PAST PERFORMANCE QUESTIONNAIRE

The purpose of this questionnaire is to obtain an evaluation of past performance on a contract that has been completed or is currently being performed by the Contractor listed above. You have received this questionnaire because the Contractor selected you to provide this evaluation in response to a request by the USPTO for past performance information. USPTO will use the information you provide to assess the confidence level associated with the Contractor's ability to perform the requirements of a contract to provide full classification services. The information is to be provided directly to the USPTO. The information you provide may be released to the Contractor during debriefings however the source of the information will not be released.

It is requested that the Program Manager or their Contracting Officer Representative complete the evaluation. Please complete this past performance questionnaire by answering the questions below and submitting your responses by email to claire.rotruck@uspto.gov no later than September 15, 2016 at 11:00am EST.

3 = Outstanding Contractor’s performance exceeded customer expectations and was technically acceptable, providing significant features or benefits.

2 = Satisfactory Contractor met customer expectations or contract requirements and demonstrated an acceptable understanding of the requirements.

1 = Unsatisfactory Contractor’s performance was either marginal or did not meet customer expectations or contract requirements.
