Performance Objective PWS Reference4 Performance Threshold

Method of Surveillance

SS # 1.

The contractor shall provide an initial Inspection and Assessment for a mold remediation process that begins with a thorough initial inspection and assessment of the affected areas in Building 1539, Rooms 12 and 19.

During this phase, the contractor identifies the type of mold, the extent of the infestation, and the sources of moisture contributing to the problem.

This detailed evaluation helps in developing an effective remediation plan tailored to the specific conditions of the site. The findings from this phase are compiled into an Initial Assessment Report, which outlines the scope of the remediation required and the proposed methods for addressing the mold issue.

1.

1.1.

1.2.

Threshold: Comprehensive identification and documentation of all mold types and moisture sources within 92 hours of project initiation.

Metric: The initial report must cover 100% of the affected areas with accurate identification of mold species and moisture sources as verified by independent environmental testing.

Percent Inspection Validated Customer complaint.

SS # 2

The contractor shall provide a Remediation Planning and Implementation proposal.

Following the assessment, the contractor shall create a comprehensive remediation plan, specifying the necessary steps, tools, and safety precautions to be employed. This plan is critical for coordinating the systematic removal of mold and ensuring that proper containment and air filtration measures are in place to prevent the spread of spores. The implementation of the plan involves the physical removal of mold-infested materials, cleaning of all affected surfaces with EPA-approved antimicrobials, and the application of preventive treatments. Progress Reports are provided daily to document the ongoing activities and adjustments to the plan.

1.

1.1.

1.2.

Threshold: Implementation of the remediation plan must begin within two weeks of assessment approval, with no more than one week deviation from the outlined timeline.

Metric: At least 100% of contaminated materials are removed or cleaned, and air quality tests show greater than a 95% reduction of mold spore count to standard indoor levels, aligning with EPA guidelines.

Percent Inspection Validated Customer complaint.

SS # 3

The contractor shall conduct and create a Final Inspection and Report.

After the remediation work is complete, a final inspection is conducted by an independent third-party to ensure that all areas meet safety and health standards. This phase includes thorough testing to confirm the absence of mold.

The results are compiled into a Final Remediation Report and a Post- Remediation Clearance Report, which provide a detailed summary of the remediation efforts, the techniques used, and the final status of the areas.

These reports serve as a formal declaration that the spaces are safe for reoccupation.

1.

1.1.

1.2.

Threshold: Achieving a clearance rate of 100% from the final independent inspection, indicating no visible mold and air spore counts below actionable levels.

Metric: The Final Remediation Report and Post-Remediation Clearance Report must be submitted within 10 days of project completion, detailing all actions taken and confirming compliance with health and safety standards.

Percent Inspection Validated Customer complaint.
