STATEMENT OF WORK (SOW)
CP Radio Upgrade Bldg.929 RM. 119 MAXWELL AFB
WO# 13631844
1.0 Introduction:
The Government has a requirement for a contractor to perform several building modifications to assist command in upgrading CP Radio at Bldg. 929 rm.119 Maxwell AFB, Alabama. The contractor shall furnish all necessary equipment, labor, supervision, and materials specified in this Statement of Work (SOW). The work is subject to the terms and conditions of this SOW and as specifically cited in the contractor’s proposal to the government.
1.1 The contractor shall furnish and provide for the government the quality and quantity of service per Section 3 of this SOW.
2.0 Applicable Specifications/Standards/Related Studies:
APPLICABLE PUBLICATIONS, LAWS, REGULATIONS, ETC: The contractor shall comply with all State and local Alabama Department of Environmental Management (ADEM) codes. Additional codes, standards, and guidelines are provided in the specification sections that are part of this SOW.
3.0 Task Description:
3.1 LOCATION: This contract is to be performed at Bldg. 929 rm.119 Maxwell AFB, Alabama on the outside wall North side.
3.2 DESCRIPTION OF SERVICES: Contractor shall furnish all necessary equipment, labor, supervision, and material to accomplish the work in this SOW.
3.3 DESCRIPTION OF WORK:
3.3.1 Contractor shall survey the area where work is to be accomplished to ensure accurate measurements are taken.
3.3.2 Core one 4” cable entry port into the exterior (North) wall of building 929. (Ref PSA Sketch 1, Sheet 2, Keynote 1). See attached drawings for exact location.
3.3.3 Install two 6-foot sections of Unistrut on exterior wall (North) of building 929. (Ref PSA Sketch 1, Sheet 2, Keynote 2). See attached drawings for exact location.
3.3.4 There is a requirement for one (1) new, 20 AMP, 110 VAC circuit with L5-20R (twist-lock receptacle) to be mounted behind proposed rack location at same height as cable ladder. (Ref PSA Sketch 1, Sheet 1, Keynote 3). See attached drawings for exact location.
3.3.5 Preform and provide ground test results for existing wall mounted busbar in comm room. Provide results to the 85 EIS Project Manager. See attached drawings for location of busbar.
3.3.6 Property disturbed in the process of the Work shall be restored to the condition existing prior to commencement of the Work. Driveways, sidewalks, curbs, and drainage swells shall be restored, in kind, to existing materials and construction profiles.
3.4 PERFORMANCE OF WORK: Services shall be conducted in conformity with applicable regulations to contribute maximum satisfaction to responsible base offices. Contractors must abide by the resulting Construction Contract. Services shall be made with minimum divergence from schedules once established. All services shall be made with minimum disturbance and maximum protection of property. The Technical Monitor (TM) and Contract Administrator shall be notified 72 hours prior to work starting.
3.4.1 The contractor must within 24 hours of notice that a delay will be incurred notify the TM/Contract Administrator in writing. This letter must identify why there will be a delay, and how long. Also, the contractor shall take any actions in their control to prevent delay from occurring.
3.5 SUPERVISION: Contractors shall provide competent supervision at all times when work is in progress. Contractor is responsible for scheduling and coordinating various trade activities. The Contractor is also responsible for assuring that all work accomplished and materials used are in accordance with industry standards
3.6 ACCESS: The Buyer shall have access at any and all times to the contractor's equipment, materials, supplies, assigned areas and sites of operation for inspection purposes.
3.7 PERFORMANCE TIME: 120 days after the ‘Notice to Proceed’ has been signed.
3.8 HOURS OF OPERATION: The contractor will be able to accomplish this work between the hours of 7:00 AM and 5:00 PM. The contractor will not normally be permitted to work on weekends or on the following legal holidays (or the day the Federal Government observes these holidays) unless they have coordinated such work with the Contractor Administrator/TM at least seventy-two (72) hours in advance:

If interruption of any building utility services such as but not limited to the following, chilled water, electrical power, domestic water, fire protection water supply, or communications are required, the contractor shall perform the work after 5:00 PM and before 7:00 AM on weekdays or normal work week hours during the weekend. The contractor shall coordinate the interruption of the utilities 30 days in advance with the Contract Administrator and TM. The contractor shall include any additional cost of performing the work in their proposal.
3.9 GROUND FAULT CIRCUIT INTERRUPTERS: Whenever the contractor uses portable electrical tools or equipment in an outside location where operator will be in contact with a grounded surface or in an interior wet location where floor is conductive such as concrete, contractor shall provide and use a portable ground fault circuit interrupter (GFCI). This shall apply wherever electric power is supplied through government-owned facilities. Contractor shall be responsible for maintaining the GFCI in operating condition and testing it before each use.
3.10 PASS AND IDENTIFICATION ITEMS: The contractor shall ensure the following pass and identification items required for contract performance are obtained for employees and non-government owned vehicles:
3.10.1 DD Form 1172, Application for Uniformed Services Identification Card (AFI-36-3026, Identification Cards For Members of The Uniformed Services, Their Family Members and Other Eligible Personnel, and AETC Instruction 36-3001, Issue and Control of AETC Civilian Identification (ID) Cards).
3.10.2 AETC Form 58, Civilian Identification Card (AETCI 36-3001).
3.10.3 AF Form 1199, USAF Restricted Area Badge, or locally developed badge.
3.10.4 AF Form 75, Visitor/Vehicle Pass (AFI 31-204).
3.11 RETRIEVING IDENTIFICATION MEDIA: The contractor shall retrieve all identification media from employees who depart for any reason before the contract expires; e.g., terminated for cause, retirement.
3.12 FREEDOM OF INFORMATION ACT PROGRAM (FOIA): The contractor shall comply with DOD5400.7-R_AFMAN 33-302, Freedom of Information Act Program (FOIA), and requirements. The regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding For Official Use only (FOUO) material.
3.13 REPORTING REQUIREMENTS: The contractor shall comply with AFI 71-l0l, Volume1, Criminal Investigations and Volume-2, Protective Service Matters, requirements. Contractor personnel shall report to an appropriate authority, any information or circumstances of which they are aware may pose a threat to the security of DOD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment and as required thereafter.
3.14 PHYSICAL SECURITY: Areas controlled by contractor employees shall comply with Base Operations Plans/instructions for THREATCON procedures, Random Antiterrorism Measures (RAMS) and local search/identification requirements. The contractor shall safeguard all government property, including controlled forms, provided for contractor use. At the close of each work period, government training equipment, ground aerospace vehicles, facilities, support equipment, and other valuable materials shall be secured.
3.15 TRAFFIC LAWS: The contractor and its employees shall comply with base traffic laws and regulations.
3.16 INSPECTION AND ACCEPTANCE: Inspection and acceptance of Services will be performed at Maxwell AFB, Alabama. Inspection will be completed by the TM and acceptance by the Contracting Officer. Inspection and acceptance will be conducted in accordance with this SOW and the Maintenance Service and Repair Agreement provided by the Contractor in their proposal.
3.17 COMMUNICATIONS CABLING PROTECTION: Contractor shall protect all communications cabling while working and if damage is caused by the contractor than the contractor shall replace the damaged communications cabling with same or for internal cable base standard CAT6 cable. All CAT6 cable must be home run to communications room. Copper tie cables must be replaced with same. Fiber optic cables must be a continuous strand with no splices. All communications cabling must be wired to the T568B standard.
3.18 AIR FIELD DRIVING: All contractors must receive airfield drivers training prior to receiving notice to proceed or prior to driving on the airfield IAW (AFI 13-213, 2.1.3.39.7.) This training will be provided by Airfield Management at Bldg. 844, phone (334) 953-7406/7407.
4.0 Travel Requirements: Reserved
5.0 Data/Reporting Requirements:
5.1 INSTALLED EQUIPMENT: Contractor will furnish manufacturer warranties as needed for any component incorporated into this project such as equipment; any component installed in or made a part of the existing structure and any exterior or interior finishes, etc.
5.2 AS BUILT DRAWINGS: The contractor shall provide as-built drawings regardless of any deviation from the original design.
5.2.1 If the contractor provides the original design plans, they will submit those plans (whether changes made or not) at the end of the contract marked and signed as ‘As-Builts’. The drawings will be delivered in a useable electronic format as an AutoCAD “.dwg” file, 2007 or greater.
6.0 Deliverables:
6.1 A 1 year warranty on material and labor.
6.2 The manufacturer’s warranty, if applicable.
7.0 Government Furnished Property/Facilities/Equipment: Reserved
8.0 Technical Monitor (TM):
8.1 The TM is a contract employee under a Maxwell AFB contract assigned to interface with the contractor. The TM is authorized to provide technical direction within the scope of the contract. All technical direction shall be issued to the contractor via a contract Technical Directive (TD) countersigned by the government.
8.2 The TM can assist the contractor in the interpretation of technical requirements and performance of the effort required. The TM shall have no authority to impose additional requirements or to change or delete existing requirements of the contract. Any clarification provided by the TM concerning the work to be performed shall not be construed as a change to the contract. Any changes to the original SOW must be approved in writing by the Contracting Officer prior to work being performed.
