About this file
This document is Addendum #2 to the Invitation to Bid (ITB) DOH22-015 for the State of Florida Department of Health's Sentinel Chicken Program. The purpose of this ITB is for the Department to obtain competitive prices for establishing, maintaining, and routinely sampling eight coop sites of sentinel chickens for their Arbovirus Surveillance Program. The addendum provides responses to questions received by the stated deadline, including clarifications on submitting documentation for coop sites, local business tax receipt requirements, and employment of local staff. The timeline for this ITB has been revised in prior addenda.
The ITB requires all information requested, but allows for submission of both an original and a redacted copy to protect proprietary information. There are no local business tax receipt or local employee requirements, but employees must be from Palm Beach County or adjoining counties. Pricing terms, set asides, incumbents, funding sources, and other salient commercial details are not provided in this addendum, which focuses on clarifying the technical and administrative requirements of the solicitation.

No, all required information must be submitted with the bid. However, the Respondent is able to submit both an original copy for the Department’s use and a redacted copy which would be used in the event of a public records request. The original copy should contain all information requested in the ITB. See section 3.4, Public Records and Trade Secrets, of the ITB.
Does the Department of Health require the company bidding have a current local business tax receipt in Palm
Beach County or a bordering County (Broward County, Martin County, or Hendry County) with the same name where the office is located?
No.
Must the vendor provide operations with local employees from Palm Beach County to service the Department's
Sentinel Chicken Program? Hiring local employees offers opportunities to the Palm Beach County community.
No, see section 3.10.9 of the ITB. Employees must be from Palm Beach County or one of the adjoining counties.
Failure to file a protest within the time prescribed in section 120.57(3), Florida Statutes, or failure to post the bond or other security required by law within the time allowed for filing a bond shall constitute a waiver of proceedings under Chapter 120, Florida Statutes.
