’ tower. (See attached photos for position of anchorages and anchorage type) |
| 3.1.12 | Tighten and mark all bolts. |
| 3.1.13 | Weight test all Fall Protection anchorages to 5,000 lbs. in accordance with (2.2) ANSI Z359.18-2017 |
| 3.1.14 | Replace any anchorages not meeting the 5000 lb. requirement. (See photos for anchorage type) These anchorages can be welded or bolted on. |
| 3.1.15 | Prepare and prime any areas where paint has been removed or bare metal is showing and paint entire 40’ tower gray. |
| 3.1.16 | Prepare and prime any areas where paint has been removed or bare metal is showing and paint entire 20’ tower gray. |
| 3.1.17 | Remove scaffolding, debris and trash. |
| 3.1.18 | Provide reports for all tests and installed anchorages in accordance with ANSI Z359. |
| 3.2 | The Contractor shall submit reports as follows: |
| 3.2.1 | Email a maintenance report to TCHR Representative (TBD) within 14 days of job completion detailing all issues noticed and work completed. |
| 3.2.2 | Email an invoice to TCHR Representative (TBD) and submit your invoice to WAWF within three days of completion of services. Invoice should state all work performed. Also, all charges should be clearly identified and explained. |
| 3.3 | The contract personnel will be required to obtain and maintain a DD 1172-2 Common Access Card (CAC). The CAC is required for access to military installations. |
| 4.0 | DELIVERABLES |
| 4.1 | The Contractor shall: |
| 4.1.1 | Email a maintenance report within 14 business days of service to TCHR Representative (Section 3.2.1). |
| 4.1.2 | Email an invoice to TCHR Representative within three business days of service. (Section 3.2.2). |
| 4.1.3 | Submit an invoice to WAWF within three days of completed service. (Section 3.2.2) |
