G. Community Transformation Grants (CTG) – Capacity Building Awardees: Guidance for Proposing Pilot Implementation Activities in Year 2

The CTG funding opportunity announcement (CDC-RFA-DP11-1103PPHF11) offers CTG Capacity Building awardees the opportunity to “pilot one or more implementation activities with their awarded funds, as their capacity increases and resources allow.” Awardees should use their base funds for these pilot projects; should additional funds become available, additional pilot projects could be funded with supplemental funding that is not greater than 25% of year 1 base awards.

Pilot Project Conditions:

1. As part of the Year 2 Continuation Application, CTG Capacity Building awardees should NOT request additional funding to transition to Implementation Status.

2. CTG Capacity Building awardees may propose Pilot Projects within their existing budget allocation/base award.

3. As part of the Year 2 Continuation Application of the current CTG Project Period, CTG Capacity Building awardees may request supplemental funding – up to 25% of their Year 1 base award- for Pilot Projects only.

4. Any increase in funding level for Year 2 will only be for one budget period; there is no expectation that an increase for the Year 2 budget period will become part of the awardees’ “base” award.

5. No more than TWO Pilot Projects may be proposed per Capacity Building awardee.

6. Pilot Projects should not begin until the start of the Year 2 CTG Budget Period. The projects should not be initiated until a community health assessment and policy scan have been completed and approved.

7. All proposed Pilot Projects must meet the following “CDC Requirements for CTG Pilot Projects” in order to be considered for approval.

8. Approval of Pilot Projects is dependent upon awardee performance in Year 1 and the amount of funds available.

CDC’s Requirements for CTG Pilot Projects:

1. CTG Pilot Projects should be designed to help achieve one or more of the following Project Period Objectives:

a. Tobacco Free Living- Increase the number of people with access to smoke-free or tobacco-free environments

b. Active Living- Increase the number of people with access to environments with healthy food or beverage options

c. Healthy Eating- Increase the number of people with access to physical activity opportunities

d. High Impact Quality Clinical and Other Preventive Services- Increase the number of people covered by systems or opportunities that support control of high blood pressure and of high cholesterol

2. Pilot Projects should be designed with a “Start Date” no earlier than September 30, 2012 and an “End Date” of no later than September 29, 2013

3. Pilot Projects must be proposed on the most recent version of the Community Transformation Implementation Plan (CTIP)

4. Pilot Projects must implement only evidenced-based strategies

5. Pilot Projects should have a “Population Focus” on a “Disparate Population”

6. Pilot Projects must have a plan for measuring the proposed outcome

7. Pilot Project proposals must include data to justify why the particular community and intervention were selected for the Pilot Project; this data can be from a community health needs assessment (CHNA)and policy scan, OR other local data presented with the project proposal(s) demonstrating the need for the Pilot Project

8. Pilot Project proposals must be submitted as part of the Year 2 continuation application; proposed Year 2 budgets should identify any funding that will be allocated to Pilot Projects

9. For Pilot Project proposals that request supplemental funding, a Pilot Project budget and a budget justification must be provided that is separate from the “base” budget and justification; the requested supplemental funding amount cannot be greater than 25% of the Year 1 award. Include the proposed amounts for the supplemental budget on the same 424A form that is used for the base budget, but list in a separate line or column as directed. For the supplemental funding, complete only Sections “A” and “B” on the 424A form; under “Grant Program Function or Activity” use the title “CTG Pilot Project.” Use the same “Catalog of Federal Domestic Assistance Number” as is used for the base budget.

10. Pilot Project CTIP(s) should be included as part of the document package that is uploaded under “5. Additional Program requirement” – “D. Community Transformation Plan.”
