About this file

This document is an addendum to a Request for Proposal (RFP) issued by the Florida Department of Health (DOH) for a Children's Hearing Aid Program. The program will assist families with children ages 0-18 who are Florida residents and have been diagnosed with hearing loss, by providing funding for the purchase of hearing devices. The RFP also requires the successful vendor to develop and implement a marketing campaign to educate stakeholders on the program's services, eligibility, and application process. Eligible children must have family incomes at or below 400% of the federal poverty level and cannot be enrolled in Florida's Medicaid or Children's Health Insurance programs.

The addendum provides responses to questions received by the DOH, clarifying that the program covers the full range of hearing devices and related technology, but the vendor will not be able to bill directly for these devices. Instead, the DOH will reimburse the vendor for the approved cost of hearing devices purchased on behalf of eligible children. The addendum also clarifies that there is no defined funding maximum per child, and the required documentation to determine eligibility will be developed as part of the program's start-up plan.

# Question Answer

As per the RFP, we are under the impression that this contract covers pediatric hearing aids, including BAHA with soft-band, assistive listening technology such as FM systems and Roger systems, cochlear implant processors and any needed parts such as coils, mic covers, batteries, cables, controllers, headsets, ear hooks, repairs to the processor, magnet, and headpiece, and hearing aid repairs. Is this understanding correct?

Correct.

Can the provider bill for hearing devices in the grant?

No. See Attachment A, Scope of Services, Section B.1.a., Task 12).

The Provider will be reimbursed by the Department for approved hearing aid devices purchased by audiologist and prescribers.

If a patient has private insurance, will the DOH allow partial payments for services for the remainder the private insurance does not cover?

No, the Department is not paying for direct client services under this RFP. The Department is only paying the Provider’s administrative costs and reimbursing for the cost of hearing devices. See page 28 of Attachment A, Scope of Services, and page 30, Attachment B, Cost Proposal.

On Page 22, Section B1, what is the definition of

“technology”?

See sections A. and B. above. All references to “technology” have been deleted from this RFP.

How often is a patient eligible for assistance under this program? Our impression is every two years. Is the fund per child available for repairs and ear molds until the

$3,500 is exhausted?

The eligibility determination will be developed as part of the Services

Start-up Plan described in Attachment A, Scope of Services, Section

B.1.a. Task 2). There is not a funding maximum per patient referenced in the RFP.

If the patient does not have medical insurance at all, what documentation is required to provide assistance to the family, as there is no denial or EOB from an insurance company?

Required documentation to determine eligibility will be developed as part of the Services Start-up Plan described in Attachment A, Scope of Services, section B.1.a., Task 2).

Failure to file a protest within the time prescribed in section 120.57(3), Florida Statutes, or failure to post the bond or other security required by law within the time allowed for filing a bond shall constitute a waiver of proceedings under Chapter 120, Florida Statutes.
