About this file
This is a State of Wisconsin Department of Administration contract template form (DOA-3049, R5/2018) that serves as a standardized contracting document between the State of Wisconsin and prospective contractors. The template is designed to be completed with specific department information, contractor details, and contract specifications, though the specific contracting department and contractor information fields remain blank in this version.
The document establishes the framework for procurement contracts but does not specify particular commodities, services, pricing terms, or contract amounts, as these fields are left to be filled in for individual contract awards. The template includes standard provisions requiring contractors to comply with Wisconsin's non-discrimination policies covering age, race, religion, color, disability, sex, sexual orientation, and national origin in employment practices. For contracts exceeding $50,000, contractors with 50 or more employees must submit a written affirmative action plan within 15 working days of contract award, with compliance oversight provided by the Wisconsin Contract Compliance Program.
