DESCRIPTION OF SERVICE (DOS)
FOR
INSPECTION, MAINTENANCE, AND CERTIFICATION
OF
OVERHEAD HOISTS, CRANES AND ASSOCIATED HOOKS
SELFRIDGE AIR NATIONAL GUARD BASE, MICHIGAN (ANGB)
1. Description of Service:
1.1 Scope. The contractor shall provide all personnel, equipment, tools, materials, supervision, and quality control necessary to perform on-site annual inspections, testing, load testing, and provide certifications for all inspections and tests performed on all hoists, cranes and associated hooks listed in Technical Exhibits 3 and 4, located on Selfridge ANGB, Michigan.
1.2 Background. The Government has a requirement for the performance of on-site annual inspections, testing, load testing, and provide certifications for all inspections and tests performed on all hoists, cranes and associated hooks.
1.3 Period of Performance (PoP). Services to include inspection, maintenance and certifications shall be completed annually from 1 Apr 2022 -31 March 2023 and four option years thereafter.

2.0 General
2.1 Base Access/Security. The award of this contract does not create a right to access any military installation.
The contractor shall appoint a single individual to coordinate base access with the Government Point of Contact
(POC) for all contractor employees, subcontractor employees, and delivery drivers. Prior to access to this installation the contractor shall coordinate with the POC, no later than 10 business days prior to commencement of work.
2.1.1 Unscheduled gate closures by the Security Police may occur at any time causing all personnel entering or exiting a closed installation to experience a delay. This cannot be predicted or prevented. Contractors are not compensated for unexpected closures or delays.
2.1.2 The contractor’s employees shall comply with all installation traffic, safety and security regulations.
Vehicles operated by contractor personnel are subject to search pursuant to applicable regulations. Any moving violation of any applicable motor vehicle regulation may result in the termination of the contractor employee’s installation driving privileges. All contractor employees shall carry proper identification with them at all times while on Selfridge ANGB.
2.1.3 Antiterrorism / Operations Security: Access and General Protection/Security Policy and Procedures.
Should the Force Protection Condition (FPCON) change, the Government may require changes in contractor security matters or processes.
2.1.4 For Contractors that do not require CAC, but require access to a DoD Facility and/or Installation.
Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening
Database (TSDB) (AFI 31-101 and AFI 10-245), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative), or, at
OCONUS locations, in accordance with status of forces agreements and other theater regulations.
2.1.5 COMSEC/IT Security. All communications with DoD organizations are subject to communications security (COMSEC) review. All telephone communications networks are continually subject to intercept by unfriendly intelligence organizations. DoD has authorized the military departments to conduct COMSEC monitoring and recording of telephone calls originating from, or terminating at, DoD organizations. Therefore, the contractor is advised that any time contractors place or receive a call they are subject to COMSEC procedures. The contractor shall ensure wide and frequent dissemination of the above information to all employees dealing with DoD information.
2.1.6 AT Level I Training. Contractor employees with an area of performance within a Government controlled installation, facility or area. All contractor employees, to include subcontractor employees, requiring access to
Government Installations, Facilities and Controlled Access areas shall complete AT Level I awareness training within 10 calendar days after contract start date. Documentation of completion shall be provided to the Contract
Administrator and POC.
2.2 Physical Security. The contractor shall ensure all government equipment and materials in their possession are secured.
2.3 Safety/Environmental: The contractor shall comply with all federal and state Occupational Safety and
Health Administration (OSHA) requirements and standards, technical orders, regulations, and referenced publications as published in the OSHA 29 CFR.
2.3.1 Accident prevention and safety practices on contractual work under the jurisdiction of the KO are the responsibilities of the contractor concerned. The contractor shall brief all employees on proper safety and accident reporting. The contractor shall provide all occupational health services to his employees. Contractor employees shall be instructed to notify the contractor’s Contract Manager of potential or existing occupational health hazards that require attention.
2.3.2 Liability. The contractor hereby agrees to release the Government (to include its officer, enlisted personnel, agents, and employees) from any liability for any loss, damage, or injury sustained by the contractor or his employees during the performance of this contract. The contractor also agrees to indemnify the Government for any loss, damage, or injury to Government personnel or agents or other third parties, provided such loss to the
Government is caused by the negligence of the contractor or his personnel while performing this contract.
2.4 Quality Control (QC): QC is the sole responsibility of the contractor.
2.4.1 Quality Assurance (QA). The Government shall evaluate the contractor’s performance under this contract in accordance with this DOS.
2.4.2 Government remedies. The Contracting Officer will follow the requirements of Federal Acquisition
Regulations (FAR) 52.246-4, Inspection of Services – Fixed-Price, for contractor’s failure to correct non-conforming services. In the event that services fail to conform to contract requirements, and cannot be remedied by re-performance, the Government reserves the right to reduce the contract price to reflect the reduced value of the non-conforming services.
2.4.2 The contractor shall ensure that timely and effective corrective action is obtained for all deficiencies identified by the Government.
3.0 Government Property, Material, Equipment and Services: The Government will not provide GFP/M/E/S to the contractor.
4.0 Contractor Furnished Property, Materials, and Equipment (CFP/M/E): The contractor shall furnish everything required to perform these services as indicated in Section 1.1.
4.1 Vehicles. The Government will not provide vehicles for use on this contract. The vehicle shall be shall be maintained in a safe and serviceable condition, and shall present a professional appearance at all times. Vehicles found to be unsafe or unable to function as designed shall be removed from the installation and replaced at the contractor’s expense. Vehicle repairs shall not be accomplished on base. All vehicles shall be registered, licensed, and insured at all times.
4.1.1 Contractor vehicles shall have the company name prominently displayed on both sides of the vehicle and be maintained clean to present a neat, professional appearance
4.2 The contractor, at no additional cost to the Government, shall supply consumable items required to perform services as outlined in this PWS. The contractor shall include, but is not limited to cleaning, oiling, and adjusting of parts and equipment when required to perform the inspection or testing of units.
4.3 Contractor will provide Crane Inspectors Certifications for all technicians.
4.4 Personal Protection Equipment (PPE). The contractor shall provide all required personal protection equipment
IAW Occupational Safety and Health Act (OSHA) standards, other than Government-Furnished items listed in the contract. The contractor shall perform the requirements of this PWS using the Code of Federal Regulations (CFR), and are responsible for compliance with OSHA standards, and the protection of their employees. The contractor shall provide all employees sufficient PPE for performing all tasks and shall ensure employees utilize the PPE at all times.
5.0 Requirements: The contractor shall perform the services listed below:
5.1. Provide all supervision, personnel, equipment, tools, supplies, and services necessary to perform on-site annual inspections, testing, load testing, and provide certifications for all inspections and tests performed on all hoists, cranes and associated hooks listed in Technical Exhibits 3 and 4, located on Selfridge Air National Guard Base
(ANGB), Michigan. The contractor shall schedule, perform and complete all inspections beginning in the month of
April until completion of all listed cranes unless authorized by the POC.
5.1.2. Inspect, test, and load test all listed cranes, hoists, and associated hooks IAW the manufacturer’s specifications, requirements, and recommendation, and shall, unless otherwise noted in this PWS, adhere to all applicable standards of the latest edition of: MIOSHA GENERAL INDUSTRY STANDARDS PART 18:
OVERHEAD AND GANTRY CRANES.
5.1.3 The contractor shall, in coordination with the POC, prepare and submit a proposed scheduled to the
Contracting Office for approval of all required inspections and test. Any changes to the schedule shall be coordinated through the Contracting Office. All inspections are due within 365 days of prior inspection.

5.2 INSPECTIONS, TESTING, AND MAINTENANCE
5.2.1. The contractor shall load test in the range of not less than 100% or more than 125% of the rated capacity of all equipment and associated hooks listed in Technical Exhibit 3 and 4.
5.2.2. The contractor SHALL NOT perform or schedule any repairs or maintenance that will result in an increased cost to this agreement. This agreement is strictly for the inspection, testing and certification of hoists, cranes, and associated hooks. The contractor shall notify the POC of any non-certifiable equipment at the time of discovery.
All authorized repairs shall be in accordance with manufacturer.
5.3. REPORTS
5.3.1. Upon completion of each inspection, test, and load test the contractor shall complete a comprehensive written report for each crane, hoist, and hook signed by a Qualified Inspector (as defined in MIOSHA General
Industry Standards Part 18. Overhead and Gantry Cranes), identifying the Reporting Company, the date of inspection, the name of the inspector/tester, the building number, crane type, serial or unit number and capacity, hoist type, serial number, unit number, and capacity, hook manufacturer and serial number and of all items checked, inspected, cleaned, lubricated, measured, tested, and load tested.
5.3.2. The finished report shall be submitted to the Government Representative in triplicate: two written copies and one electric copy on a disk media (CD-R or DVD). The signed reports shall certify to all parties involved the hoist is in proper working condition, properly labeled, and safe to operate within the rated capacity. All documentation shall be completed and delivered within 30 days of the inspection date.
5.4. TAGGING OF EQUIPMENT
5.4.1. The contractor shall at the time of inspection and certification, affix on all certified hoists, in a manner not to interfere with operations, a commercially acceptable, non-removal-without-destroying punched plastic tag, as a certification label displaying the following information: Initial Inspection Date, Load Test Date, Make, Model, Serial Number, Capacity, and Inspector’s Name.
5.5. EQUIPMENT ADDITIONS/ DELETIONS
5.5.1. The contractor’s compensation shall be limited, without exception, to work authorized by this contract on equipment listed in Technical Exhibits 3 and 4. Only the KO has the authority to authorize any changes, additions or deletions to this PWS. All PWS changes shall be incorporated with a modification.
5.6. CONTRACTOR CHECK-IN AND REPORTING PROCEDURES
5.6.1. The contractor shall notify and report to a POC prior to commencing work and at the completion of each workday.
5.7. Repair Work Requiring Parts. The contractor shall provide a written estimate to the POC within 48 hours of an inspection or within 24 hours of any service call if replacement parts are required. Repairs shall be accomplished in accordance with normal commercial practices using parts that meet the manufacturer’s original standards and tools specified by the crane manufacturer. The contractor shall notify the POC with an expected back in service date and time when downtimes are expected to be greater than 24 hours.
5.8. Reporting Condition Changes and Safety Concerns. The contractor shall report any change in condition or malfunction due to abuse, misuse, vandalism, or functionality of crane prior to leaving installation.
