A001 Monthly Progress Report (MPR)

The MPR shall provide a brief narrative status of the technical process and status of each major task effort, any significant technical or task-specific problems, and the proposed resolution of the identified problem areas. The MPR shall identify any items that have affected or will affect schedule performance, cost, or task schedules. The MPR shall provide a status of materials, supplies, subcontracts, Government Furnished Materials or Equipment (GFM/GFE), and Contract Data Requirements List (CDRL) items for the duration of the effort. The reports shall be in the Contractor’s format as approved by the COR. Submission in an electronic format compatible with MS-Office or in pdf is acceptable. The MPR shall be delivered by the tenth (10th) day of each month for the preceding month.

Blocks 11 & 12 - 10 days after end of first full calendar month after Contract Award.

Block 13 - 10 days after the end of each subsequent month.

A002 Monthly Financial Summary Report (MFSR)

The MFSR shall include the amounts funded and the expenditures broken down and sorted by task, and by CLIN, and by WBS for Special tasks. The MFSR shall show for each Contractor employee and/or sub-Contractor the number of hours the labor cost and fully loaded cost (with overhead, G&A, and fee). Details for the current period and total accrued cost shall be given. Expenses (e.g., travel, materials, and specially requested training) shall be reported separately for each Contractor employee and/or sub-Contractor showing the overhead, G&A, and fee costs. If a Contractor employee and/or sub-Contractor worked on more than one task, then the accounting for each task shall be shown separately. The MFSR shall include a listing, including costs, of all materials, services, and supplies procured by the Contractor. The report shall be in the Contractor’s format as approved by the COR. Submission in MS-Excel electronic format is required. The MFSR shall be delivered by the fifteenth (15th) day of each month for the preceding month.

The Contractor shall provide an OLR by the fifth (5th) workday of each month for the preceding month. The report shall include the following for each Contractor employee and sub-Contractor: first and last name, FY, reporting period, labor hours worked on-site at NRL and offsite, NRL code for the task, task name and CLIN, and costs for labor, travel, material, and ODC. If an employee and/or sub Contractor worked on more than one task (and CLIN) then the accounting for each shall be shown separately. The report shall be in an electronic format compatible with MS-Office and designated by the COR.

Scientific and Technical Reports, including a Contract Final Report, will be prepared as directed by the COR. The Contract Final Report shall be delivered within sixty (60) days from contract completion. A Contract Final Report is defined as a scientific or technical report which summarizes all work accomplished under the Contract. Reports shall be prepared, as required by the COR, according to the guidelines of ANSI/NISO Z39.18-1995, Scientific and Technical Reports-Elements, Organization, and Design. Distribution of resultant documents to the Defense Technical Information Center (DTIC) shall be through NRL distribution channels. Reprints of published articles may be accepted as technical reports with the concurrence of the COR.

The Contractor shall provide Technical Documentation on COR request that shall include: engineering logbooks, designs, schematics, requirements documentation, procedures, systems block diagrams, field integration documents, interface control documents, as-built drawings, contamination, cleanliness, quality assurance, and test results.

In-code documentation shall be provided for all software. The in-code documentation shall include instructions for the user; in particular the locations of needed data files. A separate User’s Guide shall be produced for interactive programs having many options. All software will be provided as files on NRL computers, and must be able to run on these designated computers. Hardware produced or modified under this statement of work shall be accompanied by documentation for maintenance and operation.

For hardware and software developed or modified, the Contractor shall provide requirements documentation, interface requirements documentation, design documentation, test plans, test reports, and configuration management documentation.

Block 12 - 90 days after completion of task or end of period of performance, whichever is first.

Block 13 - 2 weeks after notification from the COR.
